DO NOT use this facility for someone with dementia or cognitive impairment because they are not equipped for this level of care. My mother was there for therapy due to a broken hip from a fall. She has dementia and in the evenings becomes more confused and agitated, got out of bed and fell a few nights after her admission. The facility is not allowed to use bed rails, bed alarms or cushioned pads on the floor. According to the employees themselves, the nurse/patient ratio is 1/21+ the CNA/patient ratio is 1/11-15. They can’t watch dementia patients when they are alone so they park them in the hallway by the nurses station. I spent a lot of time there and never saw a nurse come in to ask my mom how she was, even when she was in the room to care for her roommate. The best nurse mom had was from a staffing agency and not a facility employee. It once took an hour for a CNA to respond to my call for assistance. For my Mom’s safety our family paid privately for 24/7 sitter service, especially after I got COVID. Getting them to return calls was also a big issue, it took repeated requests. The only plus was the therapists. They were excellent.

It seemed like a professional place.
They had activities for people to do, and my overall impression was that, she would have gotten very good care there. They have two sides: a very heavily retirement community and a skilled nursing side.

My wife was at this facility for two months. Granted they were understaffed , and hampered by weekly new Covid cases among patients and staff. Can you imagine a healthcare facility with “help wanted” signs staked into the ground out front?
There were a number of dedicated hard-working individuals who cared and worked themselves ragged. Then again, there were many of those who were obviously just there to pick up a paycheck.
My wife was transported there from Tampa General Hospital. She was not ambulatory because of having no left knee, her leg was restricted by a external stabilizer secured by 4 pins drilled into her bones, 2 in the femur and 2 in the shin. She took Gandy Crossing detailed written instructions from her orthopedic surgeon as well as a quart bottle of Betadine. She was to have the dressings at the base of the pins changed every day and the wound sites cleaned and painted with Betadine each time the dressings were removed. This did not happen. I only saw them do it once when I was there, which was every day. This resulted in the uppermost pin site becoming infected and full of pus, causing a delay in her knee-replacement surgery due to her having a Picc Line at home for 2 months infusing Vancomycin at first and then Daptomycin. Sometime during her stay the bottle of Batadine mysterious disappeared, as well as a number of personal items including clothing
Her surgeon prescribed two 2 mg tablets of Oxycodone every 5 hours for pain. A house physician took it upon themselves to reduce her medication two one every 5 hours. After the facility stopped visitations due to Covid, she would sometimes call me in the middle of the night crying uncontrollably due to the pain. I advised her to “Start screaming! Scream just as loud and as long as you can!” She eventually got her pain meds. The only thing that kept her going was having good roommates, with their own stories to tell, with whom she still maintains friendships.

My mom is staying at Bayshore Pointe Nursing and Rehab Center. The staff is very caring, and the facility is very clean. I’m pleased with the security, which some people don’t like, but they have glass doors. When you walk in and stand in front of the glass doors, the receptionist has to let you in and out. I think that’s great. To get on the elevator, you need a code.
They make sure my mom is not in her room all the time. She goes to the dining room to eat, which at first, she was not happy with, but now I think she’s making friends. The rehab and physical therapy are good. They have a day room where they have a TV. They take very good care of her. She is clean. I had no problems with cleanliness at all. Mom likes the food. Their staff is very caring and very professional.
It’s an older facility, but it’s very clean. It has three levels. It’s not brand new. She’s still covered under Medicare; she hasn’t used up her 100 days. Last year, she was at a different location that was so loud, and the staff would yell and kid around. This place is quiet.

This is shocking and very emotional news to take in but you are not given time to take it in. Once she chose to not treat we were almost immediately given the boot from the hospital and told to find a place for her to go to die. Ybor City was within walking distance of my home and so it was tops on my list for her to go there, site unseen, I put her in there. I am not going to lie, when I walked in with my sisters we did NOT fall in love with the place, it’s not where you want to put your parent to die. But what place is? A good friend of ours said "if your mom is comfortable and safe, she will not see what you see". We kept mom there and comfortable and safe is just the beginning of what we got from this home. EVERY SINGLE person that worked in this home loved on my mom and my sister and me. They hugged us, shared stories with us, advocated for my mom, her medicines were never missed, she was cared for in ways hospice cares for your loved ones. Within one week I had met every person that worked there, cafeteria staff, maintenance… and so on. They all are a big community. Most of the staff have been there for almost 20 years or more! That says a lot. I cannot say enough good things about this place. My mom only lived another week and I am so happy that is where she passed. It might not be glamorous but the love is limitless.

Her days began very well, she ate and was very happy. Slowly, especially after the virus announcement, things began to happen, not only with Mom, but at the nursing home, their surroundings. The level of care went down rather fast, as did my mom. There were requests made by me that were not followed and when her breathing got bad, they did nothing to take care of it. I requested for her to be moved to a hospital for assistance, I asked for her to be placed on a wheelchair for a walk around the nursing home, this didn’t happen either and other requests. It is rather questionable to me since my mother’s death because I saw firsthand how quickly after her breathing problems began, she began to go downhill. Although she was up in age, I know firsthand that she had some health issues, but they were all under control up until she went to hospice, then transferred to the nursing home. Although, I can’t bring her back, to date, I feel that something went really wrong, really fast.

When I talked to them, they said they had a lot of fresh fruits and vegetables, but they don’t. Everything is canned, so that was a little bit of a disappointment. If I could go out and get my own food, it wouldn’t be so bad, but they don’t want me leaving the grounds. The rooms here are fairly small. I’ve been trying to get up every hour or so and take a couple of little steps just to build up my muscles.
Most of the staff are nice. They seem to bend over backward for you. They have a nice dining room. They have several nice conference rooms. I haven’t done any of their activities because I had other priorities. I was supposed to get my haircut today, but I was the only one who signed up for it, and they needed at least five people signed up to get a haircut. They have an activities calendar, and this afternoon they were supposed to have manicures.
The food here is better than in the other place where I was, but still, there’s a lot of it I can’t eat. They have extra grain crackers and I like those. I have to take my medication, so I’ve asked for those crackers to take with the medication. With regards to cleanliness, they should clean up the sidewalks a little bit better because they have a lot of leaves and little twigs that have fallen.

Please, if there are ANY other options, PLEASE take your loved one elsewhere. The place is absolutely filthy. My bedrails were so dirty, I could not grab them to pull myself up in the bed. I’ll chalk the food up to my taste buds. Maybe one of my meds made food taste bad. For the most part, the food looked good! Although 2 mornings they just forgot to bring breakfast. I only found out breakfast had been served, cause they came for dirty dishes. I was there for wound care. Supposed to be changed 2x a day. Once a day was all they could manage, and one day it went for 26 hours before changed. I have no idea what some of the meds they gave me were, cause I don’t speak spanish. I got sick one night in trash. They never changed trash and never brought something for stomach. Apparently the call nurse lights didn’t always work, cause sometimes they came, and sometimes it took 2 hours. When I decided to leave, I signed out, but got no assistance in carrying bags. I walked out myself with 2 carry-on size bags. I left by Uber! I feel sooo bad for the residents, who can only afford these type of places.
