I am a friend or relative of a current/past resident

My father was at Autumn Lake Healthcare at Ballenger Creek a couple of times. The only thing I liked about it was the location. The care there was horrible. Even the people that were there sometimes did not have food delivered for them and sometimes they didn't have utensils they could eat with. They gave my father a medical treatment which was not for him. It was meant for his roommate, which caused him to go into shock. It's a horrible facility. They were not as attentive. They were short-staffed, so they weren't always available. The clothing wouldn't be washed in a timely manner. We had to bring food in for my father because he didn't like the food there. The bathrooms in the rooms were not always clean, but his room was OK. The rooms seemed to be pretty enough for a bed, a chair, a dresser, and that kind of stuff.

I am a friend or relative of a current/past resident

My mom spent the last 5 days of her life here. She had been hospitalized for a few weeks with COPD exacerbation and had difficulty getting into standing position unassisted. My mom had 100% mental acuity. In her 5 days I made no less than 15 calls to various people, including the director of nursing. My mother was immediately put into a diaper (the reason for at least 5 calls) had to ASK for her breathing treatments (7more calls) and ASK for bandage change on a wound that she came from the hospital with. More calls. I was there 4 out of 5 of her days there and spoke to nursing supervisor, her various nurses and social worker about the lack of care she was recieving. All to no avail. On my mom's last day of life, I received a heartbreaking call from her BREATHLESSLY begging me to call Autumn Lake and get her a breathing treatment and someone to get her off the toilet! The call was at almost 11am and that breathing treatment should have happened by 8am. The last gory part of this story is when I went into the facility to arrange for mom's final transportation.....after giving the nurses station the documentation, I went to my mom's suite for one last visit. The stench was horrific, they hadn't even cleaned my mom from what the body does upon death. I felt terrible for mom's suitemate to have to have that smell of death all around her. The just closed the door and left the poor lady in there with my mom's dead body completely devoid of dignity. My greatest regret is entrusting my mom to this place.

Coming from the Hospital to Autumn Lake Ballanger Creek, I was embedded with hope and deepest gratitude after being alive and now in the path to recovery. I was welcomed early every morning by a group of great multidisciplinary professionals, always warm and caring, positive and smiling and making the miracle of life and full recovery a reality. Thank you so much to everyone, you’ll be forever in my heart and soul. Maricela

Ballenger Creek Center was great. My mother had a good time there and the staff was nice and they seemed pretty on it. She did OT and PT and she didn't really participate in anything else. It was clean and nice and the people were happy. The food she got was good.
