"I would definitely look elsewhere. The building is old and looks nasty needs repair. They are short staffed as people call off and then they require the on duty staff to stay who are already tired. Some of the staff is not very professional. Do not bring your loved ones here. "

"My mother is in Applewood Nursing Center. Right now, everybody seems to be very helpful. You can call and talk to the nurse at any time about your patient, I've even had the nurse Facetime me, so I could see my mom. The outside is very well-kept, and well taken care of. Even people at the hospital and the social worker said "that's a good place." They have a lot of activities for them and dances."

"Paula the social worker is an absolutely amazing lady!Even though she is very busy she makes time for family members and the residents and makes sure the needs and concerns are addressed,she also is very patient and never rushes you .With her being there I know my mom is safe. "

"My father is currently at The Lodge at Taylor and he is getting pretty good care there. The place is old, but the care is good. He was in the COVID section first and it was much nicer there, the staff ratio was better, he got much better care, and they were more attentive. The rehab section is just old. The staff comes in and checks on him. I've never seen them do that at the other nursing home he was in. They ask him if he needs to use the bathroom, he's always got water on his table, and he's clean. The dining area is also nice and this is paid through Medicare."

"Medilodge is a place that my mother was doing rehab before, and in the last year, it has been totally remodeled and brand new. It has switched owners. From what I can see, the place is pretty well run with lots of people working there. She has been there just yesterday, and so far so good. They have short stay rooms and long term stay rooms, and she is going to be there long term. The rooms are real nice, and everything there is brand new. They could probably go try to take their food that they are serving a little bit healthier. It seems like there’s a lot of canned things there, and it’s not good. "

"During our visit to ProMedica Skilled Nursing and Rehabilitation, we enjoyed the people in the rehab department. That's what we were looking at the most. They were cheerful and very knowledgeable. They'd been there for quite a few years. So, it's not like they were hopping from facility to facility. We liked that place. We like the people, all the workers, they were all friendly, caring, and answered all of our questions. The dining area was clean, and the tables weren't crowded. There were no people dining at the time we were there, but the tables were spread out, so they had plenty of room. We were told that we could take dinner there one evening and use the dining room, which we liked, so we didn't have to be crowded in a room. They do games in their rooms right now. They had a game room, but it was kind of blocked off. There was a fish tank in there which we kind of liked. The rehab people go to the rooms and do some rehab in the rooms too. They could order meals in from outside, with no problem. Right now, my husband is still in the hospital, and nobody will take his insurance, so he has to come home."

"Heartland was very clean and seemed to be ok. They have a daily activity time for the folks that live there, they do their wash, help clean them, feed them, etc. This is the only one I have ever visited so it seemed ok, not too much to complain about. "

"I have had a positive experience when I visit Imperial. The staff a Imperial are friendly and helpful. The residents seem to like their staff and I do too. The amenities at imperial include an outdoor garden, and a rehabilitation exercise room. Imperial also has a big screen TV for viewing pleasure. In general, Imperial is a well kept facility and appears to be clean and free from offensive odor. The walk way is well decorated with flowers hung on an overhead passage. There is music playing softly as you enter the door. Imperial seems very inviting to visitors and residents alike. The individual resident bedrooms are well kept. There are two residents in a room. The rooms are situated in wings that are staffed by a Nurse, ward clerk and nurses aide. Residents are allowed to travel about freely and are not restricted to their rooms. Meals are served in a dining room setting and a menu is available to select from. Hot meals are served three times a day. The residents also have several snacks and coffee breaks throughout the day. Imperial also has activities for the residents to participate in. Some of the residents are allowed to leave the facility and transportation is provided. Each Sunday a church group comes to minister to the residents. Bible study is conducted every Tuesday. I think that Imperial takes good care of their residents and that it is a good assisted living provider. "

"My father was at Oakwood. They let him go home with kidney disease. He came home too early. When he got out, he was back in the hospital. In my eyesight, he got out too early. I wouldn't recommend this place again. The rooms were nice, and that part was good. I like the way they had it set up so that the family can come and visit him. We sat in the room with the TV and were able to play games. The physical therapy person was good too. But if my father had an infection in his kidney, he should have known before he left. When he told them he was having a problem urinating, they said he wasn't there to treat him for that. My father also said there wasn't enough food, and it was the same old thing. Cleanliness was OK. It was nice and decent, but I didn't like the way that they didn't treat him for everything. "

"I love the layout of this building, the care is perfect, they always come to me. I don't have to ask for anything! Food is great and on time, I write notes to the kitchen telling them that their food is #1.. I came here in July because I got really sick. I lost 25 lbs. and my heart rate was really fast. I need to regain my strength and get my weight back up. I needed help and this building helped me more then I could have ever thought. This is the best place in the country! Period! This building is #1. This is the only place to come, you will get well here, you will get better! They people here really do care, I've made friends with everybody and I'm free as a bird here. If I want to go outside, I go outside and sit under the trees and take in the fresh air. I have nothing bad to say about this place, the people are so nice. The therapy department is really good. I can go their anytime I want and they are always helping me. The work me not to much and not to little, but just right. If you think this is a bad place then I don't know what's wrong with you. This place has helped me more than I could have ever imagined and more then any place could! Thank you to everybody at Wayne that helped me! God Bless you!"

"My father has been at this facility for the last two months. He has received excellent care from the staff. The staff is very personable and friendly. They try to make it feel like home. The residents all seem happy. They have a lot of activities going on and they keep the family involved in the care of their family member. The food has also been very good. I would definitely recommend this place to anyone. I would stay there myself if I had to. The facility is very clean and well maintained. They also have a transportation van and take regular trips with the patients to different locations in the community. My father has made a lot of friends here and seems very happy. He also receives regular physical and occupational therapy, from a top notch therapy team. The therapy room is beautiful. "

"My dad is in a long-term care facility, Pine Creek Manor. He has been there for three weeks now. The aides who work there are very companionable with the residents. The nurses are mean and short. There are three people to a room in a room that should only hold two people. So, it's overcrowded. You can't visit in the room; you have to visit in the lobby. Overall, I am not happy with the place. To get a day pass for him, it felt like we had to harass, harass, harass, harass, and then even when we got there, he wasn't ready to go. The residents are not a priority in that place. The place is very old, and it looks its age. It comes across as dirty. It may be clean, but it's so rundown that it just looks dirty. We're not allowed in the room. My dad is not eating while he's there; he's not eating well. Medicaid fully pays for it, so if Medicaid fully pays for it, I guess it's worth the money."

"Heartland Health Care Center was referred by the hospital. Actually, this is the third time for my sister. They always treat her very nice. The place is very clean, the staff is professional, and they have a doctor on site. She get nice physical therapy and psychological counseling. She also get therapy with her memory because she has an early stage dementia. Her primary doctor prescribed a certain type of medicine. But all of a sudden they decided she didn't need it anymore and they want to do it their way. I didn't like that, because I thought my sister's primary doctor who she has been with for years knew more about her medical condition. But they are the professionals and I'm not. I can't override what they say. "
