    I am a friend or relative of a current/past resident

    My mother was in the skilled rehabilitation area and received outstanding treatment while she was there for 8 weeks. The staff is very caring and attentive. The PT and OT staff were very patient in building my mother's strength back and helping her to walk on her own again after having a hip replacement. The Case Management team was very communicative and kept me up to date every step of the way. The facility, while a little on the older side, is very clean and bright. I would recommend this facility without reservation.

    The management is unprofessional. When asked to do some thing they say they will and they just never bothered to do it. Hoping you will forget. The rooms are not clean. The food is very poor quality. There is a huge turnover and staff. The staff talk about patients and other nursing staff in the nursing station so everyone can hear everyone’s business not professional. I listen to a nurse talking about my family member in a very derogatory way when I pointed this out she simply shrugged her shoulders and walked away when I told management they did not seem to care

    My relative had a very negative experience during what was supposed to be her "rehab". Care was poor, PT and OT were an after-thought. As a big fall-risk, she was left alone at the end of a long hallway, call light ignored, medication mistakes and transported to a pain consult appt without any hx or test results for the Dr who just had to send her back with excruciating pain and nausea. PT/rehab was addressed the morning of her discharge. Booted out without ever practicing transfers from wheelchair to car or anything else. Very unsafe, unfortunate situation.

    Springdale Village Health Care is a very friendly place. They allowed us to bring our rescue dog in. My husband enjoys the physical therapy, and everybody has just been very kind and helpful. The rooms are like hospital rooms. My husband is in a single room that is very clean and well managed. The food is good. There's a restaurant that the residents can go to, if they want, right across the street. They have parties and music outside in the courtyard.

    My mother stayed for about two and a half months doing rehab at Springdale Village. She’s really disappointed in it, and she didn’t think that they did a very good job as far as the physical therapy with her left shoulder and arm. She had a shoulder replacement. They did mostly the therapy with her right arm which is fine; and then her balance was really off. So when the occupational therapist came to her house, she was pretty dismayed. We thought that she was dismissed too early from rehab because there were questions about her safety in the kitchen and being able to cook for herself at that point. They’re saying someone should be there, she couldn’t go back and forth, and that was ridiculous so she started her therapy. She can hardly raise her arms and hand and couldn’t use her fingers on the left hand. The occupational therapist was really concerned that my mother has nerve damage and could’ve been from the first surgery she had. So I don’t know when that occurred or why, but it seems like something in the process there (it’s surgery or whatever), or was the rehab not helping her? It did not go well for her left hand. My mother said they had a nice staff. Also when she was released from rehab, I took her home and I was very concerned about her memory loss. She was totally confused, and she couldn’t go anywhere; I was scared. Her doctor's PA read that elderly people can have trouble with their memory if they have urinary tract infection, and when we checked sure enough she did. I don’t understand why the people at the rehab did not realize that because when she urinated it was brown. That could be something common with elderly people; was that neglect? Certainly they need to improve on the patient’s evaluation and with communication. I also said to paint their walls with a different color, maybe pastel. It was a rust orange brown; it was gloomy. The place was clean, and the halls were fairly wide; it was older. My mother’s room was comfortable. It was spacious enough for two patients.

    I am a friend or relative of resident

    The salesperson did a great job of touring and the facility looked great, but once our relative was transferred to the facility, he was unsafe (left alone without alarms on his bed or wheelchair) and at risk of falling with a serious hip injury. I tried to contact the Social Worker, Director and Administrator numerous times but they never called me back. The nurses seemed caring but the administrative staff could care less once they get your loved one in.
