Rooms have a cold feel to them, dark colors. Staff wasn't attentive. Activities are not engaging.

I go to St. Leonard for water aerobics, so I'm already familiar with it. They have a beautiful pool and the recreational facilities are good. I don't care for their housing as much. Their housing is OK, but a lot of it is very small. The food was OK. Their staff was very good. They have good programs both for physical well-being as well as other things. They have lots of lectures and activities. It's a very active place.

This place kept my mom sedated so she wouldn’t complain . Pushed her to do physical therapy before realizing she still had pneumonia . Didn’t take trash out or empty her bedside commode regularly .didn’t do breathing treatments regularly or treat bedsores! Too many patients not enough nurses . I could go on all day . My mother passed 2/25/22 after returning to hospital . Do not recommend this place

My sister-in-law lives at St. Leonard. They have a church and bus service for those that live independently and do not drive to the local stores and shopping centers. She has not used it because of Covid, but she would when that is over. They have cards and drinks even until now. She had one of the cottages and now she's in the apartments and she has been there for six or seven years now. I think some of their assisted living and rehab areas need to be updated. The assisted living is in a separate building. The staff members I've come in contact with were very friendly and competent. Everything's been kind of downplayed because of Covid. Before Covid, I had eaten there a couple of times in the dining room with my sister-in-law.

St. Leonard seemed very nice. The staff all seemed to really care and enjoy their job. You could tell by their attitudeswhich ones didn't want to be there. It seemed like it was a very well upkept and clean place. They had a lot of walking trails, and they had an atrium space that you could go out and sit. My mother-in-law was one that liked to hike a lot, so that was something where she could get out. She felt that she would be able to do more there. They're wonderful. I used to go pass the building on the one side of it on Centerville Station Road every morning on my way to work. The staff all seemed very caring, had every answer that they could give, and seemed like they did a good job. My mother-in-law was put in St. Leonard for rehabilitation. When I went to see her one night, they were just bringing in her dinner; it was pretty good. They seemed to have done a pretty good job with the food. She's one that doesn't like fried food and doesn't eat meat. All their vegetables and their salads were very well done. They had roasted squash and cranberry casserole, and it was pretty good. She seemed to enjoy the food, and it was all things that she liked. The facilities were wonderful. There wasn't a lot of commotion or anything around there considering it's only about a mile from a high school. She was there for a week and a half for the rehabilitation, so I got to see that.

St. Leonard looks like they're going to be a good option. It's nice. It's more my friend's style of living. It is expensive, but that's not her concern right now. We looked at independent apartments and independent cottages. The grounds were nice. Everything was clean, and it looked very safe. It's in a nice area of town. There were activities there also. They have nature trails that people can walk, some social gatherings and some classes like arts and things like that. They have Bridge and a dining room and all that good stuff. The staff was very good and accommodating.

My mom is staying at St. Leonard Nursing Home. The appearance, the feel, and the look were all good. The grounds were well kept. I'm kind of iffy on the staff, but the social worker there is very good, helpful, and excellent. She was staying on one side of the facility, and their staff was excellent, but after the insurance stopped paying, she was transferred to the other side. I'm not too pleased with that side. They were supposed to be doing physical therapy, occupational therapy, and speech therapy. I did not see the dining area or the food because when I usually visit, it is in between meals. I'm not there when they serve meals, but I know that she's losing weight. I don't know if she's eating or not eating, but they say she's eating. That's one of the reasons why I'm bringing her home, because I'm not pleased with how well she is recuperating. She's still weak, she cannot stand up on her own, she can barely walk, and that's not how she was when she went there. I am not pleased with the nursing staff, but not the other staff. The nursing staff need to be more in tune with the family by doing things like calling the family, keeping up with the family, and letting the family know what's going on with the patient. I understand they have a lot of patients and everything, but when you try to call into the nursing station, it's like you can't get a hold of them, and even if you did, it's like they don't know. There needs to be more understanding with the family.

My dad is a current resident of the nursing home (2/2021) and with covid, I have not been inside the facility. However, my dad is very happy with the care he has received so far. The nurses check on him often and have gone the extra mile to help him when needed. He says his room is a "good size" and the shower has easy access. Since I was not able to get him settled in after he left the hospital, his doctor called me to address any concerns I had, go over his medical history, and medication list. He even gave me his cell number. We have had some bad experiences with other facilities and we've been very happy with this one.

Independent cottages are great--well maintained and responsive maintenance staff. Nursing home has been chronically understaffed for the 6 years we have had family there. Many staff caring, but come and go. COVID brought the facility to its knees and difficult for our family member, because family could not advocate for basic needs. Assisted living much the same. Lack sensitivity to psychological and physical needs of residents--bathing + grooming missed, activities curtailed, etc.

Have Father-In-Law in assisted living and most recently now in Health and Rehab Center. For over 10 days now the family has tried for hours to make phone contact to check on current condition and 90% of the time can not get him on the phone. Have placed several request to speak with a nurse and can never get a return call. Phone service and concern for family trying to make contact and check on patients condition is extremely poor

When calling Facility they are extremely difficult to get a hold of and do not answer phone calls. Staff is very slow and even inattentive.

It is with great sadness that I am writing this review but feel I owe it to others who are searching for rehab facilities. My mom was in St Leonard's rehab and I honestly cannot say one good thing and would never recommend this facility. There was not one employee that was in the right seat as far as employment. The staff did not care and was obvious they were there for only the paycheck. My mom was not mobile and would have to wait extended periods of time for bathroom help and every day and night was short staffed. She was scared to say anything for fear of mistreatment. The food was horrible and the room was dirty. This was an absolute horrible experience for my mom and our entire family. It is worth your time to read reviews and carefully check out facilities.

My mother lives a cottage which is very nice. She moved here to set herself up in the event that she need assisted living or rehabilitation, that she would not burden her family. She had to have a extensive back surgery which gave her the opportunity to use the rehabilitation unit. When she arrived at the center from the hospital, two men came with a wheel chair(arm was held on with tape) to get her out of the car. Upon entering the rehab I notice how dirty the carpet was. Her private room was furnished with old furnishings. There was ant traps in the corner of the room and their air conditioning was not working properly, you could not breathe in the room. The evening shift there is one LPN and one aide to care for 15 rehabilitation patients, not nearly enough to care for post surgery patients. She was served food trays with ants running on them. She too had ants crawling up her legs, body and brace. Not to mention the food was gross. She is diabetic, her blood sugars were taken in the middle of eating and medication given when the nurse could get to them(randomly). They only wanted to bathe her three times a week. With her extensive back surgery she need to move around, only to wait forever for help. She was supposed to stay there for 6 weeks, after barely making 2 weeks, I am caring for her myself at home. They also sent her to have her back brace adjusted in a 35 passenger bus that was old and pounded her back so badly that it is now fractured and her screws are loose requiring another surgery. The air conditioning on the bus was broken and the windows could not be opened on a heat advisory day, temperatures were unbareable. So our observations with St. Leonard’s is, as long as you are well this is a very nice place to live. It is when you are most venerable, needing extended care that St. Leonard’s is lacking.

I like St. Leonard. They had a good independent living community, but we're going to assisted living. The staff was very friendly and accommodating. The atmosphere and environment were very good. Physically, it's more dated than some of the other places out there, but it's still adequate.

My mom went to St Leonards Rehab Private Suites after a fall down some stairs and a hip replacement. Located in the San Damiano section, which is the newer section, the building looked nice. The furnishings inside (B2) were leftover mismatched pieces. The bed did not work properly and did not have side rails There was no clock or hangers. The room was nice but the furniture looked like something from a garage sale. As for the staff . . . they were few and far between. The nurse NEVER got up from her desk except to give meds. She would not answer a call light or assist in any way. Spent most of her time on her computer and smartphone. There was 1 aide for 15 people. They were overworked and hard to find. Need a 2nd full-time person. From 3 pm - 7am finding and the staff is hard. Therapy is hit or miss. You never know when they are going to show up. OT and PT don't coordinate with each other and you end up getting both back to back. My mom was supposed to 720 minutes a week but instead, it was closer to 400 minutes. They certainly shaved off minutes in an effort to save money. The food was HORRIBLE! They had no variety and it was worse than an elementary school. They routinely served oatmeal and cheap pasta for lunch/dinner. As for visiting guests, there are no vending machines. When I asked a staff member they said they don't want people to visit long enough to need a drink or snack. I didn't want the next family to go there blindly like we did. I hope this helps.

My friend is at St. Leonard. He has no family so I am helping him get along. He is in a lot of pain and needs to rehab from five surgeries in one day. His doctor's orders are for him to receive pain medication every four hours. Every time I visit he is in terrible pain and he is not given his pain medication. I have to hunt someone down and beg them to give his pain medicine. That is not right. There is one nurse that seems to be attentive to him, but that is all. To see someone suffering when it could be alleviated to me is just horrible. I will never go there.

At St. Leonard, we like the feel of the place. My loved one has adjusted very quickly to it. They have a wide variety of activities that are geared to the needs of many residents there.

I went to St. Leonard (St. Leonard Franciscan Living Community). It was outrageously expensive. It would be fine if you’re a Rockefeller or a retired politician. It was outrageous. They want you to buy into the cottages, and they want you to pay for the gym and the personal trainer. There was a 2-hour marketing presentation. I didn’t see the rooms, and the lady I was with said, “Oh my God! I can’t afford this.” But the grounds were beautiful. They had lots of amenities, and they had lots of transportation. They had lots of stuff for you if you could afford it. I would give it a 3 because of the marketing tactics. I just thought the whole thing was silly.

The people at St. Leonard were extremely nice. They showed me their respite care, which has four floors. They advised me that it would be difficult for my husband, who has Alzheimer’s, because although they'd have people checking on him, he could still go out of his room, get on the elevator, and wander off by himself. They are extremely nice to my husband whenever we are there for his daily program and feed him lunch every day. We arrive three times a week at around 11 a.m. and pick him up at 3 p.m.

St. Leonard Franciscan Living Community is giving my wife all the care she needs and the staff has good working attitude. The room of my wife have a bed and bath, closet area and a room for her own furniture. The dining room was very well taken care of too. The food seems good because she eats it well and it looks appetizing. They have some people who come in and sing. They have some activities and television with movies that they show to the residents.

My father is still in rehab at St. Leonard HCC, and I was just there to visit. The staff seemed fine. The social worker has been good working with me, and I talked to the physical therapist there. His room seems to be adequate. I've only been there one time, and he was on a different wing; he just transferred.

I think St. Leonard’s is really nice, and it seems like the nurses and the aides really care about their patients. They’re really helpful. It is clean, and it is close to home. They have assisted and independent living, and they have memory care assisted living -- which my mother is in -- and then they have the skilled nursing. There are only 15 people to a unit, and in the dining room, there are only three to four tables. Right outside, there is a garden that they can go out to. They have crafts, exercise, and entertainment. They also have animals that come in. Sometimes they go out maybe to get ice cream or go on little trips.

The nursing home part of St Leonard was not good, but the assisted living facility is fantastic. They are giving her all the care she needs, and the staff is good. Her room has a bed, bath, closet area, and a room for her own furniture. The dining room is very well taken care of. The food appears good; she eats them well. I haven’t eaten any of it, but it looks appetizing. They have some people come in and sing, and they have movies.

My mom has been at St. Leonard’s, and I think it’s wonderful. She has been there a couple of times like once for about four weeks in the Spring and four weeks in the fall, and we had a very good experience there. I thought it was a very clean facility, and my mom seemed to like the food. The staff was very good, kind, and caring as well.

I would not put anybody there at St Leonard. It was dirty. The tour person was unorganized. Even though we caught them off guard, I don't like to make appointments ahead of time. I like to surprise them because I want to see how they really are. I don't want to have a false sense that they set things up. The patients were not engaged there, and there were no activities going on. The rooms were big, but the facility did not give a warm nice feeling. The staff was average at best. They need to have more activities, and the facility needs to be cleaner.

St Leonard is just right down the road. They are down the street close to my father. It is close to our home and their home. The staff seems fine. Everybody seems great, and I would recommend the place if someone is looking for it. The rooms are clean, and I thought everything looked just fine. They were playing bingo or some sort of cards. The dining room looks clean, and they were eating with their friends and family. St Leonard was a little too cluttered for me though. My mom prefers things in perfect order, and it might have been too cluttered for her. This is from what I saw on that day. It may not be that way all the time.

I stayed at St. Leonard Franciscan Living Community to recover from a complete knee replacement. I have also stayed at two other Living Care/Rehabilitation Facilities in the Dayton area and found St. Leonard's to be far superior in many ways. The nursing staff is responsive to needs; the Physical Therapy specialists are compassionate and also assertive in moving along the recovery process; the food is very good and the location is convenient to both my Mother's home and my own; and the cost of that facility was far lower than the alternatives. They even had a wired Internet connection in my room, so I could feel more comfortable in having a more secure connection to the Internet.

Grandmother lived there for about two years. Staff was very kind in my experience, though at times fairly negligent. My grandmother fell once and bruised a lot around her body when the staff wasn't there assisting her. They did get to know her well, and were very hospitable to my grandfather when he came to visit. Every time I visited they were doing some sort of fun activity, so it's good that they kept the residents busy.
