This facility JUST had its name changed in November of 2020 to Divine Rehabilitation. My father arrive at the facility on February 1st. Had to be quarantine for 14 days which is understandable. I called on the 15th to see if he was able to get up and around in a wheelchair and the receptionist is extremely rude to me every time I call told me that the nurse and social worker were in a meeting and they would have to call me back. I never received a call back. On the 17th, I was informed that hospice delivered a wheelchair for him and tried calling again to make sure he was getting out of bed, response. I talked to my dad today and he told me that he hasn’t been out of bed ONCE since he got there. He was supposed to be out of isolation on the 15th and is STILL in isolation. When I talked to the receptionist today she said she remembers me calling. As soon as I started asking for names of people in charge, she had an extremely rude attitude towards me. So, he’s been isolated without visitors for 30 days! It’s no wonder this facility had a “name change,” but from reading these reviews, I assure you that’s the only thing that has changed! We also sent him with a ton of clothing and he said he’s only in a gown with the back open, a diaper, and bare feet!!! I’m so heartbroken that he has to be there. Just awful.

If looking for care for your loved ones be very careful when choosing this facility. They have gone downhill BIG time. Sadly A LOT of the staff are inconsiderate, rude, and OFTEN express their frustration in caring for their patients. Heartbreaking to see this with my own eyes let alone happening with my own Mother who has dimentia. The facility cleanliness is ALWAYS an issue. The food selection/portion size is a joke. They constantly have the same food and it's as if they are feeding swine. They are not attentive to their patients needs as they ought to be. My Mothers hands have been filthy on many occasions with what look like either feces or food. When you ask for someone to take care of it or help with something you will most likely get an attitude or have to wait because everyone plays the "it's not my job" game. Many of the "professionals" lack compassion and integrity and it's truly Heartbreaking to witness how they care for your loved one as well as the other patients. There are some that really express concern and are genuine but unfortunately they have far more bad apples that drive their facility right in the ground. I could go on about all of the awful things I have witnessed but this would be too long. Again, just be very careful if looking at this facility and at this time until they get it together I highly do NOT recommend this place.

One nurse for 50 residents. It's horrible. The nurse tries but there is no way good care can be given. The administrator needs to go. Who has only one nurse to 50 residents on day shift and thinks it is ok? It is not ok!

The Laurels of Toledo was alright and clean, but it had a nursing home atmosphere. His room was nice, and it had a nice TV and a little couch. Some staff were OK, some were not. They need to have more attentiveness to the people. They need to be more caring and respectful. The young people called my father by his first name. They also need to do something with their food because their food was nasty, and he complained about it.

My dad stayed in The Laurels of Toledo for three months. The facility was small but it looked clean. Their rooms weren't large enough especially when he was sharing it with someone else. I was not satisfied with their nursing staff. They didn't treat him well for his diabetes, like feeding him normal regular diet. They also didn't help him feed himself especially when he had a paralysis and he wasn't able to feed himself. Their office staff service was a dissatisfaction. No one was informing me about anything. They never told me about my dad's appointments. 90% of the time his appointments were at the place I worked at and I was just surprised to see him there because they didn't tell me about it. I need to be prepared because I need to tell my boss about these appointments, but they never told me. It was not good. Also when they sent him home, it was against my wishes. For 30 days, they scheduled someone to come to the home and take care of his room and check his diabetes, but after 30 days, I was unable to do those things 24/7, he along with my mom and holding down a full-time job.

My husband had been placed in this facility for rehab after a major surgery. Infection was a major concern as to why he was unable to go home. They were informed of his arrival the previous day and he didn't get there until late in the day. When checking into his room, we found the living conditions apauling. The room was filthy, the shared bathroom had dried feces on the toilet, walls, and floor, and bodily fluids that were left on the floor and waxed over. The portions of food were smaller than I would feed a child around the age of 5 and not at all balanced and healthy. Medication that was needed immediately, had not been ordered and didn't show up until the following morning. When the medication did arrive, he was made to walk from one end of the facility to the other to get his medication, they did not bring it to him. This facility is terrible! The only way you should send your loved one here is if they have the best insurance in the world. This facility only cares about the money, not the residents.

The Lourels of Toledo has one of the best facilities for assisted living care in Northwest Ohio. The staff is friendly and attentive, the facility is clean and well maintained and there are many activities for the residents. The Lourels stands by their commitment of giving high quality care to all of their residents, to making each resident feel important and safe, and it's enjoyable for friends and family members to visit. The management and staff are strict with the guidelines and expectations they have set for their staff and care takers. This is a friendly environment with great amenities for the residents and for visiting guests, and the residence is located in a safe area where loved ones and residents can feel secure. This is one of the best options for someone who is looking for an assisted living center in the area for a loved one or friend.
