"My client is at Crown Point Christian Village. They have activities going on every single day. They know the residents and care about them. They call the residents by name. They even recognize me. These people know exactly who everybody is, and I like that. They get them up there, interacting with everybody on a daily basis. I see a dramatic increase in my client's momentum, her thinking, and everything else within a short period of time. She's been happy. Her room is a little small. She's sharing a double room. The window side of the room is much bigger than what she has, which is the door side, but it's very accommodating. The facility is very clean. It's a fairly new facility. She says the food is good. They have physical therapy, and it's quite adequate. They're consistently encouraging the residents to keep doing as much as they can. So far, I have zero complaints with this facility. I was very impressed. The only bad thing for me is the drive. It's a very small community. It's very friendly. I highly recommend it to anyone. They're on the expensive side. They do accept Medicaid as well as private pay."

"We went on a tour, and Golden Livingcenter looked like it would be the place for my mother-in-law because she has dementia, and they have a dementia unit. The room was spacious, but there was no kitchenette, and the bathroom was kept separate; it was not in the same room. It also looked very clean. There were several dining areas, and they looked fine. They had quite a few activities for the people. The staff was very polite and very nice. "

"My family and I wanted to thank Sebos for allowing us to visit my brother. Jackie C., Guests Relations Director, setup a patio visit. It was great to be able to spend time with him, yet still keeping everyone safe. Jackie is a natural care giver. She was very nice and very professional. Due to the virus, we weren't able to tour to the facility. I feel much better after meeting her and the nurse that was attending him that day. Kudos to Sebos!"

"My husband was at Symphony of Crown Point. It was a great and very upscale facility. The staff was very friendly. When you enter the building, there was a cozy and warm feeling. The fireplace would be lit if it was a cool day. The chef was phenomenal, and the food was very tasty. The activities were great. The rooms were wonderful because they were relatively new. That is more of a "from hospital to a home" facility, although upstairs it did have an assisted living section. The therapists were phenomenal. I lived there with him and I slept in the recliner. He damaged his motor skills, arms and legs, so he needed someone to assist him, but I didn't expect them to do that 24 hours a day."

"I visited The Waters of Hobart. They were very friendly. It seemed very accommodating and clean, and they could handle his medical needs. The rooms are double occupancy. So, he would have a roommate, but I think that's a good thing to have someone else with him that he can talk to. The rooms seem very nice. They have activities like bingo and some other games and gatherings that they do."

"My mom was a resident of Park Place of St. John. The people were very friendly. There are a lot of activities going on that seniors could participate in. The staff was very helpful. There's always someone at the front desk or in the office. They answer questions. I think the only issue really that came up with my mom was related to her dog, and that wasn't so much a facility issue as it was her dog really wasn't suited for that type of setting. The food was good, but the portions were small for us. It's very clean, bright, updated, and the decor is very nice. Everything looked really good."

"My dad just moved into Great Lakes Healthcare Center a week ago. I like it. It's pretty nice. He likes it better than where he came from. It's really a good sign that he likes it. The staff are very good. My dad likes it better because the staff is better than they were over at the other place. He has only been there a week. I see him almost every day, and he just brags about it. As long as he's happy, I'm happy. The facility is spectacular. Whatever family you recommend, they're going to like it. The rooms are big. The facility is clean and very nice. They have a two-wing section there, and that's why I really like it. Right now, my dad is in the rehab part. He'll be switching over to the nursing home part today or tomorrow. They will only allow clothes baskets. For extra space, you can bring in your own dresser and your own TV. The food is fine. They do provide cable. They have bingo games. They have other kind of activities. The activities are very, very good."

"My mom had fallen and fractured her hip, she had surgery, and so we decided to put her in Symphony of Dyer for more rehab. It was pretty good, but my mom didn't last there long. She passed away, but the short amount of time that she was there, it was satisfactory. The staff was friendly and available. My mom had to have the pureed food. The place was very nice and clean. The outside looked very nice. I liked the way it was. It was a good location because we had one sister that lived close to Dyer. It was a nice area."

"I liked the Sheffield Manor. I think it was a smaller facility, but they had full apartments, and they were either one or two bedrooms. The other thing that I liked about it was that it had one price for a level of care, and I really liked that. They did allow an animal. I thought it was very, very clean and the staff was very nice to my sister-in-law. The only negative I would have about it is that the hallways werea little long to get to a particular activity place, but she would have had a hoveround to get to places. I thought they had about the right amount of activity. They have a theater and a beauty shop. I thought the price was very reasonable plus it was month to month. I didn't have to put in an initial fee to go there. I liked this place. I thought the apartment was so nice, and the closet space was wonderful. "

"My mom was at Miller's Merry Manor. It's beautiful and nice. The rooms were good. They were clean and kept up. The staff members were very caring. They helped my mom and everything she needed to do. They assisted her. They were very kind and very respectful. They offered her rehabilitation, physical therapy, and occupational therapy. They fed her three times a day. They played games, and they took them outside. The food looked good."

"My mom is in a nursing home called Brickyard Healthcare - Portage Care Center. It's an older nursing home, but they're really, really good in their memory care. The doctor is always in there checking everybody out, has caught a few things and got it corrected, and right now I'm pleased with them. They keep a pretty good eye on her. The doctor comes in more than I thought they would in a nursing home. When I go into her room, which she now shares with two other memory care people, it's nice and clean. The floors shine. They keep the place nice and clean, and it doesn't smell, and that's amazing. It doesn't matter if I call during the day or it's 8:00 at night just to see how my mother is doing, and the staff members always are very polite. If I have a question about something in her room, they will go in and check it and call me right back. I think that's wonderful to be able to rely on them. And if there's a problem, like if she doesn't want to take a bath or something, they call me and let me know if it's medical or how her moods are doing, and things that are very important to me. With regard to activities, my mother loves bingo, and so they have bingo. Sometimes they have some specials where they bring food in with fruits and everything. It looks beautiful. They have coloring and they story-tell to keep them busy and occupied."

"The overall impression when you first enter is very unimpressive. The facility is very untidy and has a foul odor. Patients are under supervised, based on the volume of falls that occur on a monthly basis. The staff are uninformed. They have no idea of a resident's past medical history or what needs they would need to help them. They are not trained in emergency first response to save a life until an ambulance arrives. I have experienced many instances of what I believe is negligence from staff members. "
