"My brother entered for rehab after breaking some ribs and having blood drained from his lung. He was weak, disoriented, and often not rational. I visited him on a Saturday, noticed he seemed weaker and he was not eating or drinking. Blood pressure checks indicated it was dropping. I mentioned these things to several caretakers. By very early Sunday morning, I received a call that they had left him back at the E.R. By the time I arrived from 70 miles away, the doctors said they did not think he could survive because he was so ill. BP dropped to 40's, vomiting, etc. They treated him and he lived, but still has issues related back to this illness. I know part of the Doctors Lake Healthcare is wonderful, but not the part he was in. They should have been more aware of what was going on with him. "

"As I entered the waiting room area, there was an employee who greeted me and then got me signed in. Within seconds, a nurse and provided a warm welcoming smile and said, Welcome to you today! How are you doing this afternoon? The nurse continued the conversation by saying, How may I help you? When I stated that I was visiting my mom, the nurse said, You got the right place and we welcome your business. Thank you for coming in today. I can take you to her. She is playing Bingo. Would your children like some ice cream first? We were escorted to the ice cream shop. The children were amazed to see these chocolate, frozen, treats! Next stop was Bingo! Mom looked great as she sat around a clean table. The place was hopping. At least fifteen seniors were joyfully placing their chips on their boards as the caller pulled new numbers. The caller spoke in an upbeat, even tone that was easy to understand. Continuing to explore my needs the nurse questioned me by saying, Would you like a tour? The children can help their Grandma while we are gone. The nurse explored my needs by asking multiple open ended question to determine which area to visit first. The nurse used the next few minutes to provide me with a first hand look the features available here. The features included a sparkling clean outdoor area with the view of a pond, a fully staffed exercise/therapy room, and multiple private rooms to spend quality alone time with Mom and the family. This location had multiple wings that featured recently redecorated rooms and a centrally located nurse's station. This nurse stood out positively because she had a can-do attitude and a positive word for the co-workers of this location. This location had adequate nurses to handle the flow of residents and their families. My nurse provided effective communication, service in a timely manner and was able to provide multiple brochures that explained the features of the nursing home. At the end of our conversation, the nurse said goodbye and offered to help me at any point needed during my visit. I was thanked for my visit. The attention to detail and the professionalism shown to me during this interaction were outstanding. This type of service would not make me hesitate to use this location. Mom is very happy here. She has one roommate who is a real joy to be around. This is a top notch facility! Thank you for the great nursing home! "

"I am a resident at vivo and am having a very positive experience here. The staff is friendly and knowledgeable and also very helpful. The food is not the best but it is not bad. Most of the nurses are well above average and add alot of value to the facility. There are many CNA s that are extremely helpful and friendly. The physical therapy department is at least average and well managed by friendly staff . I have been at several other facilities and this one stands out in the crowd. Vivo is a good choice for your loved ones. It seems the poor reviews have no experience with other SNF"s let's face it no one wishes to be in any facility. All round Vivo is a nice place to live and I am in no way compensate for this review"

"We know several people already there at Westminster Woods on Julington Creek. The fees are high, but it looks like a nice place. They also seem happy with their place. The person who showed us around was very pleasant, and she wasn't overbearing. They have plenty of things. They have a restaurant, a small store, a barbershop, and a beauty shop. We had a good tour; we learned what we wanted. We also like the fact that they have a chapel there. The people are very friendly."

"Penney Retirement Community was excellent. The staff took me on a tour of the whole place. They have different kinds of one-bedrooms, two-bedrooms, three-bedrooms, and four-bedrooms. They have little cottages, they have a hospital environment, and they have 24-hour nurses. It's a wonderful place, and they were just finishing up the last building that they were building. The only thing they had available was a one-bedroom with a living room and a kitchen, and I just need more space than that. They have woodworking shops, they have glass-making, like the glass you can see in churches, and they have a flower shop. They have their own post office and their own grocery store. They have just pretty much anything anybody would want, and it's in a neighborhood-oriented program. Now they have two different places that are kind of like hotels. They serve meals in the dining room and everything. People can go down and go have their meal in the dining room if they don't want to eat in the room. It's very nice. They have exercise and all different kinds of things that they offer. The staff members were really nice, but I only saw two women in the office."

"It was ridiculous. I was really upset with Governors Creek. I was so disappointed to see my father. He was there for a week, and he had already been sent to the emergency room twice. They were not watching my dad, and I already informed them about his condition. His bed was not safe. There was no alarm, and it had no railing. He had no TV. It was empty. I wanted to send him home that time, but I could not. It's a very old building. I feel bad for those people who were in there. They were not friendly people as well. I've been there today because to take my dad's belongings, and they cannot even find it. They sent me to a room that looked like a mountain of junk belongings of patients. "

"The staff at this facility seemed very friendly and helpful towards the residents. I witnessed several students from a nearby private Catholic school also partake in activities with several of the residents. The students were singing a wide variety of songs and playing games using balloons with the residents of the facility. I also saw several games set up for the residents such as checkers and parchisi. I also saw several drawings done for the residents which were probably also done by some of the students from the private school. The exterior of the facility seemed very welcoming, and the landscaping on the exterior appeared to be very well attended to. The interior seemed very spacious, especially in the recreation area, which was where I was most of the time. Overall, I feel that this is a good facility where the staff genuinely care about the residents. I would definitely say that it was a favorable environment, although I have not really visited any similar facilities anywhere before. "

"My father is in a full-time nursing home now. He is at Terrace of Jacksonville. He went there from the hospital for rehab, and then it just progressed. They were so helpful in facilitating and keeping him full time, so we just chose to do that. It has been very good. We have been very pleased with it. From the very beginning, all the way through to having him admitted, the nursing staff is great, and the caregivers are great, so we have just been really pleased. It is just a relief for us in that we are just extremely pleased. "

"RiverGarden is the perfect choice for long-term and assisted living care for a loved one when they can no longer live alone. My grandmother had always been an independent woman, but the time came six months ago when she could no longer live alone due to her age. Our family researched and visited several nursing homes in the area. RiverGarden was the perfect facility. The staff is always curteous and friendly. They have awesome dietary staff to handle all nutrition needs. The facilities are always immaculate when I visit. The gardens are a great place to hang out and visit. They offer full services so we never have to worry about the care being given. Overall the best place for our grandmother. "

"The person who accommodated us was great. She was the reason that we really considered it. She was really helpful and really nice. The rooms were small, and they had no private rooms. They even had rooms with four people in them. My husband would start out in a room with two other people, but she said that when one comes available with only one other person, she would move him. They take them outdoors and fishing sometimes. They had a dining room, and the residents seemed happy enough. Signature was recommended to me, and once we get him there and if it was working for him, I would recommend it. "

"My mom has been placed at West Jacksonville Health. At that time when we were looking for a facility, they were primarily available. They were able to show us that they are very caring. The facility is close to our home and it really worked out for us. The place is very welcoming and always clean. It's very nice and I'm very satisfied with all the medical and other care that she is getting. The staff are very nice and they are good with what they do. Mom is in a room for two and it's pretty good for her. She is happy with the food. The dining area is big and it opens out to a screened porch, it's very nice and appealing. With their activities, they have the normal stuff like bingo, entertainment, religious services; pretty varied. "
