I am a friend or relative of a current/past resident

Rooms and food were ok but care was very sketchy. You're promised alot by the admin dept but the actual service given is far short. If you come in on a Friday, you won't get any rehab until the following week cause their accesment nurses are off on the weekends. Oh and wound care...they're off also. PT and OT is sketchy at best. Better than hospitals but this is supposed to be a rehab from hospital to home. Are experience was no improvement for the 2 weeks there.

Would never use this facility again. Nurses don’t return calls, room floor was covered in urine, our shoes stuck to the tacty floor every time we were there. My uncle didn’t have a phone plugged in until I called his case worker. She also failed him, he was sent home and was told home healthcare work be in touch with him within 72 hours. It’s been 5 days now and they don’t have him in their records! He’s 80 and can’t shower alone. His case is grossly miss cared for.

My dad has had some health issues form6 months now. Short time in hospital released to health rehabs. After 2 stays at 2 other rehab centers that we called 911 to get him back to hospital. Other two rehab his health would decline and due to neglect or not addressing fevers needed hospital attention. The other two rehabs have left us traumatized. Mind you we keep an eye on him as much as we can. Banner Boswell was amazing! Dad got the monitoring needed and the care. It took a while but with their constant monitoring we were able to sleep at night knowing he was in great hands! Please know wherever your loved one is sent you have rights! You can call 911 or discharge to a different facility if you don't feel your loved one is getting the care they need. We found out the hard way.

Banner Boswell Cardio. Rehab. is amazing. I entered a 12 week program in late September 2021 to build back my strength lost due to an aortic valve transplant in late July. Mark, Ashley, Jean, and Anthony all worked as a team to improve my strength and ability to function over 300% higher in those three short months. They are personal but professional with patients and keep the area totally antiseptic and all wear masks IAW COVID regulations. They were always flexible when a patient needed a day off (3 -1 hour sessions per week) to take care of personal business etc. We were given our own rehab. and diet packages to read and use in our daily lives. The rehab room is filled with at least 40 varied pieces of athletic equipment to diversify our training and were responsible for moving from one machine to another based on our own exercise charts that were updated daily. My wife had a decent place to sit while I was exercising so I'd have a ride home. I can only say I'm blessed to have chosen Banner Boswell for my cardio rehab. Best experience ever. Thank you.

I am extremely frustrated with lack of communication from the management of this facility.While I realize a pandemic has family and friends unable to visit,it is IMPOSSIBLE to reach social services, the D.O.N.,or a nurse . Any messages left are not returned. Planning a discharge of a family member is IMPOSSIBLE as well.

Banner Boswell Rehabilitation Center did a good job with my sister. She had a roommate in a usual, hospital-type room. The place was an older facility but clean. The nurses were good, and I was impressed with some of the rehab people, so I was pleased with my sister's care. There was an activities director who gave my sister a manicure, which was very nice. They pureed the food for her, and it looked OK. When I wasn't there to feed her, they had someone else who did.

I am/was a resident of this facility

First time here for total knee replacement therapy. Physical therapy team is excellent. The CNA' s are surly and sloppy in their duties. Don't come here if you are thin skinned, cause they don't care. If you are looking for compassion and human Decency, don't come here. I feel like I'm in prison.

My father was admitted to this facility after being in and out of the hospital for the previous 6 weeks. Upon admission, he immediately began to recover from him malnutrition (caused at a competitor down the street) and physical deterioration. His diet was closely monitored, he received immediate OT and PT and he began to flourish and has now been discharged. The CNA's & LPN's are quick to respond to the patients needs with a smile and loving concern. I highly recommend this facility and care team!

My father was at this facility for two weeks back in July 2016. We had issue after issue here. The main concerns was that the staff could not handle his medical needs. There were multiple instances where his oxygen was Removed for something simple like A breathing treatment, then was forgotten to be put back on by the nurse. The oxygen was left off for nearly an hour during these instances, and he was supposed to be on oxygen for 24 hours a day. Many of the staff members seemed to be aloof in regards to the severity of medical needs, my dad was diabetic and one time the assistant took his blood sugar and it was in the low 70s, which was extremely low for him. This was explained and she said she would go grab an orange juice for him. Nearly a half an hour later I walked out into the hall and that same assistant was standing there braiding another assistant's hair. Ridiculous! It may be a decent place for rehab, but not rehab patients who have other medical needs.

I am a friend or relative of a resident

Banner Boswell Rehabilitation Center was very clean. This is mom's fourth time in this place because she was treated very well. The physical and occupational therapists were really good at their jobs. The LPNs and the RNs were all pretty good. There were just a couple of them that were kind of grumpy all of the time. But overall, they were really good. Mom did not have much of an appetite, but she said her food was pretty tasty. They had some recreations, like they had an Easter party, and we were there for Thanksgiving dinner.

I am a friend or relative of a resident

My husband fell, broke his hip, and had surgery, and then when he got out of the hospital, they took him over to Banner Boswell Rehab. He was there for 17 days. He had partners in the room some of the time. He went to rehab for six hours a day, but not all at once -- two in the morning, two in the afternoon and two later on. As far as the rehab and everything, it was very good.
