"The facility is clean, staff are friendly, competent, and effectively communicate with family members in relation to the progression or deterioration of the patient or resident. Staff does well with documenting details in terms of patient care or injury and do not restrain nor over-medicate uncooperative individuals. They allow residents as much freedom as possible while still ensuring their well-being is not over-looked. Several residential rooms allow for individuals to have their own bedroom furnished with their own belongings and furniture and private lavatory, which allow for greater comfort and privacy, beneficial to both the resident and their visitors. Enclosed outdoor space is also available to patients, however, implementation of greater aesthetics and gardening could benefit the space which many residents enjoy. "

"Towne Center has different levels of care. You can move from one level to another if the need arise. The staff is fine. They're good. I have a two-bedroom apartment and it is fine. "

"My mother in-law was a resident and we could not talk more positive about the care and staff . The administrator Candy S. truly has an open door policy if you have any concerns. I highly recommend candy S. and the staff at O’Neil healthcare"

"My father is here. It is better than where he was at. I like it. The main issue was that he could have his pet with him there (because his cat is his whole world and more important than me). It was a more of a homey environment, as opposed to a hospital setting. It's not as institutionalized feeling. It looks like a Swiss chateau. It's nestled back in the woods. I didn't even know they were there, and I've lived in that neighborhood for over 30 years. The staff are excellent. They are really, really helpful. They call me any time anything happens or if they're going to do anything. Physical therapy comes in, and they let me know when they are there. They keep me abreast of visitations and what's going on. I'm very happy with them. My father loves their food. He is happy with his meals. I have seen him doing arts and crafts, but I haven't been up there when activities are going on. They give him a haircut once a month. They just took my father to go get his eye exam done, which was not free, but it was very reasonable. The fact of having that is a nice option. The staff have been phenomenal from day one. My father is in a locked ward. He can get access to the other parts of the facility, but he has to have someone with him. He doesn't have free range of the whole place. He can't go outside on his own because he has to be monitored, so he doesn't wander off. They have an ice cream or a malt shop in there. They have an actual bar in there."

"My dad was at Rae-Ann Suburban. I was not impressed with the facility itself. You walk in and you know you're in a nursing home. It was terrible. There were a couple of staff members there that didn't like their job, or they were just there because they had to be. I think they're shorthanded, and everybody was overworked. My dad ended up with COVID while he was in there, and he was ground zero, yet they still don't know where he got it from. And then they just locked him up in a room for ten days. It didn't seem like they really paid much attention to him. That was a pretty depressing time for him. However, the admission staff and his personal counselor there were topnotch. The patient advocate was really awesome, too. The food was terrible though. They need to improve on the variety and the portion of food. Sometimes they bring them a dinner that would just be a snack."

"My brother is in Westlake Rehab & Nursing Center. He told me he liked the place. He doesn't like the food, but he likes everything else. For him, it's not that tasty, but it looks healthy. The place is very clean, and they take the residents out to activities which was great. They have several levels of care."

"To start off, my grandmother is from Youngstown, Ohio. She sustained a fall and afterwards loss some mobility--due to a broken leg and loss some cognitive abilities. Luckily, she had great insurance and my sister and I set her up at this facility. We were impressed with the staff, the room and the overall accomodations. Clearly, the manager and the staff were familiar with such a difficult process and made it work smoothly for my sister and I. We were given the rules but not in a way that made you feel like a kid but in a way that was designed to show you how they do things in their facilities. They set my grandmother up with a exercise therapist who was to help her daily in trying to keep up with her movements. They also kept sheets that would document her progress so I was always informed as to what was going on with her even when she could not respond. Overall, my impresion and the facility was with overall staisfaction. My grandmother was there for a couple of months before she passed. The overall sutuation was very heavy on the heart--draining emotionally and physically. But I have nothing bad to say about the facility. the best compliment I can give is that I did not notice them which means that they did not hinder the process in such a stressful time. "

"Huntington Woods Care & Rehab Center was very clean. The wall painting and the decorations were very comforting and welcoming. The gentleman who gave us the tour was very knowledgeable. The rooms were large and they had private rooms with furnishings. So, they had dressers, end tables, and chairs in them. The dining area was clean. Residents were watching a movie when we went early in the afternoon. They also had a large room, like a physical therapy room, an outdoor patio, and areas to sit."

"My mom was there for 4 weeks . She was in very bad shape when she got there. Even though my mother doesn't understand or speak any English, nurses and all the stuff were more than nice and understanding towards her. Care they provided for her was outstanding. I felt 100% safe that much mom was in safest possible institution. I would highly recommended to my family and friends. "

"I honestly think that Lutheran Home was one of the best possible options for my grandmother. I didn't know much about nursing facilities at the time that my grandmother's health started to decline, but the individuals at this facility were more than helpful in the transition. The nursing staff and social workers were friendly and the doctors that saw my grandmother appeared to be knowledgeable about what they were doing. It seemed like all parties that were involved with the care of my grandmother truly cared about her health and well being. This seems like it would be an obvious trait of nursing facility staff, but with all of the residents that were staying at this facility at the time, it really shined through that Lutheran Home was among the best places for my grandmother to be. I was only with my grandmother during some of the daytime visiting hours, so I cannot comment on the evening staff, but what I can say is that I was very pleased with the individuals that I did have a chance to talk with. This was a very difficult time for my family, as my grandmother's health and mental status were declining rapidly, but it was made easier by the wonderful staff at Lutheran Home. "

"My father is in a nursing home called Avenue At North Ridgeville and so far, he likes it. I think that all nursing homes are kind of short-staffed so I would say staffing is somewhat of a concern but not a major one. Other than that, it's a very nice and clean facility and they treat him really well. The food is good too and he's happy there."

"My mother and my sister have been at this facility on several different occasions. They have always received excellent care and were able to return home. The therapists are very motivating with a great deal of compassion. The nursing team was on top of all their care needs. The food was very good and they had lots of choices. On top of that there was plenty to do when they were not in therapy and the outside yard was a nice place to relax. The staff was amazing to work with. Thank you Life Care for giving us so many positive experiences. "

"We like Joshua Tree Care Center for the location and its very good service. The one downside is they don't have any type of courtyard that's secure for people to sit in but other than that, we're very happy with it. The rooms are on the small side but nice. My loved ones are in a one-bedroom that's fine and fits their needs. The dining room is nice. They're in the assisted living part. "
