"My mom has been in this facility for a year now. I can honestly say at first the care was basic. Now they have definetly upped the ante in her care. They watch out for her esp taking the time and effort to make sure she is eating bathing and getting her meds. Trying to get her to interact with the other residents. Thy know her very well and know what to expect from her. Also the place while not decorated well (working on it) is immaculate and very clean "

"Spring Mill Health Campus was nice with everything they had to do. My grandmother was there two years ago and their apartments were well taken care of. The staff was nice They were great. They had bingo and card games."

"My dad just moved into Great Lakes Healthcare Center a week ago. I like it. It's pretty nice. He likes it better than where he came from. It's really a good sign that he likes it. The staff are very good. My dad likes it better because the staff is better than they were over at the other place. He has only been there a week. I see him almost every day, and he just brags about it. As long as he's happy, I'm happy. The facility is spectacular. Whatever family you recommend, they're going to like it. The rooms are big. The facility is clean and very nice. They have a two-wing section there, and that's why I really like it. Right now, my dad is in the rehab part. He'll be switching over to the nursing home part today or tomorrow. They will only allow clothes baskets. For extra space, you can bring in your own dresser and your own TV. The food is fine. They do provide cable. They have bingo games. They have other kind of activities. The activities are very, very good."

"My mom had fallen and fractured her hip, she had surgery, and so we decided to put her in Symphony of Dyer for more rehab. It was pretty good, but my mom didn't last there long. She passed away, but the short amount of time that she was there, it was satisfactory. The staff was friendly and available. My mom had to have the pureed food. The place was very nice and clean. The outside looked very nice. I liked the way it was. It was a good location because we had one sister that lived close to Dyer. It was a nice area."

"Dyer Nursing and Rehabilitation Center was a really beautiful place. We really like the place when we visited. We really wanted mom to be there. The staff were very helpful. However, they could not accommodate her because they were afraid she would just get out of the facility if she wanted to. "

"I visited The Waters of Hobart. They were very friendly. It seemed very accommodating and clean, and they could handle his medical needs. The rooms are double occupancy. So, he would have a roommate, but I think that's a good thing to have someone else with him that he can talk to. The rooms seem very nice. They have activities like bingo and some other games and gatherings that they do."

"I placed my grandfather at The South Shore Center in Gary, IN. There were many rumors and when we arrived we were pleasantly suprised. The facility was clean, friendly and professional. His stay there has been amazing. "

Kimberly was so attentive in finding the style of room my mother wanted & patiently answered all our questions & concerns regarding the current situation with COVID-19. The director, Beth, is always accessible & returns phone calls promptly. She communicates regularly with residents & family so there is never confusion about activities & policies. Maintenance has been prompt to assist my mother when called. The food quality & selection is excellent. Despite the restrictions COVID-19 has imposed on facilities, a variety of activities is still available to residents. The entire staff has an overwhelming attitude of compassion & patience for the residents they serve. It is truly a calling for them & not just a job. This gives family members peace of mind that is priceless."

"The overall impression when you first enter is very unimpressive. The facility is very untidy and has a foul odor. Patients are under supervised, based on the volume of falls that occur on a monthly basis. The staff are uninformed. They have no idea of a resident's past medical history or what needs they would need to help them. They are not trained in emergency first response to save a life until an ambulance arrives. I have experienced many instances of what I believe is negligence from staff members. "

"The staff was incredible; they really cared about my grandfather and my family. They made sure that my grandmother could spend as much time as she wanted with him, and also made sure that she had food to take her insulin with every day. The staff was very alert and aware of changes in my grandfather's condition and was proactive in treating even the slightest symptom. When he began to receive hospice care at the facility, the staff seemed just as moved emotionally as we were. The facility is also well kept and extremely clean. We were very lucky to find Tri-state, as there were not many facilities in the area that had good ratings from the state. "

"My relative is currently living at this facility and the staff is awesome! She has a learning disability, and they are so kind and caring to her. She has built great bonds with the workers and loves to be around them; they spoil her to the max!! She gets to do activities that she normally wouldn't be able to do such as bingo, nail days, and crafts. The staff make her and everyone that walks in feel so welcome. This is truly the best place for her to be at, and she loves being able to call it her home. The rooms are very nice and have plenty of space, and the facility is always kept clean, and it brings a home feeling. "
