"All Saints creates a calming atmosphere, with its friendly staff unique environment and clean facilities. Covered outside seating provides a visiting area for either friends and family, or the residents to enjoy the day. Large television rooms are available, with room to maneuver with wheelchairs and/or walkers. A community room is adjacent to the dining hall, where bingo, parties and religious services are held. The staff is friendly, knowledgeable and cares about the residents. They remember birthdays, honor the memories of relatives lost and encourage the residents to better themselves through physical therapy. The therapists create physical therapy plans to increase the ability of each resident. This is a clean and safe facility of which I place my full stamp of approval. Knowing my grandmother is in caring environment at this period of her life certainly provides needed comfort. This facility, both a nursing and assisted living facility is an extremely well run home. "

"Signature HealthCARE of Middleburg is a good place. Every room here is a private room, so you don't share a room with anybody else. There is a hall set aside for bariatrics. Half of the facility is for people coming in from surgeries or injuries and are trying to get back on their feet, while the other half of the facility is a residence facility, for residents that are here permanently. They have therapy rooms here (but they can work with the residents in their room if they can't come down), excellent food (they accommodate the diets that are recommended for them by the doctor), and good nursing care (the CNAs are excellent and they're usually right here right away when you call them). They provide church services here, too. They do your laundry and return it. They have a lot of activities here. They have board games, regular games (such as bingo), music, and making jewelry or outfits that you want to make. They provide everything for you that you could want or need."

"Isle Health and Rehab was home to my mother for the past 3 years, the staff and nurses tirelessly cared for Mom throughout her stay. I would visit several days a week and witnessed firsthand their care and compassion in taking care of their residents’ needs. Special thanks go to Ivon for making Mom as comfortable as possible and seeing to her daily needs. Tim, the activities director, through his Paws for Seniors program, arranged interactive visits with puppies and kittens and Janelle, for helping in arranging and encouraging participation in group activities/events, dressing in themed attire to put a smile on resident’s faces and raise Mom’s spirits. I had confidence that the staff and nurses at Isle Health took care of Mom’s needs on the days when I was unable to visit. I am sincerely grateful! "

"I would like to say that they're taking care of my father . He is always very clean. Most of the time that he rings the bell they come soon to attended him. He doesn't call much. The CNA s are nice to us, although they have about 30 patients to take care of. They work very Hard and always come in with a nice attitude. Therapists are good. Nurses answer what you ask them and say what you tell them. We are nice to staff and likewise.. they clean the room every day. They tell the residents to stay on their wheelchairs so they don't fall. The Lady of social services is very helpful. The nurse manager sees that staff do their work and help them too.the room are semiprivate an some primate. They are clean. Have plenty of chairs for family. I don't know about the activities because my father doesn't go because he sleeps a lot."

"This facility has taken great care of mom! I am so pleased that I made the decision to send her here. I am so grateful for the love and care she receives."

"River Garden Hebrew Home is a great place. They've taken excellent care of my wife, but the building is showing its age. The people are very nice. With this many staff, there are a couple of ones that aren't as warm and friendly as the others, but my wife has had excellent timely care. They have quite an outdoor area and a small man-made lake that's full of ducks, geese, and turtles, and you can feed them. It's very peaceful, nice, and beautiful. The food here is very good once you get used to kosher food."

"The person who accommodated us was great. She was the reason that we really considered it. She was really helpful and really nice. The rooms were small, and they had no private rooms. They even had rooms with four people in them. My husband would start out in a room with two other people, but she said that when one comes available with only one other person, she would move him. They take them outdoors and fishing sometimes. They had a dining room, and the residents seemed happy enough. Signature was recommended to me, and once we get him there and if it was working for him, I would recommend it. "

"I have had several friends who lived in Taylor Care Center and stayed there until the end of their lives. Most of their family members said they were so pleased with the care they provide; that's why I chose it for my husband, too. He's been there since January 27, and it's been a good experience. The rooms are kind of drab. I think they just need to freshen them up, but the staff has been excellent, responsive, and professional. In fact, he's getting a manicure right now, so he's really happy. He has had a few meals, and so far he said they tasted pretty good."

"My mom has been placed at West Jacksonville Health. At that time when we were looking for a facility, they were primarily available. They were able to show us that they are very caring. The facility is close to our home and it really worked out for us. The place is very welcoming and always clean. It's very nice and I'm very satisfied with all the medical and other care that she is getting. The staff are very nice and they are good with what they do. Mom is in a room for two and it's pretty good for her. She is happy with the food. The dining area is big and it opens out to a screened porch, it's very nice and appealing. With their activities, they have the normal stuff like bingo, entertainment, religious services; pretty varied. "

"My 85 year old mother was admitted for recovery and rehab due to total knee placement surgery. She was admitted on Aug 31st. The nursing staff was helpful and willing to answer any and all questions I had. If they didn't know the answer someone got back to me. The nursing staff we had direct contact with were, Deborah, Ranita, Alice, Tiffany and they were just awesome. The rehab staff were just as great. Yasir, Sebrina, Laneeka, & Tonya, the DOR were amazing. You could see the special care they gave my mom and others as well. My mom was there 31 days. I was there everyday and sometimes twice a day. The place is clean and staff is constantly picking up and cleaning up. The rooms are much like a hospital room. There a activities if the patient wants to join in. A small place but I have no complaints. My advice is to take a tour and check it out. I would use again if the need arose. "

"The Lifecare Center of Jacksonville is a blessing to the community. My mother 88 years, an Alzheimer’s/dementia patient has been in this facility for the past 3 months. This center is an outstanding, clean, warm and welcoming place. The rooms, the beds and the patients are always kept clean. The CNAs work very hard, they are so kind to the patients and always willing to do things with a smile. The Nursing crew is also very helpful. Tony, the Nursing Director is the most helpful person I have ever met. Any problems, he is the person to approach! He takes care of each and every patient as a family member! I won’t be doing justice if I don’t mention the friendly office staff right from the the smiling and welcoming receptionists. Iris, the admissions Secretary and Allison, the billing department are so efficient in their paperwork. Everything moves so smooth at this facility, you are at peace trusting your loved ones in their hands. I am so blessed and thankful to each and every person at this facility taking care of my Mom. "

"My grandmother is at Palm Garden of Jacksonville for rehab. When they go to her room for rehab, they don't just let her say no and just sit there. They encourage her to get up and do her rehab. Every time I've gone there, she's been clean and getting her therapy. I'm really impressed by them. The only issue we had was that the telephone and television in her room weren't working. My grandmother has been enjoying the food. The facility is nice and clean. It doesn't have any smells. I think the price is kind of steep for an elderly person who doesn't get much finances, but maybe they can help or get another type of insurance or something to help out with that."

"My mother spent 8 wonderful years at St. CATHERINE'S WHICH SHE ALWAYS IDENTIFIED AS "home".the last 1.5 years were in the memory unit and while my mother had a very social personality, that helped her enjoy beauty shop, meals ,others visiting. I told the nurses and staff that my mother lived as long as she did (89)because of their attentiveness, hugs,care,and love. It may not be newest in physical space, but having moved my mother from GA where the laws for care are horrific ,to here and then 3 times to get to St, Catherine's , I thank God and the Angels and bless friends who helped get her there . Dr.Narvel who cared for her many of those years And all those who adored her made my life easier since I had to continue working, snd couldnt take over the 24 hr care suddenly needed following a stroke. When mother passed they annouced "she wouLd be leving at 10 am" Over the p a, and people formed along the hall paths all the way to take my motherr out the front door,to the portico, the intent being she would go out the door ,the same door she came in,with the hall lined with staff,housekeeper,nurses,admin. Patients, visitors all to bid her farewell and for me --one of the most loving snd breautiful mome nts ,healing and showing the love and care for our family and my beautiful mother. No other facility does this and is just part of the extraordinary loving thoughtfulness that is coupled eith fine medical care. I visited each day and where any of my concerns were always heard, reported back to me as needed, and where they kept my mother so happy And as healthy as possible. I was there every day,so It hurts me to see negative reviews since I know what we went through before St. Catherine's and I am forever appreciative and grateful for it. They are all so remarkable and work hard to make all outstanding and I am forever grateful to them. "
