"What struck me right off was how positive the interaction between the person showing me to the room and the people in the hall was. It was not fake - the people knew the man and he chatted with each in a different way. My brother went here to get PT after a long stint in hospital. The PT staff were very helpful and encouraging and responded in a helpful manner to my questions and those of my brother. The facility was clean and it did not smell like an old folk's home, which is a good thing. The only complaint my brother had, and was borne out in other reviews, was he did not really like the food. Well, he still ate it! I would highly recommend this facility and our family is appreciative of their care for my brother."

"Several years ago, a family member was in need of aftercare in a rehabilative environment following a severe stroke, and we turned to Cristwood for help. I spent many hours with him there, and found this facility to be outstanding in every aspect; so much so that I commented to the rest of my family that if I was ever in need of such care, Cristwood would be my #1 choice. Fast forward to Fall, 2014. Prior to my 52nd birthday, my doctor gave me an early "present"-total knee replacements were desperately needed on both legs. The plan was to do them individually with a 2-4 day hospitalization following each surgery, and approximately 3 weeks of in patient rehabilitation at a nursing facility. Remembering my positive experiences at Cristwood, I contacted them to see if they could accommodate, and they could. Surgery on my left knee commenced at the beginning of November. Due to a severe infection, my hospitalization was longer than anticipated, and I was released to Cristwood on November 11th. From the minute I entered the facility, I knew I had made the right decision for me. The staff at Cristwood-from the housekeeping associates to upper management not only exceeded all of my expectations as to what a rehab center should provide to its clients, but it raised the bar completely. During my stay here, the medical staff worked very closely with my surgeon and wound care specialist to ensure that the treatment plan for this infection was the same across the board. CristwoodTransportation diligently handled the many appointments I had offsite perfectly, often juggling schedules to compensate for last minute changes and additions. Even my diet was changed in order to address nutritional deficiencies associated with 'round the clock use of the wound vacuum. In addition, the occupational and physical therapists not only put up with my moods, but they curtailed my exercise plan to effectively work for both of us (that would be me and my "crazy straw" wound vac). There were many days when allI wanted to do was cry or scream, and Cristwood's wonderful team of aides would do or say something to bring me out of that darkness and cause me to smile. The same holds true for the maintenance and housekeeping crews-Throughout my stay at Cristwood, there have been numerous examples of kindness, compassion and care bestowed not only on me, but on every single client; these attributes possessed by Cristwood's staff are what makes this facility head and shoulders above the rest. I am slated to leave on Friday, December 19th, and thanks to the combined efforts of all, I am well on my way to recovery. Cristwood was a major player in all of this, which is why I will be returning here in February following the second surgery. "

"AKA Careless Manner... Our dad was transferred the Manorcare AKA Careless Manner on high recommendations. It was our first family experience of touring and then admitting a family member to a SNF...so you be the judge: took over 2 hours upon arrival for my dad to get any personal attention. no formal care plan 100% NPO not observed! OMG 3 times while we were in dad's room Na's came in with ENSURE and Water for my dad! My dad, confused from stroke, could have seriously drowned, had he accepted it...esp. If had we NOT been there to observe. feeding peg tube was not flushed after use, pretty basic... failed to change simple 4x4 bandage at feeding peg tube, even after the initial change was so smelly, as they left in on without changing for over a day and a half. (Hospital recommended 2x daily) next day, RN was asked by family 5 times in 6 hours to change bandage as the smell was getting bad again--she never did by the time we left ...then we were informed in middle of night peg tube area was infected and they sent him to ER. 2nd day I arrived at 9am and my dad was not not checked by a Na or Lpn/RN until 4pm--this was a norm...My dad was incontinent at that time, so we had to change his overly wet briefs... not attentive to his specific needs. during a training session with new Na's one girl wouldn't even come close enough to bed while they turned my dad on his side...what a safety issue! saftey issues: I found my dad's bed high off ground and my dad was falling out of bed, I caught him before he fell. (He was high risk of moving, so bed was always to be left at floor level.) failed to keep dad at recommended 30% grade for breathing and preventation of choking. nursing supplies were just thrown into room everywhere, some on the floor, no order. As there appears to be no set care plans and order to patient care, hence, "Careless Manner," we as a family had to station several of us there in shifts from 9am to 9pm when we could. We kept notes of care given and needed amongst ourselves so the next family member knew what to request... After they sent him to ER, my dad spent a week in the hospital . He is now being cared for at a professional SNF. ;) There definitely is a management problem at this location. PS: Manorcare was fully aware of my dad's condition, and his special needs. They visited him in the hospital prior to admitting, AND they had his hospital records for review and approval... "

"Richmond Beach Rehab facility was just what I needed in order to heal from a broken right hip and survive all the difficulties that were a part of my recovery. It was a random choice made from a list provided by the social worker when I needed to transfer from the hospital. Sometimes there was a waiting time after I pressed my red button for help, and yes, sometimes there did not seem to be enough employees to handle the workload, but I found those who helped me to be dedicated to their work and truly concerned about my well fare. Their work often concerned helping with the very personal needs of commode use and trying to answer every call for help. I asked one helper why he did this job, and he replied, "It's what I signed up for." Only dedicated people should bother to apply for this kind of work. My needs were met every day, and the physical and occupational therapists were skilled and helpful. They challenged me every day to accomplish a more difficult task so I would be ready to go home. Each time I succeeded, I felt like I should put a "gold star" on my chart. I was assigned to a private room. The food was excellent, and we were allowed to list our food preferences. Activities were planned for each day, and I was able to attend three church services in the activities center and two musical concerts by guitarists who sang and played. After 24 days I was able to go home and had learned the skills necessary to navigate around my condo with a walker. I would highly recommend this rehab facility."

"This facility has continually shown to be a caring and compassionate place for my loved ones. I really appreciate the staff's commitment to giving the residences a meaningful time there, and caring for them, no matter their condition. The facility is not the fanciest, or the newest for that matter, but I always love to see my grandmother smile there. She was frightened for her entire adult life of nursing facilities, and this place is the only one that I have found to give here the care and love she deserves to have. The managers have befriended our family, and we really love that she has been so well cared for there. The only complaints I have are of one staff member, who is somewhat unfriendly to our family, but she has been reprimanded, and we are glad to be able to have such a great place as part of the community. "

"My husband was in Shoreline Health and Rehabilitation Center for six weeks. They were great with him. They were so cheerful and helpful. The orthopedic doctor really doubted that he would be able to walk on that leg again because he's in his 90s now and didn't think it would heal, but they worked with him out there, did physical therapy, and he's walking on it again. They had wonderful physical therapy there. He had a room all to himself, so he liked that. He could look out the window and get in his wheelchair and wheel himself around, too, if he wanted to get out of there, and wander and visit. I asked him how the food was, and he said it's all good. We didn't go into the dining area. He had his meals in his room. They brought it to him. The staff couldn't have been nicer. I can't complain about anything. They were constantly cleaning, and they kept it neat. There were exercises they could do sitting in their wheelchairs or regular chairs, and I joined in a couple of times. It was fun. We donated a bunch of books to the little library they have there, too. They had different things going on, and it was just nice and very pleasant. They have areas outside where you can wheel yourself right out or wheel someone else out and sit out in the sun. We did that a lot. They transported him over to a hospital, so they could do X-rays and scans on his hip over there, and got him back to Shoreline."

"This facility appears to provide good care. Special services are available for those who need them, including physical therapy. The physical therapy provided was evidently of good quality. The facility also offers a variety of other special services, including occupational therapy, speech therapy, dentistry, podiatry, and nursing care, each of which is available only for those who need those particular services. No evidence of injury to any resident was demonstrated. Staff members were pleasant and acted friendly. House rules required some restriction of residents' freedoms, such as an inability to go out alone. Not all residents were pleased by the rules.Rooms are shared between two people; when occupancy of the facility is relatively high, residents have roommates whom they do not know. This is not popular with everyone. Curtains can be drawn between beds to provide some privacy. In-room television screens allow residents to entertain themselves alone with a variety of interesting programs if they choose to do so. Magazines are available for those who like reading and are able to see well enough to do it.A variety of nutritious food was available. "The wonderful things about the Hearthstone come easily to mind: 1) Green Lake and its park is our front yard. Almost every morning, I walk along the lake and savor the natural beauty which reveals itself with such abundance in each season. The people I meet, some of whom have become friends, the children, the dogs, the birds, the turtles, the squirrels, and the bunnies combine to create a delight at the start of my days. 2) Area businesses provide a variety of contexts for a vibrant multi-generational, multi-ethnic community to gather and enjoy their services; shuttles and buses take us where we need to go. 3) As a resident of the Hearthstone, I deeply appreciate our caring staff, the clean and well-maintained continuing care facilities that offer me support throughout the range of my needs. "My husband was in Columbia Lutheran Home. All of them were so kind, patient, and helpful. They had a terrific room for the exercises he needed for rehab, and good equipment, and they were also encouraging, so it was good. He shared a room with another man. They got along just fine. There was plenty of room, and the size was good. He didn't complain at all about the food. He thought it was just fine. They had church services there for Catholics, Lutherans, and whatever. They could wheel themselves around and do what they wanted, more or less, as long as they didn't do anything to hurt themselves. I thought it was a good place. They would wheel the people outside, walk around the block, or do something with the wheelchair. Other than that, I can't say anything about activities there. He was in his room most of the time, or he was getting physical therapy."
