About Brookdale Crown Point in Jacksonville, Florida

At Brookdale Crown Point we know that memory losses due to Alzheimer's disease and other forms of dementia are progressive and residents' needs are continually changing. That's why we provide balance in our residents' capabilities, personalities, and preferences to create daily routines that enhance their spirits and increase their sense of purpose. We believe the amount of satisfaction our Alzheimer's and dementia care residents receive from the tasks that fill their days goes a long way to maintaining their sense of purpose and overall satisfaction with life. An integral part of this is our promise to encourage each resident to find Daily Moments of Success. To help our residents achieve these moments, our associates receive the tools and training needed in such key areas as specialized dining and the ongoing monitoring of healthcare and behavioral needs, to name a few. We provide Alzheimer's and dementia care residents with the lifestyle, housing, and services they need in comfortable surroundings combined with the amenities, programs, and care to help stimulate recognition, recall, and wellness in a safe environment. Through years of caring for individuals with dementia, our professional care staff has learned to create optimal surroundings and approaches to care. Familiar environments that our residents have enjoyed throughout their lives are recreated to help them feel reassured and comfortable. The Alzheimer's and dementia care programs at Brookdale Crown Point are built to embrace, not restrict our residents' desire to wander and to provide a sense of belonging. To learn more about this providers license and review other available state reports, please visit: Florida Agency of Health Care Administration

I visited this facility

We're planning to move Mom into Brookdale Crown Point. What I liked is that it is solely focused on memory care. It doesn't have any assisted living residents. We liked the director. We loved the fact that she knows everything about all the residents there and seems to be so involved and caring in their care. We love the grounds, the way the rooms are, the way they have these beautiful walkways and butterfly gardens and nice patio courtyard. Everything looked very clean and well taken care of. It's a very homelike community. It feels like a big family home instead of a nursing home. I loved their menu. They showed us what they have for food for the residents. I liked the fact that the residents seemed truly happy and well taken care of. The maintenance was perfect. The rooms seemed very orderly and clean. The staff seemed amazing. They seemed like they truly cared about the residents, and they took good care of them. They also know everything about them. The residents were gathered in the main room watching a show and doing karaoke. The food looked like food that would be served in a high-end restaurant. We liked the way that the facility was situated. It had nice, wide hallways, common rooms and the actual bedrooms. They all seemed clean and taken care of. The outside area was beautiful, where the residents can go and sit and enjoy the weather. It stood out from the other ones that we visited.

I am a friend or relative of a current/past resident

My father lives at Brookdale Crown Point. The least I like about it is that unfortunately, they have had some staff changes. What I liked about it is the location, the facility and they are trying staff-wise to assist with memory care. The atmosphere at Brookdale Crown Point is very peaceful and quiet. The dining area is very clean and very organized. The size and layout of the room are nice. It's a good space.

I visited this facility

I toured Brookdale Crown Point, and everything was nice. It's a nice facility. The staff looked really happy, and they looked very on purpose. Nobody was trudging along, if you will like they were bored at their job. The place is very clean and as far as appearances, it looked very good. The staff was very nice, very understanding, very caring, and very astute. I have not tried the food, but it smelled good. When I walked in there, they were having lunch. It was an older building. Every other building I've toured has been brand new, but on the other end, it was very well maintained.

I visited this facility

We appreciated the diversity of Brookdale Crown Point's staff and their willingness to talk and sit with us. We liked the very home-like quality of the facility, the fact that they were able to show us the types of rooms that we were looking for, which were private rooms. They would be able to accommodate our particular needs, and we would be able to come and visit my mother when we wanted. We liked that the it was very up to date and that it was small. Overall, we thought it was an excellent choice.

I am a friend or relative of a current/past resident

My mother is a resident at Brookdale Crown Point. She is happy and secure there. The staff is devoted to making sure all residents progress through their days in a positive manner, with their needs met, feeling productive in the moment that they are in. I am so happy, if my mother must live with dementia, that she and our family can live with it with Brookdale Crown Point.

I was very pleased with the care my father received at the facility. In particular, the staff quickly got to know him and what approaches made him comfortable and at ease. I visited him frequently and at various times of the day and night, weekdays and weekends, so I got to see how the facility operated during all of those times. My father also was visited an independent hospice case manager to help my family in ensuring he was receiving good care and to recommend any changes in his care. The staff at Crown Point was very open to any ideas and help suggested by hospice or requested by my family. The staff also made my family members feel welcome and were very accommodating with all of our needs. My thanks go out to all of the staff for making my father's time there as pleasant as possible.

I visited this facility

We visited Brookdale Crown Point (Formerly Clare Bridge of Jacksonville), and we have a friend there. The atmosphere was very nice. They were going through some repairs, but I was impressed with the care and the people there. It's only one storey.

I visited this facility

I was very happy with Brookdale Crown Point. They seemed very kind and caring, the food seemed good, and the activities seemed great, but my grandmother just didn't need memory care yet, and that was pretty much the only reason why we didn't move her there. The dining area was very nice, open laid-out, and there were specific areas for people who could still feed themselves and those who needed assistance, so they kind of have their own spaces, and that was really nice. The rooms were very nice, open, and bright. They had lots of activities and things to keep them occupied and to keep their minds kind of working.

I visited this facility

I found Clare Bridge on the internet and I visited this place. I was looking for a place to stay for this gentleman and Clare Bridge is close to his home. That was an advantage that it has. The place was fine and I had no problem with it. The only problem I had was the size of the rooms and their cost.

Clare Bridge was the first one I visited. I was very impressed with the knowledge of the gal that runs the place. The rooms were very spartan.

Clare Bridge is one of the ones that I personally like. The staff was great and very helpful. If there wasn't anything that hadn't been talked about in the tour they followed it up with calls. They have been accommodating, very thorough, very patient, kind, and compassionate. The facility is also very clean.

I am a friend or relative of a resident

I went to visit my grandma there a few times. I came in and the place was nice. The staf was helpful and polite. It was pretty boring but granny seemed to enjoy it. They had all kinds of activities. Including swimming, games, sports, arts and crafts, and classes. The neighborhood was also a nice place. It wasn't in the inner city. The only thing I didn't like about it was the short visiting hours. They weren't as long as I wanted. Overall good place though. There were no signs of elderly abuse. The food there was okay. It was a little bland for my taste. I would recommend any one I know to this fine establishment.

I visited this facility

The two things that stuck out to me the most were the food and the view. I understand that may be selfish considering I was just visiting this facility, however their services are exceptional and I feel under my situation those were the coolest things. One on the other hand, who is considering living there, might love the friendly staff or spacy rooms. The place was remarkably clean, with activities almost 24/7. The staff works 25/8 and does what ever they can to make your stay as a pleasant as possible. The needs per resident are different which is most likely why they have such a broad staff to serve you!
