This place is not the cleanest. It smells. Most times the urine smell is so strong u can barley take it. The staff is not really nice to the patients. They do not keep family informed of what is going on with patient.

This facility is a DUMP!! Please listen to me when I tell you! They let my Mother ROT in there! She suffered for months without any help from the nurses! Anytime I would try to call my Mom it would take weeks for her to call. My Mom unfortunately passed away . DO NOT SEND ANY LOVED ONES TO THIS PLACE! After she passed it took them almost a month to send her check to the Funeral Home. I’ve only been getting the run around from them. There is only 2 staff members that truly care about anything! Ms Lakeisha and Ms Evyonne. As far as the nurses I can’t tell you, I never received any communication from them about my Mom. All the place wants is the MONEY!! EVERY TIME MY MOM WAS TAKEN TO THE HOSPITAL THEY SAID SHE WAS MALNOURISHED AND in other words DIRTY!!

Consulate Health Care of Orange Park was friendly. They had activities and a nice dining room. The rooms were nice.

My mom was in this God Awefull place, the cna's are rude ,my mom got mersa ,she had bed sores, the nursing staff would be better off working with the dead ,specking of death my mother passed in consulate , i as her daughter had to tell her nurse ,there was something wrong with my mom, it took 8 days for urine and blood work ,i was there ,with my brother everyday for months ,on July 29,2015 I was on my way there when i got a phone call , they had just started iv medication ,well it was to late ,my mom passed 7/30/2015 at 12:36 am ,i wouldnt send my dog to that place!!! R.I.P. momma

my mother was there for 10 years and the staff are very rude and the food is very poor they neglected my mother to the point that she has mrsa were her bed sores are because they never rotated I would never recommend any ones loved one to go there its a nightmare!!!

I have been here at Consulate Health of Orange Park for three-and-a-half years. They are terrible, the staff is very rude, the food is very poor, and the help is non-existent. The rooms are very small, and the outside is an accident waiting to happen. I will move to another facility, but one thing that’s keeping me here is the therapy, I love the therapy here.
