"Daily update of care especially when there care is being monitored. I am very pleased with the way the rooms are decorated for comfort. My interactions so far with the staff has been good. I did appreciate when one of the nurses called me with a changed in medication. However I am very upset with the laundry service. Missing my relatives clothes and now I have go buy more and another chance for them to come up missing. They have to do better and especially when his name is on his clothes. No activity is needed at this time."

"My mother moved into SKLD Plymouth. They are very caring and very thorough. The facility is clean. They have knowledgeable staff members. The food is good, and my mom is eating it. They do have activities, but my mom's unable to participate. I love their group dining. You can get your hair done. They have crossword puzzles and coloring. I haven't seen anybody unhappy. Everything is covered because my mom is a dialysis patient, so we don't have to pay anything. They make adjustments where needed. If something is off track, they try to ensure that it gets fixed immediately. For example, when my mom gets back from dialysis, they try to make sure everything is warm and ready when she gets situated. I appreciate that."

"Our Moma ilhas been in Four Seasons nursing home 4 times for rehab, we are very pleased with the quality of care, the PT dept, the activity center, Moma loves playing Bingo and I'd course the 2 best nurses ever...Janet & Stacy. They're the most hardworking nurses ever, they always have a smile on their faces, willing to answer any & all questions or concerns even if they are the only nurse on duty..which is a shame, because having one nurse to a whole unit it a bit much to ask..but you'd never know it from them...I think they need to be recognized for their above & beyond call of duty!! We couldn't ask for any better...also the Dr. (Sam) is just awesome! She takes her time to really evaluate our Moma, & even answers all of our questions or concerns. We have been very blessed to have such great staff taking care of Moma all 4 times she's been here. "

"I got my mother into Camelot Hall. I used to do eye care in nursing homes, and this was one of the nursing homes that I used to go to. I have seen the care they provide for the residents in there. The staff is so nice. Before I put her in there, as I took her and had her eye examined, they found out that she had really bad cataracts, they arranged for her to see an ophthalmologist and sent an aide with her to the ophthalmologist because I wasn't able to be there, and they also arranged for her to have the cataract surgery and then sent an aide with her that day because I couldn't be there with her that day either. My mom said they have really, really good food. She is pretty picky, and I chatted with her while she was eating dinner, and it looked very good. They play little games, and if she wins the games, she gets her nails polished at the end of the game. The whole staff is very, very caring. I know my mom is safe; it is a safe place. "

"Upon entering St. Jude Nursing Center I was welcomed with friendly staff. My mother felt very comfortable with the facility with not only the staff members but other residents. I absolutely adored the dedicated staff at St. Jude Nursing Center. The therapy team helped my mom walk again! Its a very lively place during the day. Activities are always going on. The social worker helped my mom get into subsidized housing! The staff at St. Jude Nursing Center really took the time and dedication to help us throughout this whole process! "

"This facility is very clean. It provides high quality nursing care. I am an ICU nurse and am qualified to address this issue. They are a very conscientious group of health care providers including PT, OT, and nursing. It is a a warm environment that treats people with respect, including family and friends. The food is not so good but they make every attempt to provide alternative choices. "

"My dad is in a long-term care facility, Pine Creek Manor. He has been there for three weeks now. The aides who work there are very companionable with the residents. The nurses are mean and short. There are three people to a room in a room that should only hold two people. So, it's overcrowded. You can't visit in the room; you have to visit in the lobby. Overall, I am not happy with the place. To get a day pass for him, it felt like we had to harass, harass, harass, harass, and then even when we got there, he wasn't ready to go. The residents are not a priority in that place. The place is very old, and it looks its age. It comes across as dirty. It may be clean, but it's so rundown that it just looks dirty. We're not allowed in the room. My dad is not eating while he's there; he's not eating well. Medicaid fully pays for it, so if Medicaid fully pays for it, I guess it's worth the money."

"The Maple Manor kind of has a private room, therapy was very nice, and it didn’t smell like pee. The staff was wonderful, very nice, and helpful. Everything was OK and the only thing I did not like about this place is their social worker. She had a really hard time being a social worker. It had a homey atmosphere. "

"I love the layout of this building, the care is perfect, they always come to me. I don't have to ask for anything! Food is great and on time, I write notes to the kitchen telling them that their food is #1.. I came here in July because I got really sick. I lost 25 lbs. and my heart rate was really fast. I need to regain my strength and get my weight back up. I needed help and this building helped me more then I could have ever thought. This is the best place in the country! Period! This building is #1. This is the only place to come, you will get well here, you will get better! They people here really do care, I've made friends with everybody and I'm free as a bird here. If I want to go outside, I go outside and sit under the trees and take in the fresh air. I have nothing bad to say about this place, the people are so nice. The therapy department is really good. I can go their anytime I want and they are always helping me. The work me not to much and not to little, but just right. If you think this is a bad place then I don't know what's wrong with you. This place has helped me more than I could have ever imagined and more then any place could! Thank you to everybody at Wayne that helped me! God Bless you!"

"Staff is friendly helpful and caring in assisted living. Mom is happy there. Her room is spacious and she has it to herself. Charlotte RN Kaye Bryan Oscar have been a blessing to us in her care. There are more staff members also but I don’t remember their names. Front office staff as well. The facility is clean and inviting. We feel safe and comfortable with Mom’s care. "

"Whole building was clean and orderly. Had a "homey" feel to it. Rooms kept clean and sanitary. Most meals were prepared and delivered hot/warm. Much better food then given in hospital. Good variety offered. ONLY main concern was the midnight shift of staff. Either they were understaffed-or just unresponsive when I used the call light, and would wait indefinitely for help to arrive. Once I could get out of bed and use the walker, I would search the building until I found a staff member and convey my need/problem to them. Otherwise, I may never have gotten help!!!! Day, afternoon and evening staff was the opposite and usually quick to respond. PT and OT were very thoughtful and through. Exercises and strength-building rapidly improved my condition. Saw the doctor regularly-three times a week. Social Worker and other staff were very cordial and very helpful"

"I visited my friend in this facility. I found it to be attractive, even smelled ok! The staff and administrator were most helpful, warm and caring. "

"Amy's Place was outstanding in every possible way. It had a very calm atmosphere. The staff was always right there 24/7. It was very clean, exceptionally well-organized and the care was outstanding. When my mother first arrived, she knew she had come to a new place and she was very happy and she liked the people very much. I interviewed 10 different nursing homes and 10 facilities and it was the finest place. "

"It's a good place to stay. Staff is very good , nurses, cena are very helpful and kind . I like activities here , everyday have fun feel like home. Thanks everyone for good care. "
