"This is a skilled nursing facility and they also do short term rehab. They were renovated in 2007 into a 58 bed home and all private rooms. This is a one story building. When you go in it smells good and interior is clean. The building and grounds are well maintained. They have a central patio area. They have housekeeping and laundry. They serve three meals and snacks in diningroom or room as necessary. The food is ok and they do a rotating menu. They have an RN and LPN on duty 24/7. All caregivers are CNAs and licensed. They have two stations.They have a house doctor and do transport to appointments. The staff is nice. I always thought this a good care center.They have a full rehab department. They have an activitiy director and do games, parties, movies, and have music. They go out for shopping and other events. I feel this is a good facility. "

"Just got home today, and I have to give credit where it is due. I got two theropy sessions a day. I got strong enough to go home and I'm sure of myself. No worries. I would recommend this facility to everyone. All of the employees are great, I didn't ever run into one person who didn't care. They help you get well so that you can go home.... "

"Having to place my husband in a nursing and rehabilitation center due to his injury I worked diligently to find a facility that could meet all of his needs. The professionalism, service and care that has been provided to him while temporarily residing at the rehabilitation center, has been superb. The staff is compassionate, caring and very friendly. They are always interacting with patients and family members. When my husband slid to the floor they called immediately and reassured us that he was just fine. The staff members are always giving updates about my husband and we know them all on a first name basis. The staff members have a special place in our heart and we would recommend them over and over again. "

"The Gardens of Scottsdale offers different levels of care. The facility is clean and very nice. It feels homey, warm, and inviting. Dad has been there for a week now and from what I saw the staff seemed friendly. Their food is good. "

"My father was discharged from Advanced Health Care of Scottsdale, and it was excellent. He was only there for four-and-a-half weeks. Everything was phenomenal, from the admissions process to their Covid protocols, and how they cared and protected the patients. The facility was beautiful. Their meals were wonderful. They have onsite physical therapist and occupational therapist, and they were wonderful. Just everything was absolutely fabulous. They have private suites. The nurses, the caregivers, and everything was topnotch. I couldn't have asked for more. On the discharge, they followed through and made sure that we had everything we needed. I couldn't more highly recommend the facility. The food was excellent. Not only was it well thought and beautifully prepared, but it was healthy and restaurant quality (really). Everything was beautiful and well kept. It really looked more like a 5-star hotel. It did not have the smell of a care facility as some of them do. It was beautifully furnished. It felt like home. It didn't have a cold feeling. Everything about the facility was excellent. It is a pricier facility and probably one of the top prices, but it meets or it goes above and beyond the price. It actually is worth the money. It costs more, but you get what you pay for."

"The facility was simple but attractive. There was a beautiful courtyard that patients and family members could spend time together away from the room. I liked that there was a dining area that looked like a restaurant that patients could go and eat instead of eating in their room. The tables were set up as if it was a fine dining restaurant including linen tablecloths. This was a nice amenity for the patients to socialize in. This dining area was impressive and made the facility not look as if it was hospital like. The staff was nice and attentive. When we arrived there was a check in desk and the receptionist was very helpful in directing us to the room we were looking for. My mother in law also has a mental illness and the staff there treated her with respect and understanding when she would have one of her Bi-Polar episodes.My mother in law enjoyed her stay there and has even kept in contact with the roommate she had while staying in this facility. "I checked out a few facilities and I was most impressed with Life Care Center. They did a pretty good job. The people were friendly and helpful. It was well-decorated. It was a nice place. At that time, my mother had a private room. It was very spacious. The food was fine. She was in her final stages, so food wasn't a big deal for her. Overall, they took care of my mother there. What I like best about this facility is they seemed to have a capable staff. "

"This is a nice 2 story building with 23 units. They are one bedroom with partial kitchens. All handicapped equipped have emergency system. Cable and internet is included as well as housekeeping and laundry. They have a formal dining room which serves three meals. They a good variety and the food is good. There is an activities room, a fitness room, large shaded patios off the second floor and downstairs,a beauty salon and a pool.The interior decor is nice. Out side they have some nice greenery and walking paths. They have doctors who come in and they transport to appointments.There are LPNs during day and on call. They certified caregivers. They have an activities director. They have lots of in house activties and go out to shopping, restaurants and other events, This a nice smaller community with a good and caring staff. "

"My mother-in-law went to Phoenix Mountain Nursing Center in March. This place is right in the center of the city, but it is tucked in -- very quiet -- and the garden is landscaped really nicely with fountain and flowers. It's a single story, so we don't have to take her upstairs or downstairs for the activities. The design is very unique. The nursing station is right in the center, and the halls are spread out, so the facilities are bright, clean, and very quiet. The staff is absolutely great. They're really involved with the patients. She was in rehab for about six weeks and then went to the permanent care. We have regular meetings, a doctor makes rounds, and she is seen twice a week. We are so glad that we found the place. Her room is semi-private. There are a few private rooms, but they are all occupied, and she is on a waiting list. All rooms are simple but clean. They do have a monthly printout schedule. They show movies, they take them shopping, and they play bingo. Everyday, there is an activity. If they can feed themselves, they would go to the main dining. There's a separate dining area for somebody who needs help with feeding. They don't let them stay in the room all the time. They let them get up in a wheelchair and bring them out in the hallway or the dining area, so they can watch TV and be with other people. They also have a beauty salon right within the facility. Also, it's close to our home, which is about 15 minutes away. One thing she said is lacking is that other than banana, they don't give her fresh fruit or fresh vegetables. They use a lot of canned fruits and canned vegetables. "

"Plaza is a large medical facility which emcompasses a convalescent home and an assisted living as well. It is a memory care assisted for dementia residents called Mimosa. The assisted living is in the front of the nursing home but is set-up toyally separate. It houses 50 residents and currently there is 30. They have large private and semi-private rooms. All handicapped equipped. The staff are all certified caregivers and certified med techs with previous healthcare background. There are 4 on days,3 pm and 3 on nights. They have an RN during days on staff who oversees this unit and comes in frequently and as needed. There is a nurse practitioner who visits weekly and some doctors who come in. They have a psychiatric group they work with. They have all meals cooked in the nursing home kitchen so they have an available dietician. They serve three meals and several snacks. The food has a variety diet but choices at meal times limited to that meal entree. The food is typical of a nursing home or hospital style. It is nutritious however. The residents eat in their own dining room. They do, however, have a choice to go to the 50 diner just behind the unit in the building. This is usually when family visits. Outside they have a beautiful courtyard with walking paths, shaded areas to sit, lush flowers and greenery. There areas for outside activities when it is cooler here. The staff take them for walks in the morning. They have an activities director and wide group of things to do. They have bingo, karaoke, exercises, current events, birthday parties and they put a day on where family is encouraged to come and they do a social or have outside entertainment. At times they may go out on the bus or with famiky but rare due to the residents mental condition. They have a beauty salon. The staff appears to be caring and residents taken care of. For someone that has a debilitating mental condition and cannot be at home or in a more independent setting it is a good facility. "

"My dad is currently at Desert Haven Care Center. They are doing a good job with him. Whenever we have gone in there, he does not smell like pee or poop. He is relatively cleaned up. We have not had any problems. "

"The Terraces Assisted Living had a nice outside balcony where I could be by myself, as well as other outdoor areas where I could sit around. The apartments were spacious, large, and very well done with their own balconies. There's an extra storage room, which comes in handy. It was fairly new and up-to-date. They also had a lot of activities. There were several eating areas, and the cuisine was excellent. It had a very friendly atmosphere and was nice. "

"I loved my stay there. They cared for me like I was one of the family. I had problems with my medication making my stomach upset, but they had a menu I could order from any time, like room service. "
