My mother had rehab in Cedar Mountain Post Acute Rehabilitation (Braswell's Hampton), and our experience with them was excellent. I was very happy. They had bingo going on all the time, music, movies, manicures, and every day there's an activity. They're under a very expensive remodel right now, so part of the place had been remodeled and part wasn't. The care that she got and the people are very loving. I visit there every day and sometimes at odd hours, and everyone was very attentive. I really like that they had six rooms with three on each side of the hall, and a nurse and an LPN always in the middle. If anything is needed, they're right there. The rooms were adequate in size but needed a lot of work. The paint and the bathroom were a little outdated. They were working on that when we were there. The staff was wonderful, really nice to my mom, friendly and great. I really liked the staff.

I visited Braswell, and the place is dirty and it smells. The staff is very disorganized and rude. The food is nasty, and the rooms are dirty. The facility is cheap, but I wouldn’t recommend it. The cleanliness of it would probably be the number one thing to improve on.

This facility is one of the Braswell Family facilities. This is a skilled nursing facility in the city of Yucaipa. I've visited this facility several times to visits patients and to meet with families that have just placed their loved one. There is a parking lot located in the front with additional street parking. When you enter there is usually a person sitting on the left at the desk to greet you. They can also assist with questions if needed. Further into the facility is the social service office on the right. The social worker is helpful too. Centrally located is the nursing station which is a busy place coordinating care of the patients. When I visit i usually see patients roaming about in their wheelchairs or sitting in the halls. This facility accommodates hospice patients and those who need rehabilitation. I observed a dining room and a room for activities such as bingo. Unfortunately, this facility has the smell of urine in some parts of the facility, however I always observe staff cleaning and cleaning. The nursing and social work staff I've interacted with appear knowledgeable and attentive to the patients
