This facility was a five star facility a few years ago....I am fortunate to have a loved one living there....I visit him weekly and have never noticed any one being treated badly....The only thing I notice is there is a large turnover in staff...The administration is excellent....Do not forget your loved ones...They matter...

My wife was here from the evening of June 22 through June 25 when I bought her home. We were told she would receive physical therapy three times a day; she didn’t. When she needed help from her bed to the portable toilet she didn’t receive any in time so . This happened many times. She never spoke with or saw a physician. Saturday she was told because she arrived in the cafeteria ten minutes late, she would have to eat her lunch in her room at twelve. I wouldn’t suggest using this facility ; very unprofessional & very uncoordinated.

If you really want your relative to return home healthy, this is not the right place. If you want to leave a loved one lying down to death your family member, this is the right one. The place very little hygiene, smell of urine in the rooms, vomiting without cleaner in hall for half an hour or more before they clean the area. broken beds, patients who can not walk together with other patients who can walk. the staff some good others insensible. I honestly tell you, if you feel something of love for a loved one, do not take it to this place. I give it zero star if I had the option. I do not recommend it!!!!

I went to this facility to visit someone and what I seen is awful.. I saw the patient with dirt under her nails. No interaction at ALL. It crowded in the rooms.. Floor dirty as if it was a bando.. They leave patients that can't care for their self left for dead.. Is very brutal to see ppl left like that.. The ppl not even putting their self In those ppls shoes on how it be like. Do not send your ppl their. Had to put star. But wouldn't put any.

I would not recommend this facility. If you have the choice, please send your love one somewhere else. They show little concern for their patients. I show up for a visit only to be told my loved one was not there, that he had been admitted to the hospital. Yet no one at First Coast had thought to call me or any other relative. In addition, that same day I went to his room I noticed all of his clothes were missing. When I asked the nurse said they were all in the laundry. After coming back a few days later to collect his belongs, no one could find his clothes. Pants, sweatshirts, t-shirts, everything was missing. I was told they would look for the clothes and call me. I never received a call back. Later I discovered the cash in his wallet was also missing. When I called the administrator to find out about the clothes (which they never found) I told her about the missing cash as well. Her reply to all of it was basically "this happens sometimes." The lack of concern about having such unethical people on staff that can take advantage of those that can't help themselves is truly upsetting.

I've been at First Coast, and it was busier than I would have wanted. It seemed a little crowded, it was a smoking facility, and I don't care for smoking facilities. I did see activities that they were doing, and people that were there seemed very pleased. Another thing is that it seemed like it was in a flood-prone area.

I had no problems with First Coast Health & Rehab at all. They gave good care, and everything worked the way it was supposed to. They were very good. The staff was very knowledgeable and friendly, and it was very clean. My dad said the food was fine.

I would never recommend anyone to ever go to this place. It was horrible. They neglected my father everyday. He just laid there suffering. He fell because noone was watching him. No bed alarm no rails nothing! After he fell he died 9 days later from his injuries. Please listen dont let your loved ones be placed here. The only reason there is one star is because i could not submit otherwise.
