"The Santa Rosa Health and Rehab Center is very nice. I would recommend it to anyone looking for quality senior care in a convenient location. When I toured the facility in Dec 2011, the first thing I noticed was the grounds. The lawn is well manicured and the simple landscaping appropriate. Upon entering the facility, the lobby was clean and had no foul odors. Two residents standing at the nurse's station waved to me. They appeared well groomed and were being attended by one of the CNAs. After a short wait, one of the nurses kindly directed me to the guest services areas. The tour started in the residents' room which were a little small but nice and had no showers in the bathroom, just a sink and toilet. Most all rooms accommodate two residents with the exception of the "suite" rooms. They were single rooms with private showers. The communal showers were probably the worst part of the tour. They were dark and had an unpleasant odor. The staff told me they were planning to renovate/update them very soon. The common areas were pretty and full of residents. A local church was preparing to sing. I was told that they have bingo and other group activities. A hairdresser comes in every Thursday. They also provide weekend outings, a fishing trip was planned that day. The dining areas was nice but the food looked just okay, a little bland. Overall, I was pleased with the tour. The staff was nice, appeared to enjoy working there, and interacted well with the residents. The residents seemed content and safe. "

"I would give Azalea Trace a high rating but it was very expensive. They have a waiting list that is about a year and a half to two years. Their grounds are landscaped to the Nth degree. They're beautiful and really well designed. It is just amazing. It is more amazing than I can describe. A lot of their residents are very active. They have their own private golf course. Their apartments have bedrooms, kitchens and living rooms. Each one of them has a beautiful view. You have meal prepared for you but you have to go to the dining room. Their second level is for medical level. If you have a lot of medical problems, you go into it. Then there is a third level and it is for people that are mentally impaired. People who are fairly wealthy here want to go there. They're pretty exclusive. If you're very old and just wanted to retire and have people take care of you, I definitely recommend Azalea Trace if you could get in. "

"This facility is undergoing many changes. For those who don’t know the new Executive Director, stop & meet her, the door is always open!!! There’s an entire new leadership team. The Director of Nursing is amazing & is working hard to get the nursing department on track. The new Social Worker Margaret is a Godsend & Myrtle the activity director is second to none. Your loved one will never be bored…I have a family member here & I can tell you the improvement has been great the past 3 months. Even the food has gotten so much better. It takes a village to make a nursing home a home for our families and that’s what these ladies are doing!! We now have a monthly family night. Thanks to everyone at UH for their hard work. "

"I came here from the hospital and was very, very sick and most people did not think I would get better. I was very weak when I got here and could not do much. Thanks to the excellent doctor, nurses, CNA's and therapy staff I am now able to walk and going home today in really good shape. The food was good and I enjoyed the activities that they had. Therapy was great and I cannot thank each and everyone of them enough. Thank you Life Care for the great nursing care I received. "

"My wife is on the waiting list in Bayside Manor. The place seemed to be very clean and well-maintained. It was a bright sunshiny place, it has sunrooms on both sides of the building. The staff seemed to be more genuinely concerned and hands down better than the other places that I've visited. The place was a beehive of activities. They had three staff to facilitate activities to entertain the residents and they have a beautiful dog for the pet therapy. The other facilities that I've visited didn't come close to Bayside Manor. "

"I would recommend The Haven. They specialize in people that are mentally impaired. We had a lady client there who was an older woman but very active. She is the athletic type, but when she got West Nile virus, she went into The Haven. They thought she was going to die and, I mean, we did too. However, they rehabilitated her to the point now where she is actually able to walk a little bit better and it was just amazing. They're very effective and they do take care of the people in all stages of just anything wrong with them. Their staff is very attentive and caring. They don't just go and put their time in, they did more than punch the clock, which is good. Their rooms tend to be a lot smaller and a lot like hospital rooms. In terms of acvities, they have common areas in each section where people can watch TV together and so on. They really try to keep their patients engaged. "

"the nursing staff was fifty fifty qualified some nurses did not know how to handle dialysis ports. the rehability staff was the best part they know what they were doing and were able to get me onto my feet in less than a month. they were strong willed and did not take no for an answer which was what i need ed at the time. the building was divided into two sections one was a nursing home for the elderly and handicapped and the other was a live in rehab facility. the worst part was the food. "

"My mother was in skilled nursing here for 3 months due to a stroke and broken collarbone. From there she moved to the ALF for the remainder of her life. I can not put into words how wonderful the staff is. They loved and cared for my mom as if she were their own. The day before she passed, every staff member of the ALF visited her room with hugs and kisses and "i love yous". Our family was so touched and my mom smiled at everyone one of them and told them she loved them too and my mom never said anything she didn't mean! There could be no better place for my mom to have lived out her final years. She couldn't have been more well taken care of. I have gained lifetime friends (more like family) at Bay Breeze ALF who I will forever be thankful for...thankful for making my mom's life happy and safe, and for giving me peace of mind knowing how well she was being cared for by people would go above and beyond to make sure they were. "

"Are you thinking about coming to Park Place. Give it a try, I did. Let me tell you a story. I had lost the will to live. After after a year in bed , i had never walked. My family and friends convinced me to give Park place a try. For the first four months I did not want to do anything. I was not the best patient. The staff were very patient and slowly convinced me to try some of the thinks that I should be doing. I couldn't stand, so the staff lifted me into the wheelchair and back into the bed. One day they convinced me to go down to the therapy room. Oh yeah I said, but was I ever surprised!... The room was filled with people like me. They taught me things from my chair Music filled the room and the played anything the patients wanted. They served us coffee. I had a good time. I looked forward to my therapy days. Eventually, they taught me to walk I now am out of park place, but I will never forget them. The staff went beyond what was required. The housekeeping staff, kept the place so clean, that my guests noticed it. The maintenance staff came by and stopped daily to see if they could fix anything. They came by on there breaks just talk. The nurses were very caring, and asked if medications were working. The certified nurse assistants saw me every day, and I became very close with alot of them. The rehabilitation staff worked a miracle on me. They made me feel like I was the only patient they had to take care of. I can't name all the people that touched me, but you personally know who you are. I can say that the entire staff at park place are the following: A wonderful combination 😍 of professionalism and a down home feeling of tender loving care. "I visited my grandmother last night. She is terminally ill and will never see her again. The nursing staff Was wonderful with her. I could see the bond between her and Denise, she treated her so sweet. I felt releaved Knowing she was in good hands with the staff there. I worked as a nurses aid many years ago and know how Difficult it is to see the patients health decline and residence with no company. Satisfied granddaughter. "My experience with Fort Walton Rehabilitation Center so far has been very positive. The staff seems quite professional. Everything is just spic and span and great. My father said the food is very good, and he is happy with that. The common areas outside are great and includes a wonderful gazebo in the back. This will be my father's second day, so it's kind of early in the game, but as of right now, I would certainly recommend them. "
