"Life Care Center at Wells Crossing was excellent. I had no complaints about staying in that place. The only problem with them was that they don't have very many private rooms, and you have to have a doctor requested. Otherwise, you end up in two to a room. They also have a chef that just loves to put spices and everything and not everybody likes to have spices put into their peas or carrots or even their mashed potatoes. Other than that, the things that they do for their residents were the whole nine yards. They have an activities room and a day room where they do a lot of activities They play bingo, do a lot of trivia, and play mini-games that would help increase your coordination and ability to function. They have reading, movies, and even an ice cream parlor that you can go to on Mondays, Wednesdays, and Fridays. Plus, it's free. Another thing is you can notify the kitchen that you want to have dinner with somebody and they will make an extra plate for that person. So, they encourage the family to participate."

"My husband stayed at Penney Retirement Community and it was excellent. I would highly recommend it. He went in there at night from the hospital because he was discharged late. I got there the next morning and the staff came into the room to personally meet me. They were just kind and considerate as could be and granted our every wish. He got three good meals a day that he really enjoyed, and he got bathed as scheduled. It was really a good place. Plus, they had a good physical therapy department. The pavilion is the building that he was in. There's nursing home, assisted living, care center, memory care, and physical therapy there. He was in physical therapy. It was very good care. There was a lady in his room cleaning every day. He had a big bathroom. It was really, really nice. The staff were popping in quite frequently to see how he was and giving him his medicine or helping him to the bathroom. At that point, he wasn't able to walk because he couldn't even stand. They really helped him in every way he needed. I wasn't there every day, but the food I saw was very good looking. If it wasn't good my husband would say so, but he enjoyed every meal. Medicare and TRICARE took care of it. If you had to pay for it on your own, it's $400 a day. They had transportation for him. The staff listens to you. The place is very clean, and the pavilion is relatively new and very modern. It's a community all by itself. They even have their own post office there."

"It was ridiculous. I was really upset with Governors Creek. I was so disappointed to see my father. He was there for a week, and he had already been sent to the emergency room twice. They were not watching my dad, and I already informed them about his condition. His bed was not safe. There was no alarm, and it had no railing. He had no TV. It was empty. I wanted to send him home that time, but I could not. It's a very old building. I feel bad for those people who were in there. They were not friendly people as well. I've been there today because to take my dad's belongings, and they cannot even find it. They sent me to a room that looked like a mountain of junk belongings of patients. "

"My younger brother started in the hospital and transferred to ARU. The staff was wonderful. PT and OT staff exceptional! He said the food was very good as well. I don’t normally do reviews unless it’s bad but his experience there was excellent. He was there for almost 2 weeks. They established what caused his problem. He was able to bring home what he learned to help him in the future. "

"All Saints creates a calming atmosphere, with its friendly staff unique environment and clean facilities. Covered outside seating provides a visiting area for either friends and family, or the residents to enjoy the day. Large television rooms are available, with room to maneuver with wheelchairs and/or walkers. A community room is adjacent to the dining hall, where bingo, parties and religious services are held. The staff is friendly, knowledgeable and cares about the residents. They remember birthdays, honor the memories of relatives lost and encourage the residents to better themselves through physical therapy. The therapists create physical therapy plans to increase the ability of each resident. This is a clean and safe facility of which I place my full stamp of approval. Knowing my grandmother is in caring environment at this period of her life certainly provides needed comfort. This facility, both a nursing and assisted living facility is an extremely well run home. "

"I would like to say that they're taking care of my father . He is always very clean. Most of the time that he rings the bell they come soon to attended him. He doesn't call much. The CNA s are nice to us, although they have about 30 patients to take care of. They work very Hard and always come in with a nice attitude. Therapists are good. Nurses answer what you ask them and say what you tell them. We are nice to staff and likewise.. they clean the room every day. They tell the residents to stay on their wheelchairs so they don't fall. The Lady of social services is very helpful. The nurse manager sees that staff do their work and help them too.the room are semiprivate an some primate. They are clean. Have plenty of chairs for family. I don't know about the activities because my father doesn't go because he sleeps a lot."

"River Garden Hebrew Home is a great place. They've taken excellent care of my wife, but the building is showing its age. The people are very nice. With this many staff, there are a couple of ones that aren't as warm and friendly as the others, but my wife has had excellent timely care. They have quite an outdoor area and a small man-made lake that's full of ducks, geese, and turtles, and you can feed them. It's very peaceful, nice, and beautiful. The food here is very good once you get used to kosher food."

"The person who accommodated us was great. She was the reason that we really considered it. She was really helpful and really nice. The rooms were small, and they had no private rooms. They even had rooms with four people in them. My husband would start out in a room with two other people, but she said that when one comes available with only one other person, she would move him. They take them outdoors and fishing sometimes. They had a dining room, and the residents seemed happy enough. Signature was recommended to me, and once we get him there and if it was working for him, I would recommend it. "

"My mom has been placed at West Jacksonville Health. At that time when we were looking for a facility, they were primarily available. They were able to show us that they are very caring. The facility is close to our home and it really worked out for us. The place is very welcoming and always clean. It's very nice and I'm very satisfied with all the medical and other care that she is getting. The staff are very nice and they are good with what they do. Mom is in a room for two and it's pretty good for her. She is happy with the food. The dining area is big and it opens out to a screened porch, it's very nice and appealing. With their activities, they have the normal stuff like bingo, entertainment, religious services; pretty varied. "

"My 85 year old mother was admitted for recovery and rehab due to total knee placement surgery. She was admitted on Aug 31st. The nursing staff was helpful and willing to answer any and all questions I had. If they didn't know the answer someone got back to me. The nursing staff we had direct contact with were, Deborah, Ranita, Alice, Tiffany and they were just awesome. The rehab staff were just as great. Yasir, Sebrina, Laneeka, & Tonya, the DOR were amazing. You could see the special care they gave my mom and others as well. My mom was there 31 days. I was there everyday and sometimes twice a day. The place is clean and staff is constantly picking up and cleaning up. The rooms are much like a hospital room. There a activities if the patient wants to join in. A small place but I have no complaints. My advice is to take a tour and check it out. I would use again if the need arose. "

"My sister is in Bartram Crossing. It's a very nice facility. It's very appealing, well-lit, modern, and laid out nicely in terms of the physical layout of the facility. There are single-patient rooms in 90% of the facility, and they're there specifically for the rehab portion of it. They have a small number of beds for longer-term skilled nursing and those are double rooms. My sister has a single room. It's large. It has seating. It's very light. It's not depressing. The staff is attentive in terms of taking care of the patients. My sister is a two-person lift, so they're better equipped to handle her. They're getting her up more often than they did at the other facility so that she doesn't lay in bed and have muscle waste. They are proactive in terms of what her medical needs are. They were the ones who identified that her heart was in afib and sent her back to the hospital. They have bingo, movies, and games. They have a printout of the activities for the day."
