My father was just discharged from this facility – he was in the rehab center post hospitalization – and it was nothing short of a nightmare. The level of care and responsiveness of the staff was horrifying. I honestly don’t have it in me to go into deep detail, but do NOT send your loved ones here, unless you’re fairly indifferent on whether or not you see them breathing again after. This facility was recommended by the Mayo Clinic – we trusted them, not knowing better. Do not make the same mistake. Oh, and his wallet "mysteriously" went missing there. Do not – please – make the same mistake we did and send your loved one here.

My mother-in-law is at The Terrace of Jacksonville. She’s in rehab right now, but she will transition over to long-term care. She’s only been in there for a couple of weeks and as of now, the care seems very good. They see about her often. She’s doing good on the rehab side. They’re helping her get a little bit more energy by exercising her and building some muscle, to help her move around and such. The care is really good. The food is good for what she says. It looks like an older facility, but there are always people around. If you need help, the nurses’ station is close. So far, I’m appreciating them.
The staff is very informative and very helpful. They’re helping us with the Medicaid process and getting all that lined up. They’ve been extremely helpful in that regard. They have a courtyard where they can go out and sit, a visitation room, which is a large room where they do a lot of activities, bingo, arts and crafts, and things like that. They have quite a bit of activities. She’s given a monthly calendar of activities. They have a Sunday devotional that she does attend. River Garden was negligent in their care for my mother. She was admitted for rehabilitation after falling getting into her bed and laid for several hours while she was living at The Coves. While under their care, they glazed over a SLUMs test they administered in a conference call we had with them as her family. This test indicated my mother was suffering from full on dementia at a score of 13 out of 30. Knowing her cognition was this far gone they proceeded with a hard sales pitch with her to keep her in their Long Term Care facility. They refused to take care of her hygiene needs and as a result she transferred to another facility with rashes and sores. They refused to order her a hospital bed after our decision to move her to another facility. They indicated she didn’t need one and we should buy her one. She had repeated problems transferring from a wheelchair to the bed at River Garden and this was discussed on conference calls and her file was documented as such. Some of the staff said she did need a medical bed and they had documented her file as such, but the administration said she didn’t need it, they would not try and request one from Medicare and they advised us we could go buy her one. The social worker assigned to her case. as an advocate for her care, stopped participating in conference calls with us. When we asked what they would provide for the Long Term Care they wanted us to transfer her to, they could not answer. The price was over double what every other facility wanted. $13000 seemed outrageous. They did state if she left it would be close to impossible to ever get readmitted. We were never told my mother would need Memory Care or that they could provide it. We have since moved my mother to another facility. She has been PROPERLY evaluated and has been diagnosed with dementia and severe anxiety for which she is receiving care and medications. We as her family have had to make decisions and enact our Durable Power of Attorney to ensure her financial, emotional and care needs are met. Unfortunately River Garden never truly addressed my mothers state of mind and did not prepare us for what we would need to do for her. When they told us we could go buy her a bed it became apparent they did not care about her welfare unless they would benefit financially from it.

The Lifecare Center of Jacksonville is a blessing to the community. My mother 88 years, an Alzheimer’s/dementia patient has been in this facility for the past 3 months. This center is an outstanding, clean, warm and welcoming place. The rooms, the beds and the patients are always kept clean. The CNAs work very hard, they are so kind to the patients and always willing to do things with a smile. The Nursing crew is also very helpful.
Tony, the Nursing Director is the most helpful person I have ever met. Any problems, he is the person to approach! He takes care of each and every patient as a family member!
I won’t be doing justice if I don’t mention the friendly office staff right from the the smiling and welcoming receptionists. Iris, the admissions Secretary and Allison, the billing department are so efficient in their paperwork. Everything moves so smooth at this facility, you are at peace trusting your loved ones in their hands. I am so blessed and thankful to each and every person at this facility taking care of my Mom.

The facility is close to our home and it really worked out for us. The place is very welcoming and always clean. It’s very nice and I’m very satisfied with all the medical and other care that she is getting. The staff are very nice and they are good with what they do. Mom is in a room for two and it’s pretty good for her. She is happy with the food. The dining area is big and it opens out to a screened porch, it’s very nice and appealing. With their activities, they have the normal stuff like bingo, entertainment, religious services; pretty varied.

The staff was outstanding, and the rehab people were outstanding. I just can’t say enough good things over there. The only thing that deterred me from putting him back there was the drive over there. They were outstanding from cleanliness, to staff, decorations, friendliness, and helpfulness. He was on regular food, and he had a physical therapist that came to his room and helped him with swallowing and gave him exercises. They even recognized that he might have a problem down the road.

I witnessed several students from a nearby private Catholic school also partake in activities with several of the residents. The students were singing a wide variety of songs and playing games using balloons with the residents of the facility. I also saw several games set up for the residents such as checkers and parchisi. I also saw several drawings done for the residents which were probably also done by some of the students from the private school. The exterior of the facility seemed very welcoming, and the landscaping on the exterior appeared to be very well attended to. The interior seemed very spacious, especially in the recreation area, which was where I was most of the time. Overall, I feel that this is a good facility where the staff genuinely care about the residents. I would definitely say that it was a favorable environment, although I have not really visited any similar facilities anywhere before.

They even had rooms with four people in them. My husband would start out in a room with two other people, but she said that when one comes available with only one other person, she would move him. They take them outdoors and fishing sometimes. They had a dining room, and the residents seemed happy enough. I waited 2 hours to have my bedpan changed because I couldn’t get out of bed. Some of the nurses are mean and nasty and 90% of them are lazy and don’t want to change the bed sheets or bandages. They speak down to you and when you get a nurse to finally come in your room, they tell you they’ll be right back and don’t come back. I don’t recommend this place at all. He’s been at Regents for months and it has been a nightmare. As you will read over and over again, getting anyone on the phone to discuss care or general well being is a joke. While I do not feel he’s being abused, I do not feel as though his care is remotely a priority. They are short staffed, which is understandable given the state of the world. My room was nice, it was kept clean, I made my own bed (out of habit). I had plenty of room for my belongings, my laundry was done by staff. My interactions with the staff were positive, I made many friends among the staff. The service was good, my needs were met, My therapy was well taken care of. The food was good and suited my tastes. I did not avail myself of te activities, I kept myself engaged. All in all, I had a satisfactory experience!

The staff at Lanier Rehabilitation Center seem to be very friendly and were taking good care of my dad. He was first in quarantine, so he was alone in his room; he was happier then. Now that they moved him, he has had two different roommates; he was not happy with either one. The first one wasn’t friendly, and the second one seems to be mentally disturbed. From what I could tell, it’s not very good for somebody that’s just there for rehab.
They had no activities, no common interest things that I can tell. Daddy was very bored because there was nothing for him to do. He’s very unhappy, and he’s getting more and more depressed every day. He’s not happy with the food because they won’t give him regular liquids. Everything has to be thickened, and he doesn’t like the way it tastes. They puree his food, and he’d rather nor puree his food; they’re just going according to the dietician’s recommendations.
The place looks nice and well-kept; it has trees and flowers. As far as I could tell, it has good parking. He told me that he’d have to wait for hours before somebody would come and put him in bed or take him out of bed. The staff that I’d talked to were very kind, and they’re professional. From what I was able to see, the place looked clean enough.

The staff at Lanier Rehabilitation Center seem to be very friendly and were taking good care of my dad. He was first in quarantine, so he was alone in his room; he was happier then. Now that they moved him, he has had two different roommates; he was not happy with either one. The first one wasn’t friendly, and the second one seems to be mentally disturbed. I would like to thank the Staff of this facility for the loving care and kindness they gave to my family member while she was there for rehabilitation 3 different times within a 2 year time frame. I have had a Mother, Aunt, Aunt in Law and several friends whose family have been there and have nothing but good things to say about this facility. While the location is a turn off for most people, I found the facility to be clean, comfortable and very homelike. The staff was exceptional and were always courteous, kind and helpful to my Mother and all of us when we visited. I would highly recommend this facility to anyone.
