Situated on the east side of Fairfax County in northern Virginia, the city of Alexandria has over 159,400 residents, with 11.3% being aged 65 and older. Ranked 13th in Virginia and seventh in Washington, D.C., the Inova Alexandria Hospital is high-performing in eight procedures and conditions, including diabetes and nephrology. The Inova Mount Vernon Hospital is high-performing in procedures related to orthopedics, as well as three other conditions and procedures, according to the U.S. News and World Report.
Sometimes referred to as skilled nursing, nursing homes offer a higher level of personal care that cannot be provided at an assisted living facility. Nursing homes are for those who require 24-hour monitoring and professional management of a chronic medical condition that affects everyday living. In Alexandria, the average cost for a semiprivate room in a nursing home is $11,726 per month, while the national average is $7,756. With that high of an average, other senior living options, such as assisted living and home health care, can be a better fit for seniors who don’t need skilled nursing care.
This guide includes a list of several local resources for seniors, along with cost comparisons for different levels of senior care in Alexandria.

I had heard horror stories about nursing/rehab facilities but I was truly hoping and praying that if the time came for a loved to HAVE to go to one of these places, they would provide at minimum professional and basic care as they boast a “skilled nursing” facility.
Sadly that was not the case at this facility. My mom arrived shortly after 8 PM, transferred from the hospital where she had stayed four days after a terrible fall resulting in broken ribs and hematomas (due to landing on cement steps) on her head and pelvis resulting in difficulty walking.
Within two hours of arriving, the nightmare started; the hospital bed didn’t work and nobody bothered to come help. She asked for an extra pillow at least to TRY to make herself somewhat comfortable. They put a disposable underwear on her even though she refused as she had not had the need even while in the hospital – she was already going to the restroom with assistance; had even started walking a few steps in the hallway during her physical therapy sessions at the hospital. She just wasn’t ready to navigate the stairs to get to her bedroom and of course she doesn’t have a hospital bed at home to help her change positions… The pain medication that was given to her didn’t even take the edge off. The first night she called several times to go to the restroom to no avail. There were two females who not only ignored her calls for assistance but also proceeded to make fun of her accent (now mind you these two individuals are foreigners themselves with extremely heavy accents)
When we tried to address these issues early in the morning (this required several phone calls attempting to reach the social worker or any other staff member – I won’t get into that as that would require a whole other long paragraph) the day staff made changes and she was more at peace, her bed was fixed, stronger medication was given finally around 2 PM -she even tolerated physical therapy. They helped her go to the restroom. We trusted that things were resolved so we allowed her to stay one more night in hopes that she would get stronger with the “skilled nursing staff” they boast of!!!
The same two females returned for the night shift, same ineffective pain medication was given. (Unbeknownst to us, they kept her in a disposable underwear the entire stay) once again, the call button that the social worker made sure she trained my mom on 🙄 (my mom knows how to use the call button thank you very much – it seems to work just fine during with the day staff) was ignored. Early in the morning after the fourth time of her requesting help to go the restroom, one of those female staff members came into the room, proceeded to turn off the call button and went back out of the room. (A young female assistant came in later wanting to help you at one of those females stopped her from doing so reminding the assistant that she had the disposable underwear)
My mom was fearful of these two female staff members as their demeanor was aggressive. They hurt her physically while wiping her down (even though my mom told her she didn’t need to be – she had just taken a shower right before she was discharged from the hospital; with assistance of course), when they shove the bed pan under her injured pelvis, when they wiped her off to change a disposable underwear that she didn’t soil…
She recalls several residents screaming in pain and agony throughout the night – male, female, some sounded younger than her, others older than her. She was terrified of retaliation from these female staff members. She begged me not to say anything but I had to before we picked her up. Our wish is that they will make several changes for the sake of those residents that are staying; those that don’t get to go home.
To say that she has been traumatized by the lack of professionalism, compassion and care from the Night Shift staff at this facility would be a huge understatement!!!!
She still hears the resident screaming…

PERIOD OF CARE: August – October 2020! This is an Absolute Awful place. They almost killed my brother who was in there for physical rehabilitation. That included speech therapy, occupational and physical therapy. They gave him 2 medications that shut his bladder down. Then they put a catheter in and stopped giving him fluids. They were apparently too lazy to empty the bag. I didn’t see him because of the Pandemic and I had no idea how sick he had gotten. I finally took him to a urologist about the catheter and he collapsed in the parking lot. He was rushed to the ER and spent 3 weeks on IV antibiotics.
In addition, while he was in the hospital , his electric shaver, Galaxy S9 phone charger and some clothes were stolen!
To make matters worst, they still intend to collect a co-pay. I filed charges against Woodbine with The Adult Protective Services for Abuse and Neglect

I would not advise anyone to have their relative stay at Manorcare. My sister was there for a month and they tried to discharge her because they said she "threw herself on the floor" and was yelling. She told me that she fell on the floor and no-one would come to assist her! This facility was not attentive to my sister’s needs until they were in a position where other eyes were on them, like people from the Long term Care Ombudsman program and DMAS. An infection landed my relative in the hospital and she said she was glad to leave this place. This is not a place we could recommend for anyone.

Envoy rehabilitation and nursing home at 900 Virginia Avenue in Alexandria, Va. is a total nightmare. My husband was there April 15 to May 7, 2022 following a hospitalization for a fall.
The nurses and aids were unkind and unhelpful. Yelling outside the door waking him up and disturbing him. They didn’t greet him nor did they help him to the bathroom. It took 20 minutes to respond to a call bell. I needed to hire someone when I couldn’t be there. There was no daily care: teeth brushing, washing hands after bathroom, hair. The water container was never cleaned. Maybe rinsed but ice was continually added in the container. Disgusting and unsanitary. In addition it wasn’t placed where he could reach it. The bathroom was filthy as well. The food was nauseating. I have pictures to verify. He lost 18 pounds since his last doctor visit a month ago. My husband was shutting down. No one even noticed he wasn’t eating or drinking. When I talked to the nurse administrator, about the weight loss she asked how I would know. After being married 44 years i knew. She said they weigh on Wednesdays. Never gave me the weight he was when he came in nor the weight during the time he was there. My husband was shutting down. He was misdiagnosed as having shingles so he was kept in his room without being able to leave. No windows to open. No walking. Even though I knew it couldn’t be shingles because it didn’t follow a nerve path but she showed half a picture to the doctor and over the phone the doctor agreed with her. I was unable to talk to a doctor. I talked to our private doctor who told me to talk to facility doctor. i was not ever able to talk to a doctor. The nurse told me to tell her what my doctor said. Very inappropriate and a violation of our privacy. One nurse refused to look at the rash as it continued to worsen. No pain. No itch. Staff asked me whether or not to give the shingles medicine. Me. I couldn’t even talk to the doctor. Very bad medical care. This place is a totally awful. Skinny people living in hospital gowns in dark rooms. Heartbreaking. Social services need to go visit the first floor of Envoy to help these people we left behind. The first floor was where this all happened. Thank you. Marilyn Siwek

Well, they work with them here every day and if they don’t want to get out of bed and exercise…THEY MAKE YOU DO IT!!! Not in a harsh way or to be mean, but to help get you back to moving. I cannot say enough good things about this rehab team and how dedicated they are.

I could also see that there was a mother and very young child visiting talking loudly someone in a patient’s room, and you could hear the child telling his mom that he was hungry and wanted some crackers from a vending machine. The mom told the child several times that she did not have the money to buy them and she was sorry. Just then the sister walked into the room and surprised the kid by giving him some peanut butter and cheese crackers. I thought it was so touching to see someone be so kind to a stranger.

Stoddard Baptist Nursing Home does not smell like old people, it does not smell like antiseptic, it does not smell like a hospital, and it does not smell like anything. The place is immaculate. It is so clean. They have a huge staff. I think they have 164 beds, and they have a staff of like 200. They have a lot of people attending to the patients. I went there at different times of the day, and every time I went, my mom was clean, happy, and healthy. They do not just leave people lying around in beds getting bedsores. I like the way they get them up and out into the day room where they can interact with other patients — or other residents rather — because they do not treat them like patients. They treat them like people who live there. The staff is very attentive like they are there all the time. They are very caring, and they treat the residents with respect and affection; I love that. My mother is very happy there. She’s well cared for, and I’m happy. She has dementia, so she does not do too much of the activities, but they do have games. They go on outings even. It is a very nice place. I am very comfortable leaving her there. The only thing I do not like about them is the parking situation. They have a parking lot, but it is very small.

It’s a good community and I personally have no complaints. There were some lapses in communication and stuff, but that’s to be expected sometimes. There were some times when my sister didn’t get any information she needed. The coordination between doctor’s offices and financials could have been communicated better, but we’re unsure if it’s solely because of Arleigh Burke Pavilion or the doctor’s office or a combination. Overall, it’s a very nice community and they seemed competent. Dad had a very nice room, it was very functional and very pleasant compared to other places. It was non-sterile compared to some other communities I have visited which were like hospital rooms, his was a little more comfortable appearing and more home-like.

My grandmother spent the last year of her life in the Alzheimer’s unit of the Washington Home. The facilities are quite pristine and well cared for and despite it’s large size and high number of patients it really does feel like a close community. The nurses and staff were always available to give status updates although you really have to plan your visit if you want to meet with the doctor. They have a good amount of activities for both the patients and their families. There are also a lot of volunteer opportunities. Visiting hours are fairly standard and the staff can sometimes be somewhat abrupt when the hours are over. Safety was our only main concern while she was there. Because of the unit she was on, there would frequently be other patients walking into her room and upsetting her. Not much can really be done about that I suppose but it really upset her sometimes and made her stressed out at times.

Financial Assistance for Nursing Home Care in Alexandria, VA
Many seniors and their families use some form of financial assistance to help them pay for nursing care. The main options available are Medicare, Medicaid, and veterans benefits. These programs can be complicated, especially when it comes to benefit terms. Below, we give a brief overview of how these programs may be used towards paying for skilled nursing care.
- Medicare: Medicare will typically cover all skilled nursing costs for the first 20 days of one’s stay in a nursing home and a portion of the costs until day 100. After 100 days in a skilled nursing facility, Medicare will not cover any part of the cost of the stay. While this is adequate when short-term care is needed, those in need of long-term care will need to either pay out-of-pocket or use another source of financial assistance.
- Medicaid: Medicaid covers most of the costs of living in a skilled nursing facility for those who qualify. Care, room, and board are covered with no time limit, but residents may be charged for extras like specially prepared food or cosmetic services. Medicaid eligibility standards are strict and complex, so not all seniors are eligible for Medicaid benefits.
- Veterans Benefits: Veterans receiving a VA pension may also be eligible for the Aid and Attendance benefit administered by the VA. Aid and Attendance is a monthly allowance that beneficiaries may use to pay for their long-term care, including skilled nursing care.
If these options aren’t available to you, check if your loved one has long-term care insurance or contact your Area Agency on Aging to ask about any local financial assistance programs for seniors.
Free Resources for Seniors in Alexandria, VA
In this section, seniors can find several community- and home-based resources to help them remain comfortable and safe in their homes. Some of these resources may help to delay the need for nursing care. Programs range from those that provide long-term care screenings to loans for low-income homeowners, which can be used to improve the level of safety and living conditions in the home.
Seniors can reach out to the city’s Division on Aging & Adult Services regarding financial aid programs. Along with providing information regarding long-term care facilities, family service specialists can help seniors compare local facilities to find the best match. The agency also educates seniors and loved ones about adult abuse protection, as well as nutrition and overall health. Typically appointed by the court, volunteer guardians can speak on behalf of incapacitated seniors in Alexandria.
This program serves low-income homeowners in the Alexandria area by providing no-interest, deferred payment loans to help seniors age-in-place in the comfort of their own home. To be considered eligible, individuals must be below 80% of the area’s median income by family size. Funds can be used to improve the quality of the home, along with installed safety features.
Administered by the Virginia Department for Aging and Rehabilitative Services, Home & Community-Based Services allows seniors to receive care in the comfort of their home rather than an institutional setting. The program is split into two categories: skilled care and in-home support. Skilled care provides care services by licensed professionals, including registered nurses and rehabilitation therapists. In-home support caregivers focus on housekeeping and meal preparation, as well as personal care and social work services.
Provided by the Division of Aging & Adult Services, Long-Term Care Screenings are available to seniors aged 60 and older to ensure they're placed in the appropriate type of long-term care facility. A family service specialist and public health nurse can assess whether seniors need home-based care or care from an assisted living facility or nursing home. To qualify, seniors must meet financial requirements by applying for Long-Term Care Medicaid for home-based care or nursing home care, or the Auxiliary Grant for assisted living facilities.
