"My mother is in Founders Plaza Nursing and Rehabilitation for rehab. They're doing a great job with her. I have no complaints at all. They have been very considerate of the special needs that she has. She could still have some social interactions with severe dementia. I told them that at the assisted living where she had been, her joy was to sit out in the lobby and wave at people or walk around on her walker and wave at people. That's no longer possible. Now they take her out to the nurse's station and let her wave at people and be out there and still socializing. She's been so happy every time I've gone there. She's sitting at the nurse's station getting to at least see people and do things with them. They would do things like feeling different textures, putting her hands on a swimming noodle or little simple toys that has different types of closures on it (the chain lock or one that you turn). That's there for her to play with, and there are some balls on the little table. There are about three or four that they will let come and sit there that are becoming her buddies, even though she can't talk to them. They're very conscious of her social needs and her physical needs. They contact me every week for an update on her physical and dietary needs. They update me on all of her needs that are being provided. Their staff is very caring and considerate. I've been there when they brought her food in. I tasted it. It's very, very good. I've had no complaints about the food at all. I don't really remember any problems. They told me that even though she can't participate in any of the activities, they take her in any way for social interaction. They also play bingo."

"My mother had advancing dementia and was in this nursing facility. Before I placed her at Park Manor I met with a social worker and other treatment staff. My mother received excellent care. The staff were knowledgeable, caring, and friendly. I highly recommend this nursing facility. "

"Garnet Hill is a beautiful facility. The outer structure of the building is gorgeous. When you enter there is a piano and a bird habitat that makes you feel at home. The cafeteria is big and has planned meal times and a menu, making it feel like a restaurant. There are monthly resident activities scheduled and daily events! My one complaint would be cleanliness. I feel they should scrub the toilets better and change the resident's diapers more often. I do believe my grandfather is enjoying is time there and that makes me happy. "

"Its a nice place. Place is kept clean. Meds. and Meals are on time. Nurses and Aides are helpful. Housekeeping is good. And the Administrator will personally work with you on anything you may need him to. I thank everyone here for being so respectful. They have on the go activities and in-house activities. Im a long term resident and only 57 years old. But i so need the care thats provided."

"They never answer the phone when you call to check on your love one and never get a call back when you leave a message. If you want your love one to get descent care I would not recommend this nursing home. Never get a call back and always get hung up on when you call to check on your love one and they are always putting other patients clothes on my spouse when he has his own clean clothes in his closet that has his names marked in his own clothes. They don't take the time to read the names in the clothes to make sure they are putting the right clothes on your love one body. They need to take time to read the names in the clothes before they dress the patients."

"People try hard and do their best to please residents. Therapy is a big help for those who need it. Management is great because they explain details of a bill or if you need advice. Paven goes past her obligations to the residents and she puts her best foot forward always. The food is unpredictable, you got to take it day by day. But overall, my wife and I are glad to be here! Activities is triple A plus because Anita goes above and beyond! I think its great that we have transportation to Walmart provided by staff to get us what we need! "

"This facility has been wonderful. The communication of the staff, the care of the nurses and doctors, the cleanliness of the building, and the therapy all exceeded my expectations. My mil has been at other facilities and you can truly see the difference a privately owned and operate facility has vs a corporation. "

"Our mother was at The Belmont in Allen, Texas from Dec. 31, 2019 until her passing on January 22, 2020. When it became clear that she needed a skilled nursing facility we did extensive research to find the best option. This was very difficult because so many reviews of these facilities were filled with negative comments. After physically checking out two, we chose The Belmont. We could not have been more pleased with this facility. At age 94 and in failing health, we did not truly expect recovery and we just wanted Mom to be as comfortable as possible and get the best care she could. The nurses and aides at The Belmont were outstanding. We watched interactions, particularly with the aides, as they dressed, changed, bathed and fed Mom. They were gentle, kind, compassionate and patient. While difficult to watch Mom’s decline, we were comforted in knowing that when we weren’t able to be with her, her needs were being met. The facility is well kept and the administrative staff is responsive and provided whatever information we needed to make the right decisions. Some of the most helpful staff that we encountered were Gloretta, Debra, Thomas, Ashley, Maggie, Angela, and Ngozi"

"My husband was in Belterra Health and Rehabilitation for physical therapy. It was OK, but the rooms were a little small, and the bed was not comfortable. The liaison was very friendly, and they have a lot of activities. I didn't like the food. When they dismissed my husband, they just rolled him to the front door in a wheelchair, took the wheelchair back, and left him with a cane."

"A lot of the staff at Beacon Harbor Healthcare and Rehabilitation are very nice. They are good, and the majority of them are helpful. The place is very clean. It has been around a while. It's not brand new, like a lot of places. It is a nice facility. My mom-in-law is in a two person room all by herself, and they keep it clean. Food is good too. They have bingo and different activities for them to do."

"I feel as though the staff were kind and friendly. The facilities themselves were adequate, but the one thing I did not like was the food that was offered to the residents. It was often either bland or salty, or sometimes, worse. There were plenty of activities that were available to the residents, but the staff needed to be more pro-active about getting the word out that there were activities available, especially to new residents. A calendar was distributed monthly to all of the residents, but the calendar was often inaccurate at times. I liked the front sitting room areas, but the rooms themselves are pretty small. "

"My mother has been at Broadmoor Medical Lodge for five weeks now. The staff members are good. They are helping her with walking. Both my parents were in it before, and they both came out walking great. They have great therapy."
