This facility was very good. They went over and above the job description. My huband was in rehab for 30 days. They really worked with him and our family to get him able to come home. The food was really good, not the usual, out of the can variety. Even though all of the staff was great, I must give an over abundance of thanks to the B Wing staff and Social Workers. They pointed me in the right direction on a lot of matters I did not know. Took the time to help me get the paper work in order. It was very clean and did not with the usual odor of a facility like this. They were always showing kindness and courtesey to our family.

Highly recommended staff was very nice & helpful. Therapy was very hands on with my mother she had a hip replacement & had short term stay in the facility, she’s back home & doing great.

I would say that the rooms are 3-4 clean. The patient really has to work with the nursing staff here. They will prescribe medical if needed, what ever, this place takes care of all ur needs. House doc is fantastic. Food, ahhhh!!! Let's just say, I hope they r working on that. Some meals r a 4 star restaurant & others, not so good but they do have a menu so u can also order from that. The chef is great & this entire staff tries really, really hard to meet all your needs

The rooms are very spacious in two halls that have been remodeled. The other hall has older style rooms but are very nice and clean. The food was great and so was the staff!! I would recommend this facility to anyone. My friend had a managed care plan and they only let him stay for 2 weeks because therapy did such a great job.

My brother is in LakePointe Senior Care and Rehab Center. He's improved and he's coming home. In the last couple of days, I did find that the attendants were very caring. One of the afternoon nurses was more informative than any nurse that I've ever seen there. My brother was a new patient. What I didn't like the most was there was no information about his care, and the labs were sketchy. It took forever for a lab to come and take the blood to do lab work. Then it took the following day to get results. I didn't like the lack of information. It was a very clean facility. The room was a good size. He had a privacy curtain that he could use any time he wanted. He liked the food there. There's more variety and better taste than the place he was at. The facility was pretty good. He checked in on a Friday and I was there that Saturday, the very next day, and I wanted to see somebody in administration, and nobody was there. They're on a weekend off. When I finally did get one of his paperwork, it showed that he had supposedly signed a contract with them that Saturday with who's in charge of admitting and that didn't happen. That was a total lie because it was not his signature at all on that paperwork. I didn't like that at all, but I didn't say anything to them. I just kept it to myself. I told my family members about it. I don't have anything favorable. If he ever got in that shape again, I would never allow him to go to LakePointe.

DON’T SEND YOUR LOVED ONES to this place!! They lie to you, their director of nursing refuses to take calls! The director of the place is a rude and uncaring. No phones for patient to use in their rooms, when you call there they won’t even take a phone to the patient. They quarantined for two weeks even though my mother was vaccinated and had a COVID test at the hospital which was negative, My mother passed away within days of going to this place and they didn’t even call us when she coded. When we arrived to drop off a phone charger we where lied to that she went to hospital for one issue and when we get to the hospital we are told she passed and it was for a completely different reason! Then they say we are not in her emergency contacts, but then their laundry manger calls a few days later and tells us to say she checked in with nothing and said our number was in the file. I requested my mother records the director says I can request them And their attorney will review my request. My mother checked in on a Thursday and passed on Sunday was never bathed. STAY AWAY from here.

They almost killed my mom! Don’t send any one there that you care about. Her nurse only returned one call to me in the 6 days she was there. There aren’t phones in the rooms and there is the typical 2 week quarantine, so if your loved one doesn’t have a cell you have no communication with them for 2 weeks!

The staff are friendly and the residents that are not immobile seem to be happy. My mother-in-law is well able to get around on her own without a wheel chair and is one of the lucky residents who do not need assistance. However, I would recommend if you have a wheel chair bound loved one unable to get around on their own, that you be there to assist with their care as much as possible.

My mother in law was in hospital,and after seven days of staying in hospital Dr sent her in nursing home for rehabilitation (means get patient in better shape and condition),secon dday of staying in this facility she fell I received phone call from one of their nurses telling me that she fell ,and how upset she is .I offered to come and see how she doing ,nurse told me there is no reason they calm her down , they put her back in bad .First thing in morning instead going to work I went to nursing home to see how is my mother in law.Soon I saw her I knew something is wrong.She was laying flat on her back,I tried to put her in siting position she started to scream,telling me that she’s in pain,she was very confused that morning .One of their nurses came and I asked what’s happened,she told me what’s happened to her it’s happening to people with dementia.I was so shocked hearing that.My question is what is purpose of rehab?When they see that she is in horrible pain they sent mobile X-ray in her room,result was broken hip.She end up in hospital with left hip pinning,month after suffering she passed away.Before you sent love ones to this kind facility make sure you got right one,this one isn’t,sorry for this review but I trying to help other

My grandmother broke her pelvis and arm in a fall. She was placed at Lakepointe for rehab. She was left almost 15 hours without her pain meds and not told why. She on two occasions in the night called for a bedpan and was ignored for over 45 minutes until she had accidents and then was left dirty for just as long before she as cleaned up. When she arrived no one was told she was coming by the prior shift so no one expects her and they had to call the head person at home to find out why she was there. She had come right after dinner and was asked if she had eaten, she said no. No one fed her and when she later asked for maybe a few crackers or jello and another pillow she was ignored and never got them. When she was fed the next morning she received two pieces of hard dry toast, a hockey puck of sausage and a small glass of milk, that was all. It was inedible. At lunch she was given a small bowl of soup with crackers and nothing else. We even had to ask for a spoon. I had an ambulance transfer and had her back to the hospital way before dinner. I could go on but these are the lowest points. She called me a little bit ago and said she has been asked by several doctors and other staff what happened and no one was surprised and that they have a bad reputation. We didn’t have time to do much research before placing her there but we are diligently reviewing where she will go next. PLEASE do not place your loved one in this horrible place.

Lakepointe Senior Care and Rehab Center has just renovated the whole area and separated it from the nursing home section. I had excellent care there in the rehab center. They had everything, including patient involvement in different activities, and a beautiful kitchen and dining area. It was a beautiful facility. They’re very friendly people who very much helped me.

My brother has been in this facility for a few months and I am more than satisfied with the care he has received. He has gotten so much love and compassion from each staff member, it was amazing to see. I came to the building every day to visit and from management to the nurses and aides, I was pleased to be around people who genuinely loved their job. While he was there, he got PT and OT and he has made great strides. The doctor was so great while he was here, that he hopes to follow up with this doctor after he leaves the facility. I felt comfortable and welcomed here and my family is so thankful for what LakePointe has done for him. He has come a long way and we know feel like it is time for him to come home. My heart is so warm thinking about the time we had at LakePointe, out of 5 stars, I would give it 10!

My father was sent here after a short stay at the hospital. The staff was very rude, and my father sat in his own feces without being changed for more than ten hours. I know because I was there the entire time with him. They delivered his food to his room, and he was unable to fully lift the cup or fork to his mouth. No one there assisted him in doing this. They simply set the tray of food down in front of him and came back and picked it up several hours later, after it sitting in front of him and being unable to eat. If I were to rate this home, I would say they were criminal and neglected my Father. We had to call emergency services for an ambulance to transport him out of the home to a hospital to receive proper nutrition and fluids. Do Not Send ANYONE to this establishment. There are several Doctor's on staff, but they all work at different homes and the Doctor's saw my Father for approximately sixty seconds maybe once a week. I would not take anyone here as they would be left to their own devices and would most probably be left for dead as my Dad was. Save yourself the time and aggravation and find a good home right at the beginning. I would not recommend this home to anyone, EVER!

As being a nursing student I have always kept my eyes open for experiences on how to become a great nurse. After being at this facility I definitely know what kind of nurse I will never want to be. To start off the facility itself was an older building but that is not what started this negative outlook. After entering the building I told them I would need a wheelchair for my dad since he is still unstable with his balance. They made us wait in the car for a half hour before we got a wheelchair just to simply wheel him to his room. After getting situated in the room, everything seemed fine until the family and I realized we were waiting for the nurse to admit my dad to get information about his plans and so forth but nobody came. I saw a nurse fly on by and asked If I could speak to her but she told me she would be gone a half hour since she was on her break and when she came back she would admit my dad. Half hour turned into... 2.5 hours!!! In the meantime of waiting my dad had to use the restroom twice but since we were not seen by anyone at this point, we did not have any assistive devices to go to the restroom so I took him myself. The call light was on for an hour prior and a CNA finally came to our room and asked us if everything was alright, I was on fire at this point and knew if we were not there my father could have fell by simply trying to use the restroom since no one was there to help. If you have a family member sent to this location please save yourself the trouble and do not take them there. They do not deserve your money or time. Needless to say they did not get any of our money.

I was very upset that my mom had to suffer through a stay here. My mom broke her ankle an was to rehab here, and the night mare began. My mother had food poising and was left in her on bowel for 2 hours. My mom had to purchase her own medical supplies while she was here. I will be contacting Medicare in every effort to shut this place down.

This facility is for"profit only"! Your loved one may be loved by you but if "you care" anything about them , Do not let them be Placed here! They have"no compassion" My Mom had been a long term resident for two years, totally dependent on the "skilled nursing staff.Dementia and Heart disease were her biggest complaints but she was basically docile and bed-ridden for the last two years! She became I'll and had to be hospitalized in March and April 2013 and before she could get out the door , they had packed up her belongings and assigned another patient to her room! The worst part is that after my Mom "recovered" and returned to this facIlity, they did not reassign the patient in her room"they chose to play musical chairs with my mom!when I inquired about the changes I was lied to by staff and ignored by management! No apologies, explanations or recourse was given and I removed my Mom ASAP and she is being"well taken care of to date" I forgot to this was done even though "I visited with my Mom "daily"! I did the research and toured the facilty prior to my Mom's placement and they were rated five star accoring to medicare.gov ,two years ago. Their rating has since go e down to two star according to Medicare.gov. If you care about your loved one, "run" from this facility. The have no compassion for your loved one , it's all about the "money"
