About Autumn Village Assisted Living in Jacksonville, Florida

Village is an 80 bed facility located in the heart of historic riverside, three blocks away from St Vincent’s hospital. We have a beautiful secure and large courtyard with beautiful landscaping. Many of our clients enjoy the flexibility and freedom of being able to walk in the riverside area, visiting the shops and cafe’s in riverside. Our staff is experienced and trained both in caring for the elderly and also in providing support and care for some clients with a mental health diagnoses.

We have a variety of units to choose from, including apartments with a kitchenette/dining room, private/semi private rooms, and some units with private restrooms. Although we are an 80 bed facility with a large staff, we have a campus of five buildings on the property with only 16 residents each. Thus our residents get the care and support offered by a large staff, but the warm feeling of living in a small home with only 15 other residents.

  • Fully staffed 24 hours a day
  • On site visiting physicians and health care providers

  • Hospice care
  • Adult day service (provide care for residents who stay with us during the day and with their own families at night)
  • Respite care (depending on bed availability we can provide temporary care and housing in times of need.)

I visited this facility

Autumn Village Assisted Living was really nice, and everybody was friendly. They gave out a lot of information to me. I think it's one of the nicest ones between that and the place I am at right now. Their menu looked really nice because it had a variety of different things. You get to choose what you want to eat from two different things they have each day.

I visited this facility

Autumn Village was not a place I would put anybody. I didn't like the people and I didn't like the facility. The staff was fine, but I didn't like them. There was just nothing about that place that I thought was nice. I know they do their best, but that place was not somewhere I'd put a relative, especially my wife.

I am/was a resident of this facility

This place is COMPLETELY different from everything written on tjis site. The food is HORRIBLE! The only place I've been tgat considers potato chips a vegetable. There aren't many activities(&they are always in the other building). Bingo for $ Tree store items..Oh, HOLD ME BACK! In other words.........DO NOT CONSIDER PUTTING A LOVED ONE OR YOURSELF INTO TO PLACE!!!!!!!!!

I am a friend or relative of a resident

We had my sister at Autumn Village, and everything is wonderful. Her room was really nice, and she is happy with it. The facility is marvelous, and the staff is just too good to be true almost. They are just so helpful. The people there are friendly. The food is very good and very well prepared. They have buses that take them places, and they also have barbecues and game nights. It's very nice. She is just happy as she can be.

I visited this facility

Autumn Village is an extremely nice and well-kept facility in Jacksonville. The food smelled great, the facility was clean, and the atmosphere was warm and inviting - much unlike many other assisted living facilities. It was only a few minute drive to the scenic river, and within a 30 minute drive to the beach. I enjoyed sitting with my client in their activity area, which had plenty for the clients to do inside - such as card games and crafts. The lady who owned the facility was very friendly, and did everything that she could to make sure that my client was happy every time that she would walk by - which really impressed me. Many owners have their employees do all of the work - but the owner of the facility was involved with the patients here - and that to me is very respectable. I would definitely recommend this facility to anyone in the Jacksonville area.

LOOKING FOR A HOME FOR A LOVE ONE?

I must say that no place is like home, but from experience and for the price you will not find a better place. The Owner is a Christan and it shows the way she treats her patients. The home is kept very clean, the food is better than I eat at home and the people who work there are very friendly. My Sister has been their years and in all that time I must say I only had one conflict and it was taken care of. My sister is VERY happy and has never once complained. She is well enough to know if someone mistreats her. I recommend this home, along with my sister.
