"The Forum at Tucson was lovely. They offer just about everything you could want; you could get anything done right there. They have so many activities like playing cards, they have a fitness room, you can go swimming, and they can take you shopping."

"My mother went to HealthSouth Rehab, and they were fabulous. Our experience was very good. They were well staffed. I wish she could have stayed there. The rooms were small, but they were clean, and the bed was comfortable. The food was good. "

"My mother was here for rehabilitation after breaking her hip in late 2024. She was in this facility from Chrictmas eve through the end of January 2025. The facility looks to have been built in the late 1950's and it shows its age here and there but it is well maintained, it didn't smell like a nursing facility (I can't say that for some other places I visited), most of the staff was really good (of course there were a couple there just to get a check). They diligently worked to get her better and back to us. One interesting thing was I was walking the hallways to leave the facility one weekday evening and I ran into an elderly man walking too and we struck up a conversation. Turns out he is one of the owners and after talking to him and the staff after our encounter I found out that he is out almost every week from Chicago & spends about half the week onsite staying current with how things are going from the residents. I was skeptical at first look (especially after being in a different facility of approximatly the same age) But I couldn't have been happier with the outcome. These are good people that care. "I am staying in Sandstone of Tucson, and I like the accommodations here. I don't care for the food, though. It's unimaginative. The staff members are good. I partake in activities like bingo, and now and then we have a field trip, but I don't get enough exercise. There's not much in the way of activities. The facility is plenty clean. They do a good job with it."

"When my mom fell at the assisted living home that she was in, she had a pelvic fracture and ended up at the hospital. When she was discharged, her doctor referred her to ManorCare Health Services because she was going to need physical therapy. My mom was there for only 20 days. It was a lot different than an assisted living facility, it was more gloomy because it was a nursing home. The place, the staff and the room were all mediocre. The rooms were okay and the space was very basic, nothing fancy. Their nursing staff also were very friendly. However, the food wasn't good, actually, it was really bad. "

"I got really good care from everyone at Sabino Canyon. Very friendly and caring people. The place is kept very clean and the nurses and nurse aides were so caring. They worked very hard to take good care of me and I am forever grateful. "

"This is a large facility. I was told by their manager they are the largest skilled nursing facility in Arizona. Their employees are very good at what they do and work hard to make the residents feel at home. There are residents who live their permanently, and patients who are there for rehabilitation or skilled treatment such as wound care. The atmosphere is made to feel "homey" and welcoming. Their are separate areas for permanent residents made to look like bedrooms instead of hospital rooms. However, there are also rooms for patients that resemble hospital rooms. There are separate areas outside the patient rooms made to look like living rooms where there is a television, a table to play checkers and cards. Their are daily activities such as art classes, exercise classes and group gatherings to keep the residents engaged. There were times where the staff members were not as friendly to the residents as would be expected. This is probably true in every facility you visit due varying levels of a caring nature some people have, but overal, this facility seemed to be a good environment. "

"My mom stayed at the Avalon (shadow Mountain) facility in Scottsdale, and the staff was very helpful. The nurses were great. The facility was clean and pleasant. A good experience after she broke her hip. "

"Room was a nice size and well-worn, somewhat like a motel room that has been well-used. There was no reading light for my wife at her bed. One was torn off the wall, the other did not work. Staff such as nurses, nutritionist, PT, OT did a very good job and were caring and professional. The management was neither. The head "case manager" was not at all patient oriented, and not interested in the least in involving family in the decision-making process. The social worker was for the most part invisible. Surprisingly the food in the dining room was better than most corporate chain restaurants such as Olive Garden and Cracker Barrel. The selection for patients was very nice and the quality of food provided in the patient's rooms was very good in our experience. The dining room is a very comfortable place to eat with your patient. Do not expect an individual program plan to be provided to patient or family members or any transparency on the part of management. The true professionals such as PT and OT do very well in that regard and communicate regularly with patient and family. Kudos to them."

"mostly pleasant . somewhat cramped if long term. staff very pleasant and very accommodating . a variety of activities, depending on your ability and willingness to participate. wonderful , well prepared meals. like home cooking. pleasant patio shaded. my son was a resident here for 2 weeks. the care was excellent. nursing staff pleasant. house keeping and all other support staff, very efficient. "I love the staff, they are friendly and helpful. My husband has been there a year and they have taken great care of him. He had a major stroke, so only reason I gave 4 stars is that the in-house therapy isnt as intense as he needs. But other than that, they are great. "

"Our Mom has been a guest at Mountain View Care Center as long term patient for over 5 months and the care she receives is professional and done with love. All the Nurses, CNA's OT/PT , cleaning folks show kindness to our Mom and are attentive to her needs, anticipating her health needs and providing good care. They identified a UTI by her behavior thus preventing an adverse outcome. She is happy, clean and well cared for. Oh, and the likes the food! So why not a 5? I called twice to speak directly with her physician amd he nor his NP returned my calls, So now I speak to the more that comipdant nurses and they get Mom's medical needs met and addressed.
