I just wanted to take the time to thank the Liaison-Kevin for assisting me and my family with the plentiful questions we had. He was very informative and helped me and my family out tremendously. Thank you Kevin for making it a great experience! We also loved the facility and recommend it to other families.

absolutely the worst care ! nothing nice to say . No resolution from management either .

4 yrs ago Cobalt was wonderful. This time around it was not. I found most of the staff to be unhappy. Some were very short tempered with my family member & some therapists seemed down right frustrated, the food was horrible and the case manager lacked communication skills Some CNA’s were great, others could care less. Major shortage of staff. I felt bad for the nursing staff as they did not seem happy either. Some of them were very good. I understand after discharge , Cobalt has changed hands and also has a name change Dr Blakey was great! 2 therapists were very good. Rooms were nice & spacious. Look at bringing on different staff in some areas, definitely improve the choice of food with some type of menu. Not hit or miss with a person coming around snd giving u one choice.rooms were not cleaned daily. Had to ask.

Got to the facility at 7:30 at night. Short staffed, I was put into a bed in a single and sat there until 9: Pm to be checked in. Next morning I was given breakfast. Mostly in edible. Occupational therapist accessed what I could do and what I needed help on. She gave my my1st showe in 2 days( at the hosp ) finally able to brush my teeth. Physical therapist came up and said I would be getting 3 hrs of therapy today. Starting the next day. Taken down in a wheel care for lunch. Was not told they had limited other choices from the main choice. Main choice, fish, was bad. Extremely thin tiliapia. Green been that turned out to be overcooked asparagus. Asked for a salad. Back in room, left alone until dinner. I could call the nurses station. Didn’t arrive at dinner when it opened, they brought it up. Extremely difficult to eat in bed as I have 3 broken ribs and a broken ankle. Dinner was edible, Turkey slices, gravy, stuffing and broccoli. Broccoli was over cooked and the Turkey was very dry. Gravy very salty. No cranberries or other condiments. Night nurse, have to be taken to the bathroom by wheelchair, call when finished. Could get my pants up by leaning against the wall. I wheeled myself out to brush my teeth. Was forgotten about. Made it back into my bed by myself after sitting in the wheelchair for 20 minutes. Very disappointed from 1st night and day. I have a hard time understanding how the food can be so terrible.

My father was a recent patient at Cobalt rehabilitation center in Surprise AZ. (December 23, 2020 -January 17, 2021) Treated for a stroke. During this pandemic we were not allowed to physically visit. The family attempted to call him daily, this was often unsuccessful, the Phone was not answered, or we were transferred several times with no success. We often sat on hold greater than 30 minutes and the attitudes we received were generally unpleasant. One nurse (Neil) did allow “window visits” others told us that was not allowed due to Covid. A “window visit “ was simply placing my dads chair in front of his second story room so he could face outside and see us standing there while talking to him on the telephone. Several messages were left for therapy and the case manager/discharge planner (Peach) Calls were never returned. After New Years the nursing supervisor Trish did call to obtain permission to treat and a DNR order. She told me she often catches the mistakes and cleans up there but she had been off for the holidays. Discharge planning was non-existent. Once Peach was made aware that my dad would be going home and NOT to a SNF there was no communication. The nurse Neil did call to alert my mom that my Dad would be ready for pickup Sunday morning at 11. My mom arrived promptly at 11. My dad was placed in her car with his belongings minus his arm brace, quad cane and glasses. When asked where these things were staff went in search of them. They could not be found. Mom was questioned if the glasses were ever at rehab with my dad. We are certain they were there as we observed them being paced on his face during a “window visit” . My dad was in a deplorable, unkempt state. He did not have his glasses on, the nurse Neil; last remembered seeing them before Christmas. He was unshaven, nails long and dirty, crusty debris on his face around his ears and between his toes. His diaper was obviously soiled with both feces and urine and he smelled despicable. No discharge instructions were reviewed. My mom was simply handed a packet of papers, told to read them and follow the directions, and asked to sign. My dad was sent home with out the medication he needed and no prescriptions either. His medication list was inaccurate/wrong upon review with his PCP. Without glasses, he cannot see and without his quad cane no hope of him helping to stand/pivot/transfer. Without medication- conditions untreated and at risk for imminent ER visit. I am so sad and disappointed by the lack of basic care. I have no idea how my dad could function at his best without his glasses on ( special prisms from a prior stroke allow him to see) How could he eat, watch TV or even use his phone or call button. I could not with any conscience recommend Cobalt rehabilitation services. What a true discredit to our most vulnerable population.

The staff was completely overwhelmed and understaffed. Food was awful - inedible. They left Mom on the toilet - unattended. Granted it is in time of COVID - the staff is stressed but Mom suffered through a 14hr seizure unattended. Inexcusable. Medications administered that no one can understand why such extreme dosage.

My husband was in Cobalt Rehab for acute care. It's clean and the people were nice. However, the day he came home, I had to take him back to the hospital with a UTI. The rooms were fabulous, huge, and meant for a single person. He was there for ten days and he seemed to like the food there.
