Nursing Home Communities in Green Cove Springs, FL

"Westminster Woods is located in Julington Creek on the banks of Julington Creek, a tributary of the St. Johns River. The facility is shaded by live oak trees draped in Spanish moss famously depicted by Harriet Beecher Stowe in her book "Palmetto Leaves." The facility itself is quite old, not in disrepair, but definitely showing an architecture of decades past. The community is large, but feels cozy and quaint. The staff is wonderful and curious, as are the volunteers. Many local church and Boy Scout groups visit the center as community service, which is always welcome. The summers are hot, but pleasant in the plentiful shade. There are bike paths, parks, ponds, and a pool. They have a dock on the creek, and chairs to watch the water and laze the day away. The staff is kind and helpful, brimming with southern hospitality. There are many options for retirement in Florida - but few have the charm and class of Westminster Woods. I can't imagine a more ideal place to spend ones golden years. "

"My husband stayed at Middleburg Rehabilitation and Nursing Center. It was OK, and I can't say anything bad about it. It's a clean place and there's no odors. They provide good care and have good food. They have a food serving area right in the middle of the facility. You could smell the food all over. The kitchen was there, too. They also had a nice activity room. I think that was a new facility, too, and everything was in good shape and very modern. The staff was fine. He was there for physical therapy, but he was getting sick from high calcium in his body, so his rehab didn't have any effect on him while he was there. He just couldn't do it because he was too weak and too sick. When they found out the level of his calcium, they immediately took him to the ER."

"My brother entered for rehab after breaking some ribs and having blood drained from his lung. He was weak, disoriented, and often not rational. I visited him on a Saturday, noticed he seemed weaker and he was not eating or drinking. Blood pressure checks indicated it was dropping. I mentioned these things to several caretakers. By very early Sunday morning, I received a call that they had left him back at the E.R. By the time I arrived from 70 miles away, the doctors said they did not think he could survive because he was so ill. BP dropped to 40's, vomiting, etc. They treated him and he lived, but still has issues related back to this illness. I know part of the Doctors Lake Healthcare is wonderful, but not the part he was in. They should have been more aware of what was going on with him. "

"Consulate Health Care of Orange Park was friendly. They had activities and a nice dining room. The rooms were nice. "

"I went to visit my mother-in-law at Life Care Center of Orange Park. The staff were marvelous and very attentive. The rooms are very nice and very clean. They were immaculate in fact. "

"The whole Signature Healthcare community is definitely outstanding. They had a person who come in and sing, as well as other social activities. They have a church group that comes in on Sundays. She doesn’t participate in the activities that much as she is 94. If I have a question, I know who to go to and they give me an answer immediately without my having to wait. The nurses are always there if she needs them. As of now, we have yet to find a downside. If we needed to discuss something with the financial people, their doors are open. You can sit down and talk to them. People who work there are extremely friendly and always smiling. They have an excellent rehab facility that my mother was in. The people are outstanding and excellent in taking care of her and so far, we haven’t had any problems.. They’re very professional in the cleaning as the staff clean the floors daily. The outside is always is well-groomed and it’s really nice. They have a patio area that is kept really nice, and the grass is cut every week or every other week. I just highly recommend it to anyone as it’s been a really nice place to go to. "

"The Adult Day Program they offer for day clients is amazing, and i would not trust any other facility to stimulate my father's mind and provide for his safety as much as i do River Garden. The staff in the ADP genuinely care about the clients (our loved ones) and are there because they care, not just because they need a paycheck. "

"When i volunteered at this facility i had the privilege to get a tour of the kitchen area. Which is very important in defining the health risks for the residents, and everything looked clean and organized, also the workers were using their thermometers, we played Bingo with the residents, and had service on Sundays, the nurses seem to be nice, "most of them". I just wish there was some more activities to do like other different games for them to play, but overall the place is clean, and very friendly whit residents with special needs like diabetics, and people with Alzheimers, we went also to read books to them, and I had a great time whit all the resident which i miss very much i hope to be able to have more time to volunteer in the future. To me the nurses were very responsible with the care that hey were giving, and had a lot of knowledge of their job,as good moral values, and not neglecting or not paying attention to the needs of the residents. "

"I wanted to take the time to thank everyone at Heartland of Orange Park for taking such good care of my husband. He had many issues that they had to handle daily and they were competent, caring, loving and absolutely wonderful to him. Everyone in the facility went above and beyond to make his experience an exceptional stay. I have to admit I was very tough on them but they treated me with the utmost respect and truly understood that my goal and theirs was the same - to get him better. thank you again for all of your excellent care and support. "Bartram Crossing was nice. The amenities were nice, and my dad was there for rehab. We ate several times there and I would rate it a 3. He had physical therapy, speech, and language therapy. The facility is new and very lovely. They had a common room with a TV, sofas and chairs for visiting."

"My mom is in rehab at this nursing center and this will more than likely be the nursing home we choose for her to stay in. The staff is so friendly and caring and so good with my mother. They talk to her like shes their family member, they treat her with dignity and respect and go out of the way to make her feel like shes at home. I am so grateful for them and for loving my mom the way we do at our house because it isn't easy deciding on placing a loved one in a nursing home. Its been very hard for me emotionally, especially around the holidays, but knowing that this nursing home will care for her like I did for 4 years living with me makes the transition a little easier. "The staff at Beauclerc Manor was very nice and very knowledgeable. They answered all my questions, so that is very good. The facility was very clean, and it was small. It has a nice little area outside where the residents can sit and read. My dad doesn't like a lot of crowds, so he would fit in well there. They do a lot of activities, and so I would recommend this facility. "

"The staff at this facility seemed very friendly and helpful towards the residents. I witnessed several students from a nearby private Catholic school also partake in activities with several of the residents. The students were singing a wide variety of songs and playing games using balloons with the residents of the facility. I also saw several games set up for the residents such as checkers and parchisi. I also saw several drawings done for the residents which were probably also done by some of the students from the private school. The exterior of the facility seemed very welcoming, and the landscaping on the exterior appeared to be very well attended to. The interior seemed very spacious, especially in the recreation area, which was where I was most of the time. Overall, I feel that this is a good facility where the staff genuinely care about the residents. I would definitely say that it was a favorable environment, although I have not really visited any similar facilities anywhere before. "

"My father is a long term-care patient in this facility, he has been a resident there for almost a year and a half now and he loves it there! This facility is far from perfect but most of the staff up to its leadership, they do care for my dad and elderly. Realistically speaking there's no nursing home out there that doesn't have problems, specially with this time of pandemic, family members should understand that it is also their responsibility to help out their loved-ones that is residing in here. This was recommended by one of my colleague and this place has been a blessing to me, my whole family and my dad! My father originally came here for rehab and boy the therapists here are really great at what their doing, they know the appropriate time when to push my dad to get stronger faster. Kudos to RJ, Hazel, Grace and Matt! My dad and I was already looking for a great place where he can get the help he needs on a daily basis, and he connected with the staff here. Thank God for nurses like Marsha, Tony; the dietitian Jena, Teri and the director and head of nursing department (sorry, I forgot their names and some other). They treat my dad as family. One thing that really impress me besides the quality of care is that my dad and I noticed that the staff always pray as a team every day. The staff here has been through a lot because of this pandemic, but they are still there for my dad and others day in and day out. I, truly am very thankful for this facility! They are a blessing in this community.."
