Our experience with Fairview is nothing less than wonderful! We had our doubts at first because of the reviews but we took the “risk” since hospital SW told us their PT is good plus they provide free transportation. We’re thankful to Fairview and all the staff who made my dad’s stay as comfortable as they can and his discharge as soon as possible.

My grandpapa got discharged over the weekend. I must say, it was a smooth process from admission to discharge. We got a call once he got to Fairview. The RN discussed what has to be done and they were nice to answer all our concerns. He did not have a hard time during his stay as this facility promotes inclusion in a sense they have diverse staff. It made my grandpapa more comfortable to speak with his native tongue. Thought that we’ll have a hard time dc him since it was the weekend. His discharge went smoothly, everything went well. Very happy with the service of this facility. Giving them 5 star. They gave a stellar service to us!

My Aunt got recently admitted to this facility and we appreciated how the staff informed us of what treatment is being done and the Rehab team was always there to answer all our concerns. We were blessed to be roomed in one of the newly renovated area and my aunt loved it. The hallways are well lit and clean and the housekeeping always pass by to collect the trash in her room.

The staff members were friendly and helpful. I noted that they were pleasantly interactive with the resident that I was visiting. The surroundings appeared very clean and uncluttered. My visiting experience was very good.

Had a tour in Fairview prior deciding on placing my aunt. Staff was very knowledgeable of the facility and its services. I saw rehab dept. they have group activities for the residents. The building is under renovation but cleanliness is still on par. Overall we are happy with the care provided. Any concerns we had were properly addressed. DNS and SW were very helpful.

I spoke to employees here about placement for my dad. It took many tries for them to answer the office phone but the lady was nice and even gave me a mobile number so I can get in touch more quickly. They were able to take my dad’s insurance and this is an important factor for us. They even provided transportation to the facility after his discharge. The facility is a little outdated but they accommodated most of my dad’s needs.

I am currently a short term resident in the facility for Cellulitis of right lower limb s/p toe amputation and because I contracted COVID-19. Its far from the best, food is what you expect from a nursing home, nursing department has its ups and downs. Some staff are aggravatingly slow, some are the best and will go above and beyond for you. I'm here for rehab and so for me that's the main topic for this review. I cant say anything bad regarding the staff as they have been more than informative and helpful for me. My only wish is to increase my workout days and frequency as there are usually no rehab in the weekends. Overall 4 stars for providing what I need to get better so I can go back home.

My uncle has been here for months, and Ive visited several times. Its far from perfect but it is a nursing home. The staff are mostly great, they listen, they care. I had issues with calling extentions and no one was picking up in a timely manner, but the supervisors were kind to give me their direct numbers so now I can reach out to them easily for any concerns. The food is average for a nursing facility, but the rehab team has been great to us, the nursing team has been more than helpful, and the recreation team has been a great assistants to any little things we need. I would say they definitely have room for improvement, but overall me and my family are satisfied if not very thankful for the Fairview Team for everythiing they have done for our uncle.

My Mom stayed at Fairview for 5 weeks while recovering from a fall. I cannot say enough good things about the place. The staff was kind and attentive, the facility was pretty and clean and the food was very good (according to Momma). They were very responsive every time I called and they communicated with me often. They even had a warm goodbye for my Mom as she left. The staff lined the hallway as she was being wheeled out and they rang a bell and waved. She truly enjoyed her stay there and if she ever needs full time nursing, Fairview will be the first place that I call.

This is the worst place to send anyone! It's more than horrible... dirty & old, rusted equipment in the bathrooms that you have to share with a room of four (disgusting). 99.9% of the staff is untrained and act as if you're bothering them if you request even simple service or items. Most don't speak english and have such heavy accents you cannot understand them, plus get angry if you ask to repeat what was said. The activities here only include a game of bingo in a dirty dining room. All of my packages come to me roughly torn open with items missing. Whatever food I had stored in the tiny frig has been stolen several times so that eliminates having any decent food as the food served here you should seriously not feed your dog. DO NOT COME HERE!!! DO NOT SEND YOUR FAMILY HERE!!! WORST SO-CALLED 'NURSING HOME' IN NYC!!!

My father was recently admitted here from LIJ. They sent him to a clinic two days in a row without informing the clinic he was coming! Completely wasting time! I confirmed with the nursing supervisor Kumar that this wouldn't happen again, but it did and he was mistakenly sent to an unconfirmed appointment in the cold... Now he has no refill for his pain medication, and they're prescribing oxycodone. When I go visit in person, they have LPNs giving meds to patients, which is fine, but at least train them to be sterile... When you call them out on it they get angry. I do not recommend this place to anyone. My dad shares 1 bathroom with 3 other patients. It was also very old and not the cleanest.

My brother-in-law was in the rehab of Fairview Rehab & Nursing Home and that was terrible. It's rehab and nursing, but don't send anyone here unless you have to. The gentleman who did PT was excellent, but other than that, it was a nightmare. They don't answer the phone. It's a disaster. No one answers any questions and our social worker never returned our phone call in five months. There were no activities and the food was terrible. The first room he was in was very large and the second room was ridiculously small. The chairs were ripped and it's just terrible. It's a medicated facility. Three people were nice, but 100 people were unresponsive.

they don't tell people anything on what is going on with the patient unless they want money. they had patients in one whole floor contracting covid. my father had a urinary infection and i was not told and i am there constantly. no one answers phones or give the right iinformation or leave you on hold for over 15 minutes. you request to speak with the head nurse and no one calls. case worker KIP doesn't know a thing. and says he never receives anythign until you show him emails its terrible place

The level of care that my mother received during her 10-week rehab stay was adequate, but the administrators for this facility are horrible. They didn't communicate with the family for anything from the time that my mother was admitted. She was originally transferred to Fairview from a hospital for a three-week rehab stint after a fall. The nursing home decided on its own that she needed to stay longer, and to pay for it, they took her off her private insurance and moved her onto Medicaid. They also performed operations to correct her vericose veins, did dental work, moved her from room to room and floor to floor, changed her medications, and a lot more, without ever discussing any of this with the family. Instead, they shoved documents in front of my mother (who is 80 years old and has Alzheimers and dementia), and told her to sign them. There were also some questionable billing practices. When we called to discuss our concerns, administrators never called back. To make things worse, when she was finally discharged, she was put in an ambulette and just showed up at the door, with no advance notice to the family. And, we received a bill from the pharmacy for $231 for medications that she didn't need. I understand that COVID-19 disrupted a lot at the facility, but there is no excuse for not picking up the phone to discuss my mother's care with her husband or other members of the family.

This facility was outdated, dirty,unorganized,understaffed,and smelly. My husband suffered a stroke but was coherent of everything. The level of care he received was subpar, in fact he was dropped twice because of incompetent staff. The food was disgusting and did not adhere to the special diet my husband needed. The food that I sent him from home was lost and never made it to his room. The staff lacks communication skills because I had to keep explaining the same things to different staff members frequently. Much of the staff was incompetent and the facility was understaffed. I am a nurse for over 20 years and I’m outraged for the treatment my husband had to endure. Many medications ,some of them with serious side effects were given to my husband with no clear explanation of why they were prescribed. I asked for more physical therapy and it was promised but never accomplished. To set an appointment for visitation was a hassle. The number provided for visitation no one picked up and finally when you do get someone nothing is available. The few visitations I did get was behind a thick sheet of plastic after a negative Covid test was produced once a week from myself and my husband. Without the touch of your love ones is devastating and discouraging. My strong husband would cry after the visits. The treatment endured there was not fit for an animal and I pray your love ones never need the services here. The only bright spots I had was nurse Annie, supervisor Francois and physical therapist Glenn. These individuals went above and beyond the call of duty.

The lack of communication between staff and patients/ family is nonexistent. They didn’t tell my mother or myself that she had tested positive for the covid-19 virus until I inquired and then didn’t tell me when she tested negative. Received no calls from administration or a doctor when she passed away. Don’t send anyone that you care about here.

please don't send your loved one to this facility, chaotic management, administration is horrible!!! Nurse aid didn't do their job... my dad was there for 10 days for rehab. but actually is hell, made him medical condition even worse than before. this facility should be inspection again by health department of state to make sure they are doing the right to the one need help.

IF YOU LOVE YOUR FAMILY MEMBER DO NOT PUT THEM IN THIS FACILITY. MY GRANDMA WAS HERE FOR REHABILITATION FROM A STROKE. SHE WAS ON A NON SOLID DIET, EVERYTIME I VISITED HER SHE HAD FOOD COMING DOWN HER MOUTH, THEY LEFT THE TV OFF AND HAD HER STARRING AT A BLANK WALL. ONCE COVID HIT THAT WAS THE LAST TIME I SAW MY GRANDMOTHER. I CALLED EVERYDAY, WHENEVER I ACTUALLY GOT THROUGH TO THEM THEY EAITHER HUNG UP OR PROVIDED ME WITH BROKEN PROMISES. THEY NEVER TOOK THE TIME TO PROVIDE ME WITH A PHONE CALL OR A FACETIME. ONE MINUTE SHE WAS DOING WELL AND READY TO GO HOME AND THE NEXT MINUTE SHE HAD A FEVER AND WAS SENT TO THE HOSPITAL. THE HOSPITAL SAID SHE HAD COVID AND COULDN'T SEE HER THERE BUT YET EVERY TIME SHE TESTED FALSE. SHE WAS THEN TRANFERED TO A HOSPICE CENTER AND ONCE AGAIN THEY PROVIDED ME WITH BROKEN PROMISES AND SHE DIED THEY DAY AFTER MY BIRTHDAY WITH OUT EVER SEEING OR TALKING TO HER. THE ONLY THING FAIRVIEW CARES ABOUT IS MONEY BECAUSE THAT IS WHAT I AM GETTING IN THE MAIL FROM THEM BILLS!!! THANK YOU FAIRVIEW FOR YOUR GREAT HELP.

I saw that they offer a way for me to see my loved one through the entire Covid-19 pandemic. They setup a little hugging booth where I can go hug my mother. It was actually lovely and they were the first nursing home that I have seen do this. My mother told me stories of how they would bring her food and they the food was actually good. She mentioned how she did not expect it to be good because it is a nursing home.

They treated my loved one with the upmost care. I remembered that they had a 200 bed facility and all of the rooms were being looked after by at least one nurse. My dad, who was the patient, told me they use to play fun games over the loudspeaker, like bingo. Of course it could not be in person due to the pandemic that is happening. Overall, this place left me and my father very happy.

I am a friend of a patient at Fairview. She was just brought there after 2 months in hospital following a fall at the Hampton Center where she also contracted Covid. She has complained that she is simply being walked around her room without any real rehab and no gym. She is being denied her insulin. She does not have a phone and her family cannot get in touch with her. There is one readily available staff member on the night shift and he is not medically trained. He's an admin. The nurses are nearly unavailable. The food is by her accounts inedible. the room she is staying with has issues with electricity, power and she does not have her own phone service. While many social workers at nearby hospitals will tell you this is an acceptable place, it is not, at least from what I can tell. Avoid this place at all costs as well as the last mentioned place.

My mother has been in rehab before and we didn’t experience anything like this awful place. It is literally impossible to get a doctor on the phone as they don’t have a phone number or a pager. When you ask to leave a message for a doctor, the reception says they don’t have a way to do that. One night when I couldn’t get my mom or anyone on the phone for many hours I was told by receptionist it was because there was only one nurse on duty for the entire floor of about 40-50 patients so I should stop calling. I tried to get mom transferred when she started to decline after 2 weeks of being here for rehab from a broken hip. I could not get anybody to help me get her transferred, despite dozens of calls and messages to the administration to get the referral that is required. When I did get the only one case manager on the phone, and in person on front of the facility, he adamantly refused to help so I finally had to call 911. She arrived at the ER with Covid, a UTI, Malnourished and dehydrated. (It says “malnourished” in the hospital record). She entered Fairview somewhere around 100-105 pounds and entered the ER after 1 month being there at 85 pounds. The day before she left in the ambulance the nutritionist told me she had gained 2 pounds during her stay and was eating 50% of her meals. Please, please, do not use this facility.

Staff are horrible. Can never find or speak to someone in charge. They avoid you like a plague. Super unprofessional. No type of assistance at all. I wouldn't leave my worst enemy there. My unfortunate brother in law went through a horrible emergency surgery and was in hopes to receive at least some type of hospitality at the Fairview Nursing Care Center. All he received was neglect and was mistreated. This is NOT a "nursing home" Staff and especially the social workers need to be FIRED or RETRAINED in "HOSPITALITY" I would NEVER recommend this place to a single soul.

My Brother has been there 3 weeks, I only was able to speak with their staff twice. When I call I get extensions that can't take messages, or extensions that let you leave a message, but I never get a call back. Even left messages with admitting. This has bee unreal. DON"T use this place if you care about your family members well being. I am going to Eyewitness news to get a reporter and make this a news issue.

My family member was in Fairview for Rehab after her fall. We ended with a tab of $0 after the last facility she was in we got left with a huge bill of co-pays. They helped us understand when the co-pay kicks in and helped us with medicaid. It was like a miracle she was better for 0 down.

My dad is very young and we really looked into a place that was not just a nursing home but really a rehab so he could socialize and not feel aged.. We took a tour and checked out social media we were impressed and decided with dad he wanted to go. Dad joined their programs, smoked, and had 2 hours of rehab daily. dad is back home now 2 months later, back at work and fully independent. I thank Fairview for fully helping dad recover, i thought i'd share if anyone is looking for a more young, vibrant rehab.

I am currently a rehab patient at Fairview Nursing home in Forest Hills. Since Thursday PM when I was admitted nothing went right. It took 2 hours to get my medications including one for pain. All day yesterday and today same story for the pain medicine for all 4 dosages. I found out that I was admitted not properly and my mess were not ordered til Saturday. Nurse supervisor was Not attentive and non respondent

I had my granddad in Fairview Nursing Care Center. It’s a beautiful place. The people are very nice, very friendly, and very kind. It has all these amenities that they never get to use. It had a swimming pool and a patio right in the back. They do have a recreational room on the first floor where the active seniors can go downstairs, read a book, and mingle with the other active seniors, but the short staff didn’t allow that. They didn’t have time to take anyone downstairs. My granddad was there for 2 months, and there were only 3 women and 1 male home attendant on the whole floor taking care of about maybe 20 to 22 people. Some of the men wanted to go to the bathroom and they wanted them to pee in a jar. Some of them would give them hell, and they would have to bring in an orderly for the orderly to take them to the bathroom. There’s too much work for them to be individually taken care of. They never took him anywhere. They take him out of his bed, put him in a wheelchair, and walk him to the cafeteria. They put all the senior citizens at tables -- men with the men, women with the women. They would be there all day, no TV, everybody looking at everybody just waiting to die. I didn’t like it. He gets more handicapped there and more helpless. They wheel them around all day long because it’s easier if they ride them around than have them walk. Everything they give them was like baby food. There was no solid food. That’s one place you don’t want to recommend people to.

We looked for a place for my grandmother, so I visited Fairview Nursing Care Ctr Inc. The staff were very attentive and helpful. Another thing I considered was the Russian floor they had which was on the fifth floor. My grandma spoke Russian so she would have a social life as well. The rooms were clean and nice. They had lots of activities like piano and movie night. It would be nicer if they had some outdoor activities. I would commend this place to others.
