I just can't say enough about the caring staff and excellent rehabilitation services here. They were there for my Grandpa at each stage of his rehabilitation. I highly recommend this facility with out reservation.

My Husband was here in December 2023. His bed was shoved against a wall facing a tv that worked less than 25% of the time. He was there for PT/OT which he did for 30-45 minutes a day 5 days a week. The rest of the time he was in his bed, doing nothing. Many of the staff just completely ignored his call light and would not assist with meals or toileting. He lost 15 pounds...due to lack of assistance with meals and no one botherd to find out what he would eat. When I visited I would get him up by myself and take him outside for some sun and change of scenery. He was discharged unsafely home and ended up back in the hospital a week later.

Rooms are clean which is nice. Food is adequate but always cold I don't understand what goes on with the workers here. Is it laziness? When they are available the "I don't want to be here shows." Some residents need to be fed. The food gets put aside to sit somewhere and get cold. Then the cold food is collected without the resident getting fed. This is a difficult job no doubt. If you don't want to work and help these people; then don't work there.

This is a horrible place. my husband spent about 7 days there. Fell on his head and shoulder twice. Was left on mattress on floor lying down with foodtray on his stomach. No one to help him. Fell twice and team there were not allowed to chart it. But some told me about it and his roommate told me also. I had them send himto the hospital. He could not think clearly. Do not send love ones who need wheelchair or can't walk well, You will be so sorry. This shld have minus stars but there are none

My sister was a resident there for approximately nine days, give or take a few. I would not take a dog that I didn't like there. My sisters blind. They never told her when her food was there. She would sit and wait for hours for her food to be served, while it would be sitting right next to her, obviously unbeknownst to her. They never told her where the food was the on the tray. They let my sister lay in her poop in dirty diapers for hours after she paged them. The night CNA's were absolutely obnoxious they would scream at her. The people during the day were not too bad, but overall that place is a piece of crap. I have no idea how they stay licensed the way they treat people, especially people that have disabilities. If I had a family member that needed care, that would be the last place I would ever take them. My sister and I plan on filing a grievance with the blind association with the state of Arizona in reference to this facility. It's sad that they stay in business and for what they charge it's a pitiful facility. I asked for her records for a doctors appointment that she had on Monday. I had picked her up on Saturday . They refused to give them to me. Again I would never take anybody that I love there to be taken care of. It's an absolute disaster.

My Brother has been at this place going on 2 weeks. He is supposed to be getting speech therapy, physical therapy and occupational thereapy. He has had 1 speech therapy session which is one of the biggest issues. Other then that it has been one therapy session a day for barely 30 minutes and no weekends at all. The food in there i wouldnt feed to my dogs and not enough for a child. Only allowed 2 showers a week!!! The rooms are only the size for a single person both patients are against walls. Hopefully neither need emergency help because they would be twice as long rendering aide. This place is unsafe and very unheathly. Please do not take anyone you love to this place. Its not really the people who work there its the management that allows this to happen,totally disgusting!!!!!

My wife was admitted for rehab about two or three days ago after we fought with Medicare to approve the admission from the hospital she was in. We can now see why Medicare didn't approve it until we appealed (even though this is on the Medicare approval list). So far, no one has come to take inventory of what she brought, we have not seen any administrators and very few regular employees (most of the nurses are contract nurses for one day of work only. The food has been disgusting and I have been bringing making and bringing food to her, and bty: no facilities to warm up food and apparently no cold storage for food. The facilities are of poor quality (the furniture is very old and worn out). We chose this facility because the website gave false information. The pictures show large, private rooms with large comfortable chairs for visitors. Instead, they were small, two-person rooms with no chairs for visitors. Having a suitable chair for me has been a fight almost every day I've come. I have medical problems myself and the staff out and out stated that (in so many words) they don't care about me when I visit. As with other reviews, staff don't respond to call buttons and don't give meds when needed On the positive side, the OTs and PTs (the main reason we re there) have been professional and friendly and they has been one CNA who has been most helpful despite being overworked. I will have to wait and see if the admins allow them to work with her as much as they propose. Overall, If you are using Medicare, choose another facility.

I am staying here, found a few bugs, people have to buy their own food, place is hardly cleaned, on 9/26/22 I had told a nurse my IV antibiotic was up at 10pm, she didn't come give it to me until 3am, I finished at 4am, then at 5am tried to double up on the antibiotic, I told her that was not right, she proceeded to argue with me that it was fine, I looked it up and it is wrong, also I called poison control they told me not to be doing that either, I will be speaking with the administer about this, so if they do this to myself, imagine what they do to people that are not coherent, and don't know what is going on, do your homework on where you have your Love one going, make sure you go look at these places. So basically the nurse could have poised me or made me have really bad side effects, it's true that patients do yell for assistance and they don't get attended to, so unless your a patient to see and hear what goes on here you won't be aware.

The rooms are crowded, people keep their own food/sodas/snacks in their room in cases and boxes. The bathroom was filled with equipment making using the toilet difficult. There were crumbs of food on the floor. There were bugs that crawled on the floor and wall - I squished a couple. The food was poor quality, and unappetizing, and the drink with dinner was grape kool-aid. They missed giving me medications and did not give me my prescribed pain medication. The staff was 50/50 friendly vs just giving what they were bringing and walking out. They ignored my roommate who was asking for help (she was blind), and I was able to get up and find her bed control for her. She couldn't get up herself. I had no explanation of how things worked or where things were. The hallway was completely dark and I had no idea where the emergency exits were. I checked myself out AMA after a night of people yelling and screaming, and people calling out "Nurse, Nurse, please help me Nurse!" and no one ever came.

I would not recommend anyone trust their loved ones to this place. In 12 days, my father only received 4 rehab sessions. They left him in bed 21 hours a day. He's diabetic, and they were feeding him donuts and peanut butter sandwiches. The bathroom in his room looked filthy, the toilet was broken and rocked from side to side as if the floor beneath it was rotten. To make it worse there was ZERO communication from staff. I knew I had to get him transferred somewhere else, it took 5 days just to get someone to fax his clinical information to the new facility so we could start the transfer process. Everyone there just shuffled me from one voicemail to another, and I never got a call back from a single person. I have already filed a complaint with the insurance carrier, hopefully I can get Lake Pleasant removed from their approved facility list so no one else has to go through this.

this was a wonderful facility! when my mom got there she was given a goodie bag filled with brand new supplies she got to keep, the room was very clean. smelled good too! the entire time she was given all her medication on time, every time and she was even given a wide verity of food she could have with her special diet. she loved the food, it tasted good and I could even order some if I wanted to! her favorite days were Fridays because they had donut social in the morning and in the afternoon they had popcorn and movies, every Friday! it was such a great experience for her! her and I would highly recommend to anyone ! staff was amazing, food was great, the actives director always do fun actives like bingo, ice-cream social, pretty nails(free) and first hair cut was free here too!rooms were always clean, there is stores around and restaurants we could take her to, this is in a clean beautiful area, what's not to like about it!! highly recommend!!!

This facility is horrible. It’s dirty and grimy with cockroaches that Climb on walls. The food is disgustingAnd it’s worse than being in the cafeteria. Staff turnover is unreal and patients are dealing drugs out of the facility.

If I could pick zero stars I would. This place is horrible. My grandpa has been here for 3 days and I'm already looking for a new place. They have messed up his medication every day , don't change or clean him . Refused to give him pain pills. If you want your loved one to live don't go here. Worst neglect I have ever seen!!!!!
