"Moultrie Creek Nursing and Rehab Center was a big facility for rehabilitation and physical therapy. The facility was located in a place where it was away from traffic and there were a lot of trees. The entire building was set up for rehabilitation as evidenced by the walkers, wheelchairs and other devices that i saw. It was well-kept, very tidy and did not smell like a typical convalescent center. It looked like the staff actively took care of the residents and there were doctors around also. "

"I am in Emory L Bennett. It is nice, and the staff is very friendly. I haven’t been here that long, but they take care of me pretty good. They also have movies and an exercise room. "

"I am writing in regards to our visit to Coastal Health and Rehabilitation Center. I was with my husband and mother and we all were very impressed. When we arrived, we were greeted by several staff member that were all friendly and eager to assist us in answering the many questions we had for them. My mother noticed right away how immaculate the facility was. My husband learned that they have 6 full time physical therapists on staff. I spoke with the residents and found that they were honestly pleased with their accommodations, treatment and the care takers. We were so amazed that they have an outdoor vegetable and flower garden that the residence care for; as well as the customary outings, art n crafts and holiday celebrations. After touring several facilities, hands down Coastal Health and Rehabilitation Center is the place we will place my mother when she is ready. Thank you all for your help and guidance. "

"My mom went to Indigo Manor in their rehab facility for six days. The place very nice and very clean. It was like a hospital and I was very impressed with it. The staff were wonderful and when we were there, they helped me and my mother. My mom likes the food there. "

"My mom is staying at Solaris HealthCare Daytona and we have no complaints at all. The staff is great, very accommodating, very helpful, and very caring. I love everything and I feel comfortable with her there. Maybe they can have a little bit better food, but other than that, everything is great there and they have a nice dining room. Right now, she shares a room but she can be in a private room if she wishes. They do activities every week with the residents. They do bingo, coloring, crossword, they have a Grandparents' Day, and they have a variety of things. Though the facility is nice, it's outdated and maybe needs a little updating."

"When I parked at The Terrace of Daytona Beach I thought how beautiful the building and landscaping was. As I walked inside I also thought it was very tastefully decorated and spacious. All the residents I noticed when I first walked in seemed to be very happy and either chatting or enjoying a television show or just walking around getting excerise. I was met by a very friendly staff member and she informed me that the residents all are quite happy living at this facility and I agree. Everyone was extremely friendly and happy. I noticed that they offer a wide variety of activities for the residents and also a variety of outings. This facility is located very close to the largest shopping mall in Daytona Beach so everyday they offer a shopping outing to the mall. The menu has a wide variety of items and many specialty diets are also listed. I arrived just before lunch so I was able to smell and actually see the lunch items as they were being prepared. There are walkways outside so the residents can enjoy the fresh air, there are several television rooms, quiet day rooms so the residents can enjoy a good book if they wish and there's even a courtyard and a patio outside for those that like to be outdoors. "

"Family was in there , on call bell for 20-45 minutes, nurses arguing with residents about what meds were given , nurses look like they are all in something "

"I was a resident of GoodSam/Daytona beach in 2016. I was very pleases with my apartment which was located off a beautiful flowered patio. I found the management to be spot on. The manager, Melissa Long was professional, caring and always available to listen and make each day a good day. The assisted living facility is well managed with experienced and caring nurses at all times. The rehabilitation center is first class. The administrator, Lisa Meyers , was always caring and professional and treated each resident with the utmost care and fairness. There is a lovely chapel that is open 24/7 with Christian daily services. The apartment building is called Olds Hall which has an old world charm and welcoming atmosphere. But what sets this facility apart from the others is that the residents are really wonderful and it's so easy to make friends. I immediately felt at home. Take the tour and see for yourself. It's faith based and you can't go wrong. "

"The seasonal planters and flowers are awesome and the clearing of the overgrown shrubs is amazing. It’s exciting to see the positive changes taking place. The barber/beauty salon services is much appreciated by the residents who are unable to go off campus bc of medical restraints. I appreciate the genuine concern given to the residents which many times goes unnoticed. Every time I have asked ANY staff person a question or had a concern I was greeted politely and any issue I thought was an issue was taken care of immediately. Love the new upgraded pavers at the entrance. Job well done!"

"I was recently a resident in rehab after total knee replacement. I can not say enough about the wonderful care I got while in this facility. Day 2 we had to be evacuated to Ocala due to hurricane Matthew. Day 5 we were returned to St Augustine. With all the confusion and extra work this staff had to do i was amazed at how cheerfully they did it. The care was efficient and timely and respectful. Meals were good and the facility was clean. Therapy was cheerful and almost fun. When it was time to leave I was a little sad because I felt like the staff were my friends! I would definitely return here if needed. Thank you to the great staff of St Augustine Health and Rehab!! "

"Upon arrival I felt as though the security was minimal. I was able to walk in a side door in which there was no desk or sign in sheet. I actually had to walk to a nurse's station to ask for a sign in sheet. That made me feel uneasy because that meant that anyone could walk in and walk into a patients room and do only God-knows what to a patient without ever being detected. As far as the staff the appeared to be very friendly. I did not have much interaction with the staff though. My grandmother seemed to enjoy the nightly/weekly events that were offered at the nursing home. She doesnt have any complaints about the food. Another positive seemed to be that there were a variety of activities for the patient to choose from. Another area for improvement for the facility could be the way patients can be contacted or how they contact you. There is no direct line to patient. You have to call a portable phone in which they walk down to the patients room which is really inconvenient to the person calling and the patient "

"BayView Healthcare was pretty, but the cost was prohibitive. My mother has been in rehab there before and they did a very good job. The physical therapists were very good, personable, and made her feel important. They have art activities and groups come in to sing and different things. We didn't have any complaints about the food."

"My wife is presently at Seaside Skilled Nursing and Rehab Center. It is clean but crowded. Their staff members are good but some are not so good. It is a very busy place. I have never seen a nursing home that is so busy."
