"We engaged this organization to conduct a competency review of our mother, in preparation for a court hearing. The report we received was sloppy, poorly written and came to us unsigned. A second assessment was requested by our attorney. The nurse (Carol) did say it was going to take time but failed provide an estimate. We were shocked to be billed for an additional three hours to fill out a simple State form. It is incomprehensible that a professional could not complete the form in less than an hour. In addition, the information conveyed in the form lacked medical detail and it essentially was a rehash of my personal conversation with the evaluator. Obviously we are not happy with the service provided by this provider and would not consider using or recommending. "
