"My mom got very good care at Mercy Memorial. The nurses were really attentive, and the aides were right there if you needed them. They were really good. The rooms were spacious and bright and in a comfortable setting. It did not feel institutional. They allowed us to bring in some of her clothing and little gifts to spruce up the room. The food was excellent, and they would take her down to the community room to get her meals. It would take her out of her room and in with other people, and I would sit around on occasions or one of the nurses would. She did like the food. "

"My aunt was in this facility for 6 years. We have been unable to see her for a year due to Covid. The staff were definitely her surrogate family and it showed. They contacted us to tell us she had severe pneumonia and was being placed on hospice. Our family was able to go and see her before she passed. While there several staff members came to pay their respects on their day off. It was obvious they loved her and she was so well taken care of. The facility was spotless and very nice. I cannot say how impressed I was with how great the staff took care of my aunt. I know sometimes that could have been difficult as she was very particular. From our family I want to say thank you! She clearly loved all of you. She will be missed by us but I know they will miss her just as much. Thank you Haley. Charles and Sherri for your love. I know there were others so thank you also. Don't hesitate to send a family member here. "

"Fountain View is small, but newer. The staff seem to be very nice. They bring the dementia patients out every day to do puzzles and socialize. They try not to keep them in their room as much as possible. "

"The first thing I noticed when I came here was how positive everyone was, and it is still like that every day! All staff, including the management are so kind and helpful. There are lots of activities that keep you busy and not bored. The food is EXCELLENT, just like restaurant food, and I look forward to meal time every day. LaToya is a great worker and goes above and beyond for me and everyone else. I really like Dee, Brandy, Emily, and Tina. They are always so helpful. This is a great place to be, and I feel comfortable and like I can be myself."

"I was at Hickory Ridge of Temperance for physical therapy. The staff was very caring. The place was clean. The food was pretty good. If you don't like something, you could ask for an alternative food choice. They did the laundry, too. They were very cordial. I like the food there. The facility itself was neat. Their activities were typical crafty things and bingo."

"We were very concerned when told our mother needed to moved to assisted living and even more so when we started visiting the facilities that accepted Medicaid. However, we are so pleased with Merit House, the staff and the attention they give her. The facility is clean and is continually being updated. She has a beautiful room. The staff is amazing caring for her as if she were their relative. I cannot recommend enough this facility. "

"My husband was at Sylvania Center. He wasn't getting the therapy that he was supposed to get. When I went up there, he was out of it most of the time. When I asked them to remove some of his medications, they said they would one at a time because they didn't know which one was causing the hallucinations and the other things. My husband also had a hard time swallowing. When he was released from the hospital, he was on a thickener, but he wasn't at this nursing home. He couldn't participate in physical therapy because they had him so drugged up, so he had lost the ability to swallow and was not able to walk. He had such a bad infection in his leg that it looked like it was ready to pop. I wanted him to go to the hospital, but the doctor from the nursing home refused to call the squad. When I got him to the hospital, he was so severely dehydrated that his tongue was black. They couldn't get an IV in him because his veins were like glass and his kidneys had failed. Basically, they couldn't do anything to save him. He was in the hospital for a couple of weeks and I had to make the decision to take him off life support."

"Encompass Health Rehabilitation Hospital of Toledo was a fairly new building. However, the alarms were continually going off at the nurses station, which you could hear all the way down the hall where my mom was at. Every resident could hear it, and they were going off all the time. I understand this was in the midst of COVID, but they did not allow any family member to be with my mom during the intake process. It was extremely confusing and upsetting to her. She didn't have any of her own clothing. She was cold. She didn't know what she was doing or where she was going. There were no familiar faces. They wouldn't let any of us in until visiting hours started some 7 hours later. Also, the food was really bad. I was not happy with them. Other than that, the coordinator was excellent, and the nurses were very kind when therapy came in."

"The facility of Ayden Healthcare of Toledo is bad. I've talked to some of the residents there and it's not what it's all planned out to be. Their food isn't the greatest. I've seen it being distributed out. I wouldn't even feed my grandbaby that kind of food. The staff was pleasant, but I just wouldn't let my uncle go there. I just wouldn't. The rooms were small and they got to share bathrooms. There's two people in a room, so that's a total of four people using one bathroom. Absolutely not. It's fairly clean (not as clean as I would like it to be). They need a better dining area."

"The staff at Liberty Nursing Center of Toledo are all part of the care team for the residents, that from the janitor to the director. Everyone is involved with the residents and they believe in promoting participation in care from their clients. The approach is both mind body and soul. Residents are offered various activities which will help them achieve that goal such as religious services for those who are not able to attend church, bingo and many more. Liberty is located in the center of Toledo and is part of a vibrate neighborhood. It's a place where one can see that the staff really cares for their clients. "

"To start off, the facilities parking lot is ridiculous. There is never anywhere to park and sometimes you have to wait for hours to get a spot. The elevators (There are two) are usually jam packed with people and not always operating correctly. I have gone in this facility several times and they had one if not both of the elevators out of service. The staff is not very pleasant either. I would not put my dog in this facility. This is just my opinion, but I have never had a pleasant experience while going to this facility "

"My sister was at Majestic Care of Toledo. The atmosphere there was very nice. It's brand new, so everything was just very pretty. My sister got good care there, and they're very friendly people. I noticed the rooms were very small, so that's a part I didn't like. They have physical therapy, and my sister also said they have a beauty shop."

"The nurses, nurse aides, PT & OT therapists, nurse practitioner, and social workers were professional friendly and provided me with the utmost excellent day or night. During my time there I felt safe and comfortable. This facility is kept immaculately clean, and thankfully practices strict Covid protocols. I highly recommend this facility to anyone of any age in need of extended medical treatment, and/or Rehabilitation therapy."

"I used to drive ambulette (wheelchair transportation) for a private ambulance company. In doing this, I visited this facility numerous times. The facility was always clean and smelled nice. This is a Jewish home for the aged. I think it is neat that they have this facility for people of the same religion. The residents all seem very happy and very well taken care of. I have also heard good things about the homes rehabilitation / physical therapy program. The staff / nurses are all very helpful and friendly as well. Overall, I would say this is a very nice nursing facility. "
