"Notre Dame is a lovely rehab facility. The facility is basic without a lot of frills which did not truly detract from it because of the other aspects. The facility is very clean and Covid rules are strictly adhered to. The staff were wonderful and dedicated to providing their best care. My mother in her 90's was there for ~ 2+ months. When she was moved into a double room, they matched her with a wonderful roommate. This they gave a lot of thought too as to other aspects of her care. The staff were very patient and made frequent rounds checking in on all the patients. They were conscientious! The facility physicians were really great! .The NP and MD's listened to our concerns and followed through. The Director of Nursing is ready available and ready to assist with any concerns or just chat with. The sisters and priests make the facility extra special. Mass and communion are offered frequently. The Sisters sing beautifully at mass. The religious aspect adds a strength to the place. The OT , PT and the recreation department did a fabulous job! My mother made super progress working with them! This rehab facility is a caring, safe place for a family member."

"My wife was in rehab at Autumn Lake Healthcare at Norwalk. It was nice. It was a former junior high school that had been converted into a nursing home and rehabilitation. It was fairly large and had nice landscaping and plenty of facilities, restrooms, dining rooms, and things like that. They seem to be, from time to time, short-staffed because everybody is short-staffed in the United States during this COVID era, but they were very caring. My wife got to be close with some of the staff members and they give you good attention. They have very good rehabilitation, physical therapy, and that kind of stuff in-house and it's excellent. She was there for 100 days. There's quite a variety of food. Just about anything is available and it was very good. I ate the food sometimes when they offered. They had several dining areas and it was like you would imagine a grade school dining room would look like. It's a three-story facility, and they had several different dining rooms in different shapes. It had chairs and a big dining table and refrigerator. They were quite nice. Her room was kind of small. It was a room for a single person, but it had an attached bathroom and a nice television set. It's a little short on storage. They had lots of activities. They had a daily newsletter that they sent out. They have musicians in there quite frequently that will walk around and play. Sometimes in the dining rooms, they set up games for people like bingo, card games, and things like that. The facility was very nice and they were meticulous about keeping it clean. There was always a cleaning crew going around. They did a good job of keeping the bathrooms clean. They had a fitness room and they do physical therapy in there, too, but some people use it just for fitness. Value for money was good. The 100 days were paid for by Medicare, so it didn't cost us anything. It looks like if we stayed there, it gets to be fairly expensive and I don't know if it's a good value or not, but the care that she needed was more intense and they were very good at that."

"Waveny Care Center appears very nice and is kept well. There are many staff members including full time nurses throughout the night, janitorial staff, and other patient care staff. Given this, however, many of the staff, including the nurses speak with heavy accents which makes it hard for many of the elderly residents who are already hard of hearing to understand. Because of this, patient care is not as efficient as it could be. There are good aspects though. Each person staying in the facility has their own room with a bed, television, and bathroom. Having spoken to people currently staying there, they seem happy yet bored, and seemed all too eager to have a conversation with a complete stranger, me. "

"My grandfather was a long term resident of this facility. The staff treated him like he was family every single day. The administrator was amazing as well. For all of my grandfather's meals, the administrator made sure there was a side of grapes with each meal knowing how much my grandfather loved grapes. The staff kept up great communication with my entire family about how he was doing. I can't say enough great things about this facility and I truly appreciate how caring and dedicated everyone was."

"I'm so happy we were able to place my Auntie here at Courtland, she is happy with the meals, and they have plenty of activities for residents. I have visited different days and times and always been pleased with the staff, who takes very good care of Auntie. The facility is clean and welcoming to all. I thank God for all the staff."

"I chose St. Camillus Center because it's on the same street as to where I live, so I could come and visit, but some people did not do their job, while some did. They missed my husband's antibiotics and there was a time when he was incontinent, nobody came to help him. It's hard to get in touch with them or call them, too. He has diverticulitis and they gave him sausage. So, I have to cook and give him healthy food. They were disorganized and I was disappointed, but there were few people who were great, that included the people at the front desk, the housekeepers, and the physical therapists. They just had a lack of personnel. So, it's not them, it's the administration. They just didn't have enough personnel to attend to the many people there."

"My mother first went to the Carolton to recover from a broken hip. Five years later she fell and damaged the same hip again. She was bedridden. Carolton staff had to use a lift to move her. Within a few months they had her up and walking. Every time I visited, talking with her through an open window (due to Covid restrictions) she looked well cared for and happy. A few months later other family members had Mom moved to an assisted living facility that was unable to provide the level of care needed. Within one month Mom ended up in the hospital then died after a few days in a hospice. She spoke of the Carolton caretakers as warm and friendly. I wish she’d never been moved from there."

"A SNF which provides dedicated, individualized thorough care for all of its residents/patients. JHF Elderly offers a safe, clean, supportive environment for all patients.Aides, nurses, social workers, medical personnel and administrators are attentive and very responsie. "My aunt is now at Mozaic Jewish Home. It is absolutely beautiful. It's gorgeous there. It's very clean, too. The food is very good. The staff is very nice. The building is brand new. There are a lot of amenities. You can sit and eat lunch with your family. It's really nice. They have a full rehab staff there. My aunt goes down for physical therapy every week. They have a salon, people come in and do music, they do bingo, and they have shows. There's a gift shop, too. When the other place got shut down, this is where they happened to put her. Then I just put the paperwork in to keep her there because it was so nice. The weekend staff can use a little bit of help, but the staff that is there during the week is lovely. Overall, I would have no problem sending anybody here. It's more expensive than most nursing homes, but it's worth the money. There are outdoor areas to sit with your family and everything."

"I would like to live at Watermark, it's a lovely place. The services are wonderful and its building is beautiful. The staff and dining facility are nice. There are a lot of extra-curricular activities as well. "

Years ago, my grandmother was in there, and they have a fairly good reputation. The staff were pleasant, and they had a lot of activities. They have a good relationship with the community, and I would recommend them. "
