"My husband was at Sylvania Center. He wasn't getting the therapy that he was supposed to get. When I went up there, he was out of it most of the time. When I asked them to remove some of his medications, they said they would one at a time because they didn't know which one was causing the hallucinations and the other things. My husband also had a hard time swallowing. When he was released from the hospital, he was on a thickener, but he wasn't at this nursing home. He couldn't participate in physical therapy because they had him so drugged up, so he had lost the ability to swallow and was not able to walk. He had such a bad infection in his leg that it looked like it was ready to pop. I wanted him to go to the hospital, but the doctor from the nursing home refused to call the squad. When I got him to the hospital, he was so severely dehydrated that his tongue was black. They couldn't get an IV in him because his veins were like glass and his kidneys had failed. Basically, they couldn't do anything to save him. He was in the hospital for a couple of weeks and I had to make the decision to take him off life support."

"We choose The Lakes of Sylvania. It is very good. It is a brand new building with brand new staff. They are still in the caring stage at this point. The dining area is fine. The room is a little bit on the small side. It is adequate for her needs."

"My parents moved into Sunset Village seven months ago. Everything is going really well, and they're very happy. The staff is good, right there when my parents need help. The food is very good. They have housekeeping come in once a week to clean the room. It's a nice size, and when you walk in there's a nice living room, a kitchenette, a little refrigerator, and a microwave, but no stove. They have one bedroom, a bathroom, and it's big enough for the two of them. They have different sizes of rooms there. They're in assisted living, but my dad needs a lot of help. They have bingo, music groups come in, they take trips out to stores, they go to a restaurant once a month, they went to a casino this month, and this summer they will have outdoor concerts come to the facility. They have lots of activities if the individuals want to take advantage of it."

"This retirement community is a life changing place where I had decided to go with my relatives. When I was there I almost wept looking at all these aged people who have been abandoned by their children or do not have the proper financial resources to protect themselves. It was a sorry state of affairs when I saw them but they did not show a single glimpse of sadness on their face. They were really happy and when I started interacting with them they were talking to me happily. I asked them a bunch of questions like what is their typical day like or what do they do here for all of you and they said this place is refreshing. They actually provide financial assistance to the elderly and also ask all of them what they want during this phase of life. So they actually have a wish list. They receive a lot of gifts from visitors and we also gave them a lot of donation to bring smiles on their face. It was the best time of my life and I would love to go back there again. "

"Encompass Health Rehabilitation Hospital of Toledo was a fairly new building. However, the alarms were continually going off at the nurses station, which you could hear all the way down the hall where my mom was at. Every resident could hear it, and they were going off all the time. I understand this was in the midst of COVID, but they did not allow any family member to be with my mom during the intake process. It was extremely confusing and upsetting to her. She didn't have any of her own clothing. She was cold. She didn't know what she was doing or where she was going. There were no familiar faces. They wouldn't let any of us in until visiting hours started some 7 hours later. Also, the food was really bad. I was not happy with them. Other than that, the coordinator was excellent, and the nurses were very kind when therapy came in."

"The facility of Ayden Healthcare of Toledo is bad. I've talked to some of the residents there and it's not what it's all planned out to be. Their food isn't the greatest. I've seen it being distributed out. I wouldn't even feed my grandbaby that kind of food. The staff was pleasant, but I just wouldn't let my uncle go there. I just wouldn't. The rooms were small and they got to share bathrooms. There's two people in a room, so that's a total of four people using one bathroom. Absolutely not. It's fairly clean (not as clean as I would like it to be). They need a better dining area."

"I used to drive ambulette (wheelchair transportation) for a private ambulance company. In doing this, I visited this facility numerous times. The facility was always clean and smelled nice. This is a Jewish home for the aged. I think it is neat that they have this facility for people of the same religion. The residents all seem very happy and very well taken care of. I have also heard good things about the homes rehabilitation / physical therapy program. The staff / nurses are all very helpful and friendly as well. Overall, I would say this is a very nice nursing facility. "

"The Lourels of Toledo has one of the best facilities for assisted living care in Northwest Ohio. The staff is friendly and attentive, the facility is clean and well maintained and there are many activities for the residents. The Lourels stands by their commitment of giving high quality care to all of their residents, to making each resident feel important and safe, and it's enjoyable for friends and family members to visit. The management and staff are strict with the guidelines and expectations they have set for their staff and care takers. This is a friendly environment with great amenities for the residents and for visiting guests, and the residence is located in a safe area where loved ones and residents can feel secure. This is one of the best options for someone who is looking for an assisted living center in the area for a loved one or friend. "

"When our kids were little, they visited a buddy over there at Fairview. They went over there once a month to visit their buddy. The rooms were a little bit older. They are not quite as updated as the others but that wasn't really something that we had in mind. We wanted the care more than we care about how it looked like or how fancy it was. They had something going on, but it was quite late in the afternoon so most people were sitting around and kind of waiting for dinner. The security is fine. "

"The assisted living home that my friend was living in had pretty nice amenities. The setting that she was living in was like a mock apartment. She had her own kitchen, bathroom, common area and bedroom. The room also included a television and a window that overlooked a beautiful portion of the garden outside her room. If she didn't feel like cooking, there were meals served three times daily that included items that ranged from pulled pork to spinach soup! The atmosphere of Spring Meadows Senior Community was very amicable and included staff that were very knowledgeable, competent and hard working individuals. My only concern regarding the living community was that it had a slight odor to it; something I just cannot put into words. It wasn't overwhelming but the stench was always present and it was intensified the closer you got to the common area of the community. But, nonetheless, I really enjoyed the Spring Meadows Senior Community when my dear friend was living there. I would highly recommend it to any individual looking to place a friend, parent or grandparent in an assisted living community. "

"West Toledo is a very large community type assisted living. For starters there is always plenty of parking. They work hard to keep their grounds area neat and clean. They have spacious rooms, (the first asl my grandmother and I looked at we could hardly get her wheelchair through the room doors!). The staff is incredibly attentive to their residents and my grandma says they always have group activities in the morning and afternoons she enjoys. "

"My mother was a resident for a time at Swan Creek. We selected Swan Creek based on the patient/staff ratio that was the highest in Toledo and above the state and national average for staff/patient. We were please with the caring staff, good food, and cothempetent therapists. "

"My sister-in-law was at Genacross Lutheran Services-Wolf Creek Campus. I was not able to get there because she was under COVID quarantine for two weeks. She only lasted three weeks, but as far as I could tell, they were very helpful for me every time I called. They were helpful for the family getting them in and just being able to see her. When I went to the place, I was very impressed with it. For having a lot of old folks around, it was extremely clean. The staff was very friendly and helpful, even just seeing how they interacted with the residents. It seemed like a very caring place. I was pleased she was able to get in there. I didn't get into her room and I wasn't able to see her after she got there and get settled in, but my impression of the place was very favorable. It's a little bit expensive, but it gave her what she needed and that's what we were glad for. I was treated very nicely and very professionally, and they were very helpful. They went out of their way because I can't walk very well myself. When I first went in, I was just looking around for admissions and didn't realize I had passed it, but the lady got up from her desk and went and found the lady I was waiting to see. They were very helpful. I was impressed with the cleanliness. It just had this peaceful feeling, so I was impressed with it. I didn't realize they were that expensive. I never had to deal with a nursing home before and she was a private pay because she had money in her bank account. It wasn't a Medicare or Medicaid thing. If it would have been me, I couldn't have afforded it, but her family and she could afford it, so it was OK. They took good care of her."
