Life Care Center of Jacksonville took my husband when he was turned down by other people because he has true celiac disease, and it required a special diet. Their dietitian did a fantastic job and even made him some desserts herself. They even changed the bouillon. He was there for 29 days. They had to literally teach him how to feed himself and teach him how to walk again. He had forgotten everything. Because he was a two-person transfer, celiac was the big thing, but they did a fantastic job. He did not get sick a single time. The staff just didn't always come when we called. My husband improved to the point that they told me he had to come home or he had to be registered as a full time because this was all Medicare type thing, so I opted to bring him home and we're doing fine. The food, in general, for being celiac and everything, he never complained and he ate everything. The dietitian over there went above and beyond. The facilities were great. Rehab was wonderful. He had a private room, and it was nice and clean.

The Lifecare Center of Jacksonville is a blessing to the community. My mother 88 years, an Alzheimer’s/dementia patient has been in this facility for the past 3 months. This center is an outstanding, clean, warm and welcoming place. The rooms, the beds and the patients are always kept clean. The CNAs work very hard, they are so kind to the patients and always willing to do things with a smile. The Nursing crew is also very helpful. Tony, the Nursing Director is the most helpful person I have ever met. Any problems, he is the person to approach! He takes care of each and every patient as a family member! I won’t be doing justice if I don’t mention the friendly office staff right from the the smiling and welcoming receptionists. Iris, the admissions Secretary and Allison, the billing department are so efficient in their paperwork. Everything moves so smooth at this facility, you are at peace trusting your loved ones in their hands. I am so blessed and thankful to each and every person at this facility taking care of my Mom.

This Skilled Nursing Facility is a blessing to the community. This is our second bout with my Mom’s rehab. With the most recent change of management, Tony - Director of Nursing and Marc, the Executive Director, they have turned this building around and made it a second home for my mom and our entire family! Every healthcare building is short staffed right now but this facility is a cut above the other nursing home because of the customer service! There are no perfect facility but this building has plenty of caring CNAs, Nurses, and therapist. My mom is a very challenging, strong-willed woman and she’s been to a couple of nursing homes for rehab and by far this facility worked with her and Mom was able to be back home in a reasonable amount of days. The staff here knows how to push patients to promote holistic healing and highest quality of living. Kudos to CNAs like, Cynthia, Cherish, Terra, Curtis, Geeno, Leni, and Taija! Nurses like Stephanie, Janet, Erwin, Marsha, Carole, Katie, Irene, Allison. Therapist like Hazel, Grace, RJ and Matt makes therapy productive for my Mom! I’m sorry to the staff that I forgot their names, and it took a while for me to be able to do the review. I’m still very thankful for ALL That YOU DO for my mom and us, her family! God bless!

This place is not perfect but people here do care. Kevin, the director is a really good advocate for this facility. Jane, social services is also. Tony is very knowledgeable, the most helpful person whenever someone is in need and can't figure something out, find Tony, RN. Most of the CNAs here works very hard, even the healthcare community don't pay them enough for all the care and compassion they have for patients. Cynthia is a great CNA! They do the best they can! We need to understand that your loved ones are not the only patient these people have to care for, no issues or problem that you can't figure out with a good conversation.

In my opinion, the front office staff is very helpful to our family. The nurses are very diligent in there care of my brother Seth. The activities staff. Liz, Dominique, Tasha, Teri are very helpful in their care of Seth. The nursing staff, beginning with the head nurse ,Angela the RN coordinator , Tony and all the nurses whose care he was under , in my opinion is taking good care of my brother Seth. As of this time I am very pleased with his care.

My father is a long term-care patient in this facility, he has been a resident there for almost a year and a half now and he loves it there! This facility is far from perfect but most of the staff up to its leadership, they do care for my dad and elderly. Realistically speaking there's no nursing home out there that doesn't have problems, specially with this time of pandemic, family members should understand that it is also their responsibility to help out their loved-ones that is residing in here. This was recommended by one of my colleague and this place has been a blessing to me, my whole family and my dad! My father originally came here for rehab and boy the therapists here are really great at what their doing, they know the appropriate time when to push my dad to get stronger faster. Kudos to RJ, Hazel, Grace and Matt! My dad and I was already looking for a great place where he can get the help he needs on a daily basis, and he connected with the staff here. Thank God for nurses like Marsha, Tony; the dietitian Jena, Teri and the director and head of nursing department (sorry, I forgot their names and some other). They treat my dad as family. One thing that really impress me besides the quality of care is that my dad and I noticed that the staff always pray as a team every day. The staff here has been through a lot because of this pandemic, but they are still there for my dad and others day in and day out. I, truly am very thankful for this facility! They are a blessing in this community..

I visited Life Care Center Of Jacksonville, but they have a waiting list. The place is pretty nice, although they need more staff members just like every place. It's a newer place with very clean amenities. They're mostly a rehab. There's very few rooms, like only around five. I tried their food when my in-laws were in there, and I would rate the food a four. They even have two cute ice cream parlors; it was the cutest thing.

I recommend Life Care Center of Jacksonville. The facility is very nice and clean. They have nice equipment to help there patient. They have never ran out of planning. The food there is great. The nurses and aides are very polite and helpful. The only problems I seen was the lack of staff. And some of the management do not know how to speak to employees

Therapy was amazing. The facility is beautiful. Those are the only positives I can say. The nursing staff was a joke. If you ask for pain meds, be prepared to wait up to 2 hours if you get them at all. There's also a huge disparity in care in the different parts of the facility. Antigua was good and had decent nurses and CNA who care and were on top of things. If they try to move you to Bermuda or put you in Bermuda just offer to lay in a vacant parking space in the lot. You'll get just as much care there.

My mother was in Life Care Center of Jacksonville. I wasn't happy with the nursing and the nursing aides department. The therapy department was good, but the other part where the nurses take care of you and the assistants come and change, they were not very good. They didn't have enough staff to take care of your patient. The food wasn't any good there either. We were not happy with that place overall. The only thing we were happy with was the therapy. They need to hire more help and be more attentive to the patients. The rooms and the facility itself were nice. Looking at the facility, being in the facility and the cleanliness of the facility were all OK.

My mom was there 5 times. Only had 2 occasions with an issue. One, mom called for her pain med and the nurse intentionally waited to give her meds last. Second, the aid was nasty. Both times made sure these ladies never went near my mom again.. With that said place as good. I made sure the nurses and aids were on top of my mom. I was there every day after work and made sure she had what she needed.

There were a few good nurses. There were more who didn't seem to care. My husband was ignored, pain meds late to very late many times, found him dirty and cleaned him off myself. First night, he had no call button, and when he called me on the phone it took me hours to have his problem checked and dealt with, and he did not have his pain meds for over 9 hours. I will never allow him to be placed in there again. They answered my pleas, made promises, but up until the his last day there were unable to change things. I suggest they try to find better nurses and CNAs.

The facility was very clean. But when you call the nurses only the CNA's came who would asked what was needed and would tell that the nurses were busy and would come in a minute and sometimes it took them a long time. They asked my mom if she wanted to listen to music and when she said no, they never came back. I thought the most important thing that they needed to improve on was to have more staff.

Life Care is unlike any facility in which I've ever visited. The staff remembers each patient by name, and remembers their family members. They truly care about the needs of each individual patient and are willing to provide personalized training to ensure that the patients' needs are met once they return home. My grandfather raved over the food. It was far beyond the typical hospital and/or assisted living dining room food. The choices were multiple, the quality was fresh and the ability to call the kitchen and ask for something extra was always available. The physical therapists did not just go through the proverbial motions. They wanted each patient to receive the past possible care, whether they were 91 or 51. They created a hard working environment of only 15-20 min breaks, but allowed different muscles to rest while working on a different part of the body. Life Care certainly lives up to its name, as it does create a caring environment for all patients and their families. I believe you can truly rest easy, knowing that your family members are in good hands.

My mother has been a resident of both the Lifecare of Orange Park and the Lifecare of Jacksonville facilities for almost 2 years......Both facilities are meticulously maintained, well-staffed and provide quality care for seniors with significant disabilities.....We are fortunate to have these excellent senior citizen care facilities here in Duval County, Florida.
