" I have been surprised and very pleased by the care a client has received at The Oasis. My client's room is clean, attractive and well-equipped, his caregivers are respectful, competent and genuinely nice. Grooming help is provided regularly as needed, and my client is always dressed in appropriate, clean clothes. Meals and snacks are of good quality and presented well. The Social Worker communicates well with me and my client, both in person and by phone. OT and PT services have been very helpful to him. And I have received all the help needed in negotiating insurance issues from an excellent financial department. There has been and continues to be careful attention paid to safety and security for clients, as well as following of Covid-19 health guarding guidelines. The results of Oasis' attention to the welfare of my client has resulted in a healthier, happier and more satisfied client than ever before, and we are both very appreciative. Thank you Oasis! "

"I visited a friend at ProMedica Monroe Skilled Nursing and Rehab. The place was very good. They had totally redone it, and it was very nice. It was very clean and updated, and I liked it. I think my friend was happy there; I don't remember her really complaining so much. The place felt homey."

"Fountain View of Monroe was a nice place. They took good care of everybody there. I had a private room and an adjustable bed. They also had a hoist and everything to help me up when I couldn't get out of the bed by myself. The staff was just awesome and were very patient. Maintenance was good, too. I had a problem with my bed one time, and they did what they had to do to get the bed back up. As soon as they got it working, they got me into the wheelchair, took the bed out, got it fixed, and brought it back. The food was really good, too."

"I feel like I get great care at Point Place. When I had to go to the ER from a doctor's appointment, my aide, Shyra, stayed with me until 10pm. That was so sweet of her! The staff really care and try to help. They can tell if something is wrong with me or if I have something on my mind. They always make sure I have the care I need. They are helping me getting better so I can get home!"

"I moved into Hickory Ridge of Temperance. It's very nice. They have patios outside. They keep the grass, and the parking lot is in good shape. The staff is wonderful, not enough of them, but it's wonderful. They play bingo three times a week. They have crafting classes, they have musical people come in and sing and play instruments, and they have parties quite often. The food is sufficient. It's a little bit repetitive. There are a lot of pasta dishes, which I don't care for, but a lot of Italian pasta dishes."

"My mother was at Aberdeen Rehabilitation and Skilled Nursing Center for 3 weeks. Our overall experience was above average. The service provided was a very quality service. The nursing staff there was attentive for the most part, and they were caring, friendly, and kind. They were good to us and they took good care of my mom. The rest of the staff was very professional, kind, friendly, and supportive. The building itself was older. Aesthetically, it was not modern per se, but it was very clean and had no odors. The floors were clean and the rooms were clean. They prepared good meals in good portions."

"My mother is in Applewood Nursing Center. Right now, everybody seems to be very helpful. You can call and talk to the nurse at any time about your patient, I've even had the nurse Facetime me, so I could see my mom. The outside is very well-kept, and well taken care of. Even people at the hospital and the social worker said "that's a good place." They have a lot of activities for them and dances."

"I have been at Merit House since mid December. I was extremely reluctant coming in because I’ve never had to deal with this before. I had no idea what to expect. I had people checking places for me and did review checks of my own and must admit I found other places with higher ratings who’s beds were all full. I could not be happier that it worked out this way! Merit House has gone above and beyond any expectation that I could’ve ever had! From the rooms being single occupancy to the extremely friendly and professional staff to the professionally trained therapists who not only did o/t and p/t but mental therapy for me when needed, my experience was superb!!! I will, no doubt, recommend Merit House to everyone that needs a facility and only ever use Merit House should I ever need them again!! Thank you so very much to the entire staff of Merit House!! You should all be extremely proud of yourselves and your commitment to excellence!!!!!"

"The people at Majestic really care for and about the residents. From the executive director, administrative staff, caregivers, activities leaders, to nursing and patient aides give their best to care for our Mother in the memory unit. She has lived here for 10 years. Majestic has invested in facility consistently. Through COVID, they maintained protocols to keep residents safe and thriving."

"My mother will be moving into Aerius Health Center. It was very clean and it was a newer place. It didn't have a foul smell. This place had a more homey atmosphere rather than a hospital feel. They had the people needed to take care of my mom's needs and they catered to what we were looking for. The staff was very nice and professional. They would serve dinner, give baths, do the laundry, and do the everyday chores. There would always be somebody close by to assist her. She wanted a private room where she didn't have a roommate, and they would have that available for her."

"My husband was at Sylvania Center. He wasn't getting the therapy that he was supposed to get. When I went up there, he was out of it most of the time. When I asked them to remove some of his medications, they said they would one at a time because they didn't know which one was causing the hallucinations and the other things. My husband also had a hard time swallowing. When he was released from the hospital, he was on a thickener, but he wasn't at this nursing home. He couldn't participate in physical therapy because they had him so drugged up, so he had lost the ability to swallow and was not able to walk. He had such a bad infection in his leg that it looked like it was ready to pop. I wanted him to go to the hospital, but the doctor from the nursing home refused to call the squad. When I got him to the hospital, he was so severely dehydrated that his tongue was black. They couldn't get an IV in him because his veins were like glass and his kidneys had failed. Basically, they couldn't do anything to save him. He was in the hospital for a couple of weeks and I had to make the decision to take him off life support."

"The staff at Liberty Nursing Center of Toledo are all part of the care team for the residents, that from the janitor to the director. Everyone is involved with the residents and they believe in promoting participation in care from their clients. The approach is both mind body and soul. Residents are offered various activities which will help them achieve that goal such as religious services for those who are not able to attend church, bingo and many more. Liberty is located in the center of Toledo and is part of a vibrate neighborhood. It's a place where one can see that the staff really cares for their clients. "

"To start off, the facilities parking lot is ridiculous. There is never anywhere to park and sometimes you have to wait for hours to get a spot. The elevators (There are two) are usually jam packed with people and not always operating correctly. I have gone in this facility several times and they had one if not both of the elevators out of service. The staff is not very pleasant either. I would not put my dog in this facility. This is just my opinion, but I have never had a pleasant experience while going to this facility "
