"This is a friendly, caring community of and for seniors. I have lived here 12 years, and though employees come and go on occasion, it has provided outstanding service for these years. And the facility is a reasonable size. Covid certainly affected the familiar, but care and services continued as before. The facility is currently undergoing upgrading."

"My mom was in rehab at Freedom Plaza Care Center for 12 days. I loved everything about the facility. I loved the cost-effectiveness of everything, no matter what skilled assistance they needed. You didn't have to buy any packages to go along on top of what was being paid every month. Some facilities start at a low range and then in the event that you need more, they give you the cost of packages. Whereas at Freedom it was all-inclusive, is what we were told. The thing that my mother did not like about the place was more of the food, but this was only her taste. She had to be on a heart-healthy food diet. Things did not taste as good as she had hoped, but then they had a deli or a little cafe in there that she had gone to, which she loved. They have a beauty salon. They have workout rooms. They do advertise a pool or a spa, but they do not have that installed because the pandemic has hit and they just decided not to open it. Everyone is friendly. The manager was wonderful in explaining things. He knew everyone's name. The case manager, the front desk staff, and, I don't remember the others, but the skilled nursing staff were on top of everything. They have a music room. They have one that has a piano in it. They have bands that come in sometimes. They've got a game room, a TV room, a movie room, and a gym. They have activities where they go off campus to wherever they're going to go. They also have rides to and from the doctors. They do not provide companions but they do assist with the rides to the doctor's office or appointments. They have bingo, puzzles, and games. It was just a friendly place."

"My friend is residing in The Forum at Desert Harbor. We liked the way the building was maintained. The residents were friendly, and the activities they provided especially appealed to my friend. The room has very classic lines, it kinda takes from the Spanish decor with plenty of light, well-lit, and very spacious. They have regular concerts in the afternoon, and on Tuesdays and Thursdays, they take a bus for some kind of shopping experience. My friend gave up his truck, so they provide transportation within ten miles for him to his doctor's appointments and his speech therapy appointments. That's very helpful, and it's a beautiful car driven by a very nice man. Every night after dinner, they play some kind of card game and there's usually a blackjack game during the day, a lot of things he likes over there. There's a beautiful lake right there at the back fence and he loves walking along the lake. I think also that it was a very good value. I liked that the staff were not patronizing. They treated the residents like the grown adults that they are. The only thing I didn't like was you'd have to go through the dining room to get to the elevator. And that is secure, but not easy to do. It's easy to drive to, it's close to the freeways."

"Can't tell you how caring everyone was with my very ill Mother-In-Law at this facility during our very difficult journey. From every nurse, caregiver, cleaning staff and even the kitchen, they all did everything they could to check on her regularly and make her comfortable. The facility was convenient and clean. They were exactly what we needed for our loved one, when we needed it. There are several levels of care there and you really get a feeling that they are a close community/family. Whether it is short term or long term, when you have a loved one who needs this type of specialized care it is never a good thing; but when you do, highly recommend Plaza Del Rio to take care of them. "

"We toured Freedom Plaza Arizona. We thought it was great, it is beautiful inside and spacious. The thing we did not like was the cost, it was pretty high. We have friends there, so we saw their apartments and it was very large. We did eat there, and it was excellent. They had a beautiful pool, a beautiful workshop, and a separate room to do things in. They had a beautiful lobby. The staff were excellent and very nice."

"My dad has had some health issues form6 months now. Short time in hospital released to health rehabs. After 2 stays at 2 other rehab centers that we called 911 to get him back to hospital. Other two rehab his health would decline and due to neglect or not addressing fevers needed hospital attention. The other two rehabs have left us traumatized. Mind you we keep an eye on him as much as we can. Banner Boswell was amazing! Dad got the monitoring needed and the care. It took a while but with their constant monitoring we were able to sleep at night knowing he was in great hands! Please know wherever your loved one is sent you have rights! You can call 911 or discharge to a different facility if you don't feel your loved one is getting the care they need. We found out the hard way."

"It is with deep regret I have to Leave the care of your facility and staff. It has bee an beutiful memory for me. I would ask only that you adopt me! "

"I was just there at Lake Pleasant Post Acute and Rehabilitation for six weeks, and I will say , I loved it there. The whole staff was amazing. They helped me get better. I personally don’t like to stay in facilities like these but they made it comfortable for me to stay until my treatments were complete. If you’re looking for a place that only has one nurse to a patient then keep them at home and give them that care. The nurses have more than just your family member to tend to so they stay busy. They are not lazy. I would definitely recommend this facility, but honestly I would keep my loved ones home and take care of them myself instead of pawning them off to a facility and then complaining about it because of ignorance."

"Everyone there was absolutely wonderful from nurses, therapists, in house information I would give: Nurses and cna ***** Therapists ***** Meals ***** Cleanliness ***** Information about your stay and follows you would need ***** That they thought my British accent was cool not so much as I would love to lose it. Though I was there for a fall and had to have therapy I really loved the therapists I had a blast with them. When I left was known as the music lady love that nickname. Once again I wish I hadn't fallen but my experience there was easily 10/10"

"I visited LifeStream Complete Senior Living at Sun Ridge. The living quarters were small but very nice, and they had a little Jacuzzi outside. It's something I might go back to, but I had a lot of furniture and decorations that could not fit in a small place. The place was elegant and very nice. "

"The rooms are spacious and we'll maintained! Staff was friendly and caring. Bingo and daily activities! Nature walks on manicured and secured property."

"My husband is in a skilled nursing facility called Center at Arrowhead, LLC. It's a good place. It's clean, and he is well taken care of. The people do their job well. He's been there almost three weeks now. They cannot do therapy if he has pain in his leg. He's the only one in his room. It only has one bed. The whole building and the surrounding area are clean. The inside and the outside are clean. The people working there are clean. The bathroom is clean. The staff is good and friendly, and they care for my husband. The meals are also good. Everything is OK for me."

"Cook Health Care was a nice place. My mother was there three months. The rooms were small, but she can move her wheelchair. She was able to use the sink, wash her face, and brush her teeth with help. They had very loving staff. The food was excellent, and they had several choices. The care and the activities were excellent. "
