Having to select a "step-down" facility for a family member with little notice, I had limited time to research facilities, and was also faced with the issue of most local facilities experiencing current Covid patient cases except this one, whose Covid had been limited to several staff cases early on . So, in spite of lackluster reviews and giveaway warning indicators such as phones being unanswered until calls are dropped by the system, I decided to have my family member transferred to this facility, not realizing I would (no exceptions) not be permitted to visit until the end of a quarantine period. At the time of transfer from a rehab facility to Sundance, my relative had been feeding himself, using exercise equipment, and conversing, When finally allowed into Sundance weeks later, I was appalled to find he appeared within two weeks to have lost significant weight, wasn't eating, wasn't talking to staff who claimed he wasn't being medicated, yet I could scarcely rouse him. An untouched covered tray of unappetizing cold food sat out of his reach. I was advised that an aide would come by to assist w/ eating, but he generally refused it - cold, dried out food - go figure. The lower window frame in the room had significant damage with broken wood/trim, and exposed half driven nails. Window damage debris on the floor remained unvacuumed so the construction was started and remained unfinished before moving in my relative, and no one even bothered to clean the room before allowing him to be moved in. The bed had been lowered and pushed against an electrical outlet, forcing the plug prongs in the live outlet to be halfway out and exposed at face level of the patient. Other issues may have existed, but I'd seen enough and began making arrangements the same day to discharge my relative. Interesting to see some of the slipshod practices that occur when facilities can hide behind Covid closures....wouldn't leave my dog here.
