When she got older my grandmother moved into Oakleaf Village in Sylvania Ohio. Its a very interesting place, different from any nursing homes I had visited and therefor not what I expected.The facility has very nice one bedroom apartments complete with a small kitchen, handicap bathrooms, a fair size bedroom and nice living room. Residents may also request a small garden plot.I expected a depressing place with elderly people just sitting around but this place was the exact opposite. Just walking in the front door the place was buzzing with activity and the main lobby was welcoming with books, pet fish and birds. There was always something going on, I often wanted to visit just to join in on the activities. I attended picnics. There was a fair complete with petting zoo for grandchildren once, a Daytona 500 party, card games, music, singing. I could just keep going on.My grandmother even said that she was afraid to move there but once she got there she found it great that she could walk down the hall and visit with new friends. I joined her for dinner a few times and met those friends, the staff were kind to her and me all the time and there for her if she needed anything day or night. This is a Jewish home for the aged. I think it is neat that they have this facility for people of the same religion. The residents all seem very happy and very well taken care of. Her dresser drawers do not open or close correctly and is a hazard for her to use. We have complained to maintenance and nothing has been done. WE have been waiting on a care plan for weeks. Cannot ever find administration to talk to or social worker. No one calls back either. My aunt is supposed to be staying as long term but it is a hazard for her to stay there. She waits over 60 minutes for an aid or nurse to help her out of her bed to the bathroom. She has also now fallen and broken her hip while staying there due to lack of care and wait for help. They never have her paper work up to date. For example , her discharge papers form the hospital with after care no one could find and one nurse argued that she never went to the hospital! Food is over cooked and hard to eat .Would not recommend this facility to anyone.

This facility JUST had its name changed in November of 2020 to Divine Rehabilitation. My father arrive at the facility on February 1st. Had to be quarantine for 14 days which is understandable. I called on the 15th to see if he was able to get up and around in a wheelchair and the receptionist is extremely rude to me every time I call told me that the nurse and social worker were in a meeting and they would have to call me back. I never received a call back. On the 17th, I was informed that hospice delivered a wheelchair for him and tried calling again to make sure he was getting out of bed, response. I talked to my dad today and he told me that he hasn’t been out of bed ONCE since he got there. He was supposed to be out of isolation on the 15th and is STILL in isolation. When I talked to the receptionist today she said she remembers me calling. As soon as I started asking for names of people in charge, she had an extremely rude attitude towards me. So, he’s been isolated without visitors for 30 days! It’s no wonder this facility had a “name change,” but from reading these reviews, I assure you that’s the only thing that has changed! We also sent him with a ton of clothing and he said he’s only in a gown with the back open, a diaper, and bare feet!!! I’m so heartbroken that he has to be there. Just awful.

Encompass Health Rehabilitation Hospital of Toledo was a fairly new building. However, the alarms were continually going off at the nurses station, which you could hear all the way down the hall where my mom was at. Every resident could hear it, and they were going off all the time.
I understand this was in the midst of COVID, but they did not allow any family member to be with my mom during the intake process. It was extremely confusing and upsetting to her. She didn’t have any of her own clothing. She was cold. She didn’t know what she was doing or where she was going. There were no familiar faces. They wouldn’t let any of us in until visiting hours started some 7 hours later.
Also, the food was really bad. I was not happy with them. Other than that, the coordinator was excellent, and the nurses were very kind when therapy came in.
