My grandmother is at Palm Garden of Jacksonville for rehab. When they go to her room for rehab, they don't just let her say no and just sit there. They encourage her to get up and do her rehab. Every time I've gone there, she's been clean and getting her therapy. I'm really impressed by them. The only issue we had was that the telephone and television in her room weren't working. My grandmother has been enjoying the food. The facility is nice and clean. It doesn't have any smells. I think the price is kind of steep for an elderly person who doesn't get much finances, but maybe they can help or get another type of insurance or something to help out with that.

This facility is extremely inadequate with their service They have very few RN's and largely staffed with only LPN's The menu is poor to say the very best The rehab facility is extremely limited to options with the staff only offering repative exercises every day The facility needs additional equipment in the rehab facility A resident is not checked on by the staff on a timely manner I have a cockroach living in my bathroom More activities should be offered to the residents

I stayed at this facility for one week. It's not a rehab,only a nursing home. Nurses are sub-par and don't care for patients. I waited 2 hours to have my bedpan changed because I couldn't get out of bed. Some of the nurses are mean and nasty and 90% of them are lazy and don't want to change the bed sheets or bandages. They speak down to you and when you get a nurse to finally come in your room, they tell you they'll be right back and don't come back. I don't recommend this place at all. Please read reviews before sending your loved one here. This place is horrible.

Very snoopy people, they don't feed the residents enough, and very poor attitudes towards residents and family.

I visited Palm Garden of Jacksonville, but right now they're going through some renovations, so it's kind of in disarray. It's strictly a nursing home. We got to see a couple of the rooms, the little beauty shop, and the dining room. The rooms we saw were very old, that's why they need renovating. They're very small and the rooms have two beds. So I hope they improve on that. I met one of the residents, she was very friendly, and has been there for several years. She appeared healthy and happy, and that meant a lot to me. To be able to meet a resident is very neat, and the lady that showed us around was very considerate. The place is in a vulnerable state right now because it's in transition. I also met the receptionist and the staff seems fairly nice. I would rate them a three because it's really in disarray, although it's functional.

We've been happy with my mother-in-law being at Palm Garden of Jacksonville. They're watching her very closely in her condition. They take good care of her. We have hospice that comes in three times a week as well. They've had some maintenance issues, but they're getting them resolved. They seem to be on top of things. It's clean, and it's pretty current in its decor. The rooms are pretty small for a double room and the bathroom seems a little bit outdated, but it's adequate. They have a room that has a lot of games, puzzles, baby dolls, an artificial dog and cat, they have them do little activities in there together sometimes, they paint their fingernails, and they have a church service that comes in every week. They could do more as far as activities go. They seem to have really good food.

Worse facility in Jacksonville. My family member did not have a shower for months, dehydrated, messed up his meds, low oxygen level, developed a UTI that went septic. Sent my family member to wakiva springs instead of the hospital. Which at the hospital they discovered the UTI and told me it went septic and my family member would have died in 2 days if I didn't get him to the ER when i did. Very rude to me when I started asking questions. Tried to cover up the lack of care they were giving. Please think twice about this place. I treat my animals better than they do with their patients. Because of their lack of care, my family member died a year later.

Palm Garden is a HORRIBLE facility. It is dirty, I saw a giant roach under my fathers bed. The nursing care is sub-par. The CNA's have serious attitudes. There is NO security. Everyday I would just walk in, no-one noticed there is no sign in. My father had orders to check and see if he had a urinary infection. This was not done. If they would have followed the orders they would have noticed his catheter was clogged. He was in excruciating pain. He went from there to Hospice where he arrived in septic shock. DO NOT put your loved one in this dirty, disgusting, disgraceful DUMP!

I have a friend in Palm Garden. They have exercise and game rooms, and the cafeteria doubles as an auditorium. The food is great. The room is spacious, clean, and you don’t smell urine. The have bingo, movies, cards, and a group doing building blocks.

Palm Garden of Jacksonville was nice. It was beautiful and clean. The people were very nice. They had a lot of activities, which were good for the patients. I cannot think of anything that they can improve on. They were very helpful. The food was good.

Palm Garden was a wonderful place. Their CNAs and nurses were wonderful. The therapy was very helpful. Everything was clean, and there was no smell of urine. I had a wonderful experience. The food was good. Their rehab department was very well staffed.
