I am a friend or relative of a current/past resident

This was a terrible place for my mom to stay after being in the hospital. She was supposed to go there to gain weight and get strionger, she was told they only had frozen food and couldn’t even bake her eggs for dinner because she couldn’t chew anything else. She lived in cottage cheese and coffee for 2 weeks! They also forgot to feed her ttwice! Once for dinner and once for breakfast, the beds are gurney’s ! They didn’t even supply a chair for her to sit in , they don’t care about the patients at all and act like they are bothered by helping the people who need help most, my mom got worse instead of better and a few days later ended up in the hospital again because they didn’t feed her. What a horrible place to stay.

One of the worst places you could possibly send a loved one. My father came here after brain surgery and ended up falling out of bed. This place *REFUSED* to have him taken to a hospital to have him checked out. After BRAIN SURGERY. After hours of complaining to administrators, they finally sent him, wearing NOTHING but a shirt and diaper. No socks or shoes on a cold night in February. They lost his clothes so regularly that we had to pick them up and wash them ourselves to ensure they weren’t lost or given to other patients. The next day after the hospital trip my father was mysteriously quarantined. They wouldn’t tell us why. Wasn’t because of covid, after having to talk to person after person, it turns out they misdiagnosed him, with a difficile infection and they admitted to overmedicating him with laxatives and stool softeners. They were trying to keep him quarantined and just not tell us about any of this until it blew over. He didn’t have an infection. He was just at the hospital, they obviously would have known. How long had he been over-medicated for? No clue, they wouldn’t tell us. Just a simple “Oops, we overmedicated your dad!” They put in his records that he wasn't progressing, and took away his speech and physical therapy for NO REASON, claiming all of a sudden after a few weeks that he was beyond help, despite EVERY SINGLE other doctor and the physical therapist themselves saying he was improving nicely. We had to fight the insurance company tooth and nail to prove them wrong. Thank you for nothing! This facility literally gave up on my father and decided we should just deal with it. My mom complained to Angela the case manager, who aggressively and shouted at my mother: “You need to just accept it! He will NEVER eat again!!!” How despicable of a human being do you have to be to treat someone so distastefully? Especially in your line of business? He was without his therapy for weeks and we ultimately had to fight to get him moved to a different facility altogether, because the more we complained, the more the facility started to neglect him. They took it upon themselves to not feed him anything aside from tube nutrition, despite him needing mouth feedings to learn to eat again. These were his doctors orders right out of the hospital, but these things were either ignored or “accidentally” looked over as the facility decided to follow its own rules. This facility is LITERALLY responsible for slowing my dads improvement, and they seemingly have done everything they can to make things worse. One of the last times my mom saw him there, they just had him sitting in a diaper. When she asked why they had him like that, they had no answer. He had clothes there, they just didn't want to help him get dressed. How can you treat people like this? They are not trying to help heal these patients, they merely want to keep them like vegetables and collect payment until they die. Within TWO WEEKS of my dad being transferred to another facility he was able to eat again, completely off the tube, and was already progressing fine in physical therapy. Every single doctor and professional we have spoken to agrees that Phoenix Mountain Post-Acute GAVE UP ON MY FATHER. Not only that, but some told us they suspected he hadn't even been bathed once, and that bed sores were starting to develop on his feet. They were willing to allow him to suffer and eventually die because it was too inconvenient for them to try to rehabilitate him. YOU ARE A REHABILITATION CENTER. YOUR ONLY JOB IS TO REHABILITATE PEOPLE. My father isn't even 70 years old. How could you slime balls treat a person so lowly? Let me reiterate: Every. Single. Doctor that I have spoken to agrees: You gave up on my father. Shame on you all. Your job is to care for society's most vulnerable, and this is how you treat them. To anyone reading this far I leave you with this question: if even a fraction of these things could happen to your loved one, is it worth the risk of sending them here?

I have been to 5 plus nursing facilities in the Phoenix/Scottsdale area and Phoenix Mountain is a very very clean facility and staff is very caring great rehabilitation crew residents are well cared for and a lot of activity

I visited this facility

My mother stayed here after a hip surgery and it was great care and all the staff is really friendly, a person saw me in the hospital and answered all of our questions before we came which made transition less stressful. The food is really good the chef makes a homemade soup daily. Recovery was a good experience with the best therapy ever. More chairs for guests in the rooms would be nice, but we loved our stay there much better than facilities I toured! care and therapy is what is important. 5 star and your dog can visit too which made mom happy.

Phoenix Mountain Nursing Center is an incredible care facility with an amazing and caring staff! They really are a family; many of them having worked here for well over 10 years. They truly saved my Mom’s life. We are so blessed to have found them. Thank you Phoenix Mountain Nursing Center!

My mother-in-law went to Phoenix Mountain Nursing Center in March. This place is right in the center of the city, but it is tucked in -- very quiet -- and the garden is landscaped really nicely with fountain and flowers. It's a single story, so we don't have to take her upstairs or downstairs for the activities. The design is very unique. The nursing station is right in the center, and the halls are spread out, so the facilities are bright, clean, and very quiet. The staff is absolutely great. They're really involved with the patients. She was in rehab for about six weeks and then went to the permanent care. We have regular meetings, a doctor makes rounds, and she is seen twice a week. We are so glad that we found the place. Her room is semi-private. There are a few private rooms, but they are all occupied, and she is on a waiting list. All rooms are simple but clean. They do have a monthly printout schedule. They show movies, they take them shopping, and they play bingo. Everyday, there is an activity. If they can feed themselves, they would go to the main dining. There's a separate dining area for somebody who needs help with feeding. They don't let them stay in the room all the time. They let them get up in a wheelchair and bring them out in the hallway or the dining area, so they can watch TV and be with other people. They also have a beauty salon right within the facility. Also, it's close to our home, which is about 15 minutes away. One thing she said is lacking is that other than banana, they don't give her fresh fruit or fresh vegetables. They use a lot of canned fruits and canned vegetables.
