If I could give a negative rating I would. This place is horrible. I was here for rehab after total knee replacement. Two times I waited 3 1/2 hours after using my call button, then received an attitude when they finally did answer. They put drinking water into my cpap machine which is supposed to have distilled water only. I was sucking salt into my lungs for 2 1/2 days which caused severe breathing problems. When I told them I couldn’t breathe I was ignored. I didn’t receive my meds on time. Food was horrible. I had to get my friend to bring me cereal and muffins from my house. Plus I had to ask for water, which could take hours to get. I asked for a bedside toilet, was promised one for 3 days only to find out they don’t have them at their facility. I wasn’t admitted until the same day I demanded to leave! They didn’t even have any written info on me to give to the home health people. All they wrote was I had a total knee replacement, no vital signs, no med records, nothing. This place should be put out of business

I was brought to the facility after sitting in emergency room for three days. I was prescribed pain medication among other things and all info was sent with me. The facility shut off power for approximately 4 hours when I first got there, I was talked to like a animal, the call buttons didn’t work and I was denied my medication until almost midnight staff claiming “ there’s a process and I’d just have to get over it.” My reason for going to emergency and being admitted was due to severe pain from bone on bone hip that will be replaced. The food is disgusting and breakfast can come anywhere from 7:00- 10:00 am, lunch 1:00-3:00 pm and dinner 6:00-8:00. It’s supposedly cooked in a trailer somewhere and then brought to the facility much later. They ignore the call button and if the do answer anywhere from 30 minutes -2:00 later they are only the aide and then pass the request/need on to a nurse who may or may not respond at all. There is no security and when a friend came to see me after 9:00 pm They rang the bell and no one answered so they simply put their hands on the sliding door and pulled it open with ease.

This facility overall is horrible. They are understaffed and do not answer the contact number half the time. U cannot get a direct number to the nurses station. My family member was not given medication AT ALL the first 24 hours and 2 days did not receive breakfast one day or lunch another day and are bed bound. We know this because another family member was present. Frequently they cannot find the call button, makes me wonder if they are relocated on purpose so they do not get buzzed. Some of the staff re very nice and work hard. The facility will be reported to the medical board for patient neglect, Centers for Medicare and Medicaid for lack of care. And meeting facility standards. The next time any of the above happens Adult Protective Services will be called along with the local news station. Nursing home Administrator is NOT doing their job.

THE BEDS DID NOT HAVE ANY RAILS OR BED ALARMS. MY MOTHER FELL OUT OF THE BED. WHEN I WAS THEIR THEY LEFT A PATIENT SITTING FACE DOWN ON THE TABLE FOR OVER 45MINS. Please visit your loved ones on a regular basis if they are in this place.

While I was at the facility, it ran out of diapers. I ended up in my room naked from the waist down. They also ran out of stool softeners of all things. They also ran out of my Valium. What kind of a nursing facility runs out of stuff.

There is a new crew here. They have a very good attitude and care about the patients. I loved every one of them. Wanda Scott Activities Director is really keeping the patients entertained & busy. Mother loves her. I'm so thankful. They are going to display new food options. Mom loves her little space. Easy to decorate

I was in short term for 3 weeks. There was a few good staff but so short handed. Didn’t even have wipes to clean my ostomy bag. Had a blow out with my ostomy bag had to wait two hours for someone to come help so I had poo on my skin for two hours. Some C&A’s refused to help clean it up. Food was pretty bad never consistent on the time it was served. I had a corn chip I had dropped on the floor and it stayed on the floor for the entire time I was there. They didn’t want to get me up and walk with me because I had 3 catheter’s and I was too much trouble. But if they had I would have gotten out and home sooner. I needed wound care for a severe would ( hole) and it needed to be cleaned and packed at least every other day. I went as long as 3 days without any attention to it. I pray I never end up it another place like this . I would soon go on to Glory if that’s the case. This place needs to be shut down. I feel sorry for the old ppl that has no one or can’t speak for themselves

If you love your family member don't take them here please find somewhere else before you do this to them. I had a family member here which died there Administration staff was more about concerned with the money, no compassion for the clients short staff very smelly place please

Staff does not care at all. When you ask a question, you are told they don't have time. The only caring staff is the PT and speech. The PT change and clean patients then have no real time for doing their job. Pts shouldn't be sitting in their waste!! They ignore family requests even tho they say they will take care of your loved one. Food is cold and looks disgusting. I'd give negative stars if I could.

my mom has been in the rehab center and i visitied from NYC, a couple of weeks ago. being in health service, for 40 years, this rehab center , is terrible. FOOD cold, NO staff, HOUSEKEEPING isnt good, CHANGING, is too long a wait for any resident (4to 5 hours ?), no this place is under horrible management. I feel for the residents here, and i will file a complaint, for i have no FEAR , of that

As a caregiver for my mother, this facility was a godsend compared to another facility that my mother had resided in. I've had multiple interactions with the staff (i.e., receptionist, doctors, nurses, social workers, CNA, and therapist(s). While the work can be tedious, the staffing remained professional. The facility itself is clean as well as the individual rooms. There was never a lack of communication. Always detailed and thorough which I personally appreciated. These are hard times and I recognize that when it comes to the management and care of my loved one. Thank you Old Dominion Rehab and Nursing for the great work you are doing during these uncertain times.

When my dad went into the emergency room, St Francis Nursing Center was the only one that was able to take him for rehab. So far, everybody's been good. I've talked to the social worker and she has kept me abreast of his physical therapy. He never had a complaint about the food, and that's the first time it happened, because the last place where we had him in, he had a complaint about it. I asked him the other day what he had and he said it was pretty good. I will have a meeting with them today actually because he's complaining about his call bell. It is not working, and so he has to yell out. That's the only thing. I haven't been to his room yet. He only had one vaccine shot, so they had to quarantine him for 14 days. He can't come out nor can I go in. I got to drop off clothes, some toiletries, and stuff for him though at the front desk. The outside looks peaceful, surrounded by trees and everything, and they have a big hospital right across from it. So, that's good.

My Mother was there for rehab when she had knee surgery. Her room flooded every time it rained! We would alert the staff, standing water on her floor for a whole day! They are short staffed, some staff very rude & uncaring. Don't be in a hurry if you need assistance, heavy smell of sanitizer/sickness when you enter the facility, very gloomy, we moved her out of there after only 2 weeks, her PT was very uncaring & harsh, they actually caused damage to her repaired knee, that they never admitted to doing. I DO NOT RECOMMEND this place!

First I want to say they are understaffed, under trained and noncaring as a whole. However, there were a few staff members that seemed to like their jobs and care for the patients. My wife was only there for a month during that time they had no hot water for 10 days and no cable for 2 days. After calling for assistance the wait could take up to 3 hours on some occasions but at least an hour on most occasions with multiple calls. The managing staff is the root of most of the problems ( no follow through, no returned phone calls and the discharge was horribly managed). I had to remove her from the facility way before she should have left but it was the best decision I made. With all that said the PT staff was amazing

My father has been a patient at St Francis for 2 in a half years, he suffers from dementia and he is 98 years old.I am very pleased with the staff and the care he is receiving.The staff attends to his daily needs and they closely monitor him.My father likes his meals, they keep his room clean and change his linen daily.There are all sorts of activities that caters to all levels,even those who are wheel chair bound. Nursing home residents are at a high risk for infection, serious illness and death from covid-19 so they are monitored closely.. Testings are conducted at the nursing home and of now there are no suspected or confirmed cases of the coronavirus. The nursing home is following the guidance of the CDC and the Virginia department of health.Every Wednesday during the health crisis you can call 757-886-6046. I am very happy with the care and attention my father is receiving at St Francis.

Visiting St.Francis was a wonderful experience .I was pleased to see how well the people were cared for when brought to church service.The staff was very friendly with the patients and was very helpful to the visitors.The facility was clean and did not have that "nursing home" odor. I am looking forward to visiting again.

St. Francis is a facility which I would not recommend. Not all of the nurses and CNAs are non-caring, but the majority of them need some extensive training in long term care for the elderly. Residents sitting around the nurse's station are not watched as some (my mother) have had serious injuries due to falls. Nursing staff does not show that they are cognizant of the needs of their residents for I have had to diagnose/recommend the care. Nurses appear to be pill pushers only. CNAs are on their cell phones during their shift with no discrepancy as to when/where they should be in use. Some feed two residents simultaneously while on their cell phones. Both groups use their cell phones at the station ignoring my presence. Have found my mom in a soiled bed too many times. The disposal units for soil items are on the floor most times and even during feeding hours. CNAs are not professional; speak harshly to the residents and movement with them like they are ragdolls. Spend no time in feeding those not capable of feeding themselves. No facility for long term care should operate without a kitchen on the premises. Food is terrible and lack presentation and food choices. Floors throughout, except for the areas for rehab, are filthy and so are the wheelchairs. Due to its physical location, snakes have been seen around and in the building. Heating/AC does not function properly throughout the building. Stifling in many rooms whereby residents have to bring their own fans. In spite of all the problems I have experienced there, the PT department is the best, always catering to your needs. All of the therapists are Great!

We have had my Mother In Law at St Francis for about a year now. Last Fall, we had several issues including poor care, missing bedspread, they had thrown out her religious painting by mistake and told us they would find a replacement, which they did not. They seem to say yes to everything but do nothing. My MIL was scratched on the face by another resident and was moved to another wing. They made some effort to improve her care and we were pleased as one particular nurse made sure she was being changed regularly and had her hair done as we requested. That nurse was recently fired and the whole team of nurses were replaced with a whole new set of people whom my MIL does not know. This crew seems to like to stand around the nurses station and laugh and joke a lot . When I recently visited, I pushed the call button for my MIL, no one responded, so I tried 4 more times. No response, so I finally went out to the nurses station and asked if the call button was in fact working, they looked at me and said "yes, it's on" These people have no clue. My MIL was dressed in a zippered robe which was unzipped, she had not been changed, eaten breakfast or lunch. Her roommate told me she had nothing on all morning, just a sheet covering her with her brief, Her hands were filthy, and black gunk was under her nails. When I asked why, they responded that she did not want her nails done. They do not wash her hands after she eats or before, and she eats most everything with her hands, I purchased a new bedspread for her as the last one went missing. When I bought the new one, I told the nurses, it was brand new and cost $160 and I made sure they knew. They're response was "Make sure you keep the receipt". So now this one is missing. Yesterday, we received a call that they had given my MIL someone else's prescription.....twice, but it was nothing to worry about because she had no ill effects and it was only an Antibiotic. Today I went in and asked for her hearing aids to be put in, they are suppose to put them in the morning, but only put them in when we visit and ask. They could not find them, turns out my MIL's roommate went to the nurses station and told them where to find them. They wake my MIL up at 5 or 6 in the morning, before the 7 o'clock shift comes in, dress her and put her back to bed, this is ridiculous. The level of care here is appalling! I would not recommend the facility to anyone!

St. Francis Nursing Center is AWESOME!!!! My loved one was a patient there recently and I never had to worry about the care that was provided. The therapist were all amazing as well.. St. Francis was actually not even a choice for us, but they made us feel welcomed before we even stepped foot in the building. The admissions nurse, Jamie reached out to us while we were in the hospital to let us know that we were accepted for admission and even came to visit us and to answer any questions or concerns that we may had and invited us over for a tour. No other facility even bothered to call us, let a lone visit us in the hospital!! Once we got there, Courtney and Debbie were very helpful and competent in explaining any insurance questions we had. When we arrived we were welcomed by a cheerful and delightful lady named Renee at the desk. Nursing was prompt and helpful. Rebecca and Heather are above the best of the best! The DON, Farisa is wonderful! How many managers do you see out on the floor helping their staff? Not too many, but she was A1 in my book. St. Francis is world class and I would definitely recommend this facility in the even that anyone close to me needs 24hr nursing care or rehabilitation!!!! Way to go St. Francis!

This is one of the worst nursing homes I have ever visited. The staff don't care about the patients or their well being. If they ring the buzzer for help, the nurses at the desk just sit there playing on their phones, laughing and talking with each other. They act just like they don't hear the buzzers buzzing. My heart aches every time I go there to visit my friend The patients are up at the desk asking for help and they ignore them. If these so called nurses don't like their job, then maybe they should find another profession.

Do not send your loved one here. My mother was a resident for about a week, In that week, we saw all we needed to see. On the weekends, it is understaffed. Why on earth do they think elderly and/or sick residents do not require as much help on the weekends? A neighbor next door to my mother was an elderly man who needed help. He yelled for help for 20 minutes one Saturday. I left my mom to see what was wrong. He had fallen halfway off the bed and I could not get him back on myself. I searched for someone and couldn't find anyone for several minutes. There was not one staff member in sight. That is extremely worrisome. The staff who change the sheets are mean. They were very cold to my mother and I noticed some of them wear their name tags turned around so that you can't see their names. I do not buy that the name tags accidentally turned around. They were always turned around. There is a hearing impaired nurse. Why on earth would you employ a hearing impaired nurse in a nursing home?! Elderly already can't speak loudly. She could never hear me or my mom, and that made me worry when we were not there. Staff has communication issues. Any time we were assured something, it don't materialize and there was always someone who failed to communicate properly in the chain of events. It was frustrating,

St. Francis Nursing Center is excellent. My daughter's room is very nice. She has her bed, her television, and anything that she needs. The dining area is very nice. It is well kept. I would recommend it to anyone who qualifies for it.
