The rooms are large with good space. Large tv in. rooms. Very clean center.Housekeeping mops and cleans everyday. I have had 4admissions to this facility because of the good therapy staff that really care for the patients. The food has greatly improved.The cnas really care for their patients. I am very great ful to have had Evon and Michelinea as my caregivers

Isle Health and Rehab was home to my mother for the past 3 years, the staff and nurses tirelessly cared for Mom throughout her stay. I would visit several days a week and witnessed firsthand their care and compassion in taking care of their residents’ needs. Special thanks go to Ivon for making Mom as comfortable as possible and seeing to her daily needs. Tim, the activities director, through his Paws for Seniors program, arranged interactive visits with puppies and kittens and Janelle, for helping in arranging and encouraging participation in group activities/events, dressing in themed attire to put a smile on resident’s faces and raise Mom’s spirits. I had confidence that the staff and nurses at Isle Health took care of Mom’s needs on the days when I was unable to visit. I am sincerely grateful!

Isle Nursing Home is the best. It is in a safe high end neighborhood. It is shinning clean. The rooms are comfortable with great view of nature. It has several therapy rooms with fantastic equipment. But the most important things are care and food. My mother is there for long term. Isle provides doctors, therapists, podiatrists, and any specialist that my mom could need. The nurses are wonderful. My mom is not an easy patient. She has so many issues that would try anyone's patience. However, the entire staff at Isle, from the Administrator, to the Manager, Social Workers, Nurses, who are so many to mention by names, to the Nursing Assistants, these people are dedicated to my mother, and to so many other residents that they care for. They go far beyond the call of duty to keep her calm, well fed, clean, warm, cuddled, happy, and satisfied. I am happy and satisfied. The food is excellent and if she does not like it, which is a lot because she has specific cultural tastes, the Food Administrator will send replacement food to satisfy her taste. My mother had an incident in which she injured herself out her own difficult behavior, and the staff at hand, acted promptly, with outmost professionalism, and speed, and took more than the necessary steps to resolve the situation, help my mother, notify me, and now my mother is fine, recovering, and she has received great attention so that does not happen again. Accidents were happening at home, and at other facilities where she fell many times including the hospital, and that is why she is now at Isle, because they care and go beyond the call of duty to keep her calm and protected.She will get a special chair in a couple days, she has a special concave mattress to prevent falls, she has pads to relieve a fall, and all extra precautions in the event that other steps fail. She has all the supplies that she can possibly need, such as, diapers, bed pads, wipes, lotions, creams, shampoo, wound supplies, everything she can need, and complete supervision. I observe other residents and they look happy, singing, talking and doing activities. I highly recommend Isle Nursing and Rehab in Orange Park, Fl. I give them a 100.

My grandma was at isle health for rehab after her hip replacement. The building was beautiful the staff was great. From admission to discharge everyone was very helpful. I would recommend this facility to anyone

This place is horrible from the front desk to management. The staff never comunicates with each other and you get all kinds of different info. They never answer phone's and if you happen to get lucky and they do it's like pulling teeth to get info or if you leave message they never call back. Food is nasty and if patient not in room they just leave the tray it may sit there for over an hour they don't try and find patient or keep food and deliver when patient comes back. They loose all your loved ones belongings, laundry attendants must not know how to read, all my sister's new pajamas disappeared and gave her nasty old clothes. Rooms are depressing, need painting and some pictures and plants. Some of the nurses horrible rude, won't listen management does not communicate with you or the staff.

I have been in Isle Health and Rehab for two years now. When I came here, I liked it very much. Recently, they were sold to a new owner, and we seemed to be having some trouble keeping employees. Some of them whom I know and care about have left here, and I'm not sure why. I was extremely happy here, and when I came here, it would have been an overall score of "five" for sure, but now, with my friends/employees leaving, I would have to say it's a "three." A lot of them are still here, but some of them are leaving. I love the ones who are here. The food is very good. We used to have activities, but with COVID, we don't have anything right now. Before, we had games, bingo, and art, and we had groups come in and sing. There were lots of activities. We used to have trips, we went out to eat, or we went to Walmart. The facilities are excellent. It's a beautiful and very clean place.
