I was at Life Care during the heighth of Covid so I had to spend a lot of time in my room. Aimee literally got me walking again after a broken femur and a shattered wrist. I always ate in my room (due to Covid) and the food was amazing and always on time. Every morning, my favorite nurse, Duane, would gently wake me for morning meds. I am currently in a different assisted living because Elkhorn was full but I'm definitely getting on the assisted living list!!

The rooms are small but adequate. The food is excellent! The care staff is helpful and knowledgeable. The therapists worked so hard to prepare me to go home! I would not have been able to make it without the help and encouragement of Marissa, Kelsey, and the entire therapy staff. Stacy from the social work staff helped me a lot with anything related to insurance, future home therapy needs, etc. I must confess that Life Care Center of Elkhorn changed my concept of nursing homes (care centers). I would definitely recommend Life Care Center of Elkhorn to my family and friends!

Life Care Center of Elkhorn is providing my loved one's needs and they have added additional therapy to help with her condition. Unfortunately, her room is shared with another one, but that's all that Medicaid will pay for. Maintenance has all been fine. She hasn't had any problems with the food, especially because she is gluten-free, so she is on a special diet, but she's been fine. They accommodate her. I have not seen any activities. I think that's one of the downfalls, that there needs to be more. As far as an activity aide that is a head of that and to present that, they don't have the staffing for that, but their staff is very pleasant.

I would want the staff , both administration and nursing to have caring and compassion for my mother. I would like them to be more aware of a persons cognitive abilities and treat them respectfully. There were no activities or help to engage the residents so that their days had some fulfillment of enjoyment. The amenities that we were aware of did not come to fruition both because of covid and because i feel that that was a good excuse not to do anything.

The Therapy department was absolutely fabulous at Life Care Center, They would come and work with me sometimes twice a day to assist me with my upper and lower body. When I have needed something, the nursing staff is attentive to making sure I had what I needed. Kat and Susan were especially helpful in anticipating my needs. I have enjoyed the meals here especially after being in the hospital. When I would push my call light they were right here to assist when I needed something. If I need this kind care again I would like to come back. They take very good care of their patients. I would be disappointed if I were not able to return if needed.

The staff here at Life Care Center, friendly, helpful kind and compassionate. The Therapy was by far the best I have ever seen. Susan RN is super efficient, thoughtful and brings treats around at night. Stays in the room to make sure I have taken the medicine. The food is good and I have had to reduce what I order. I have been counting calories so I have ordered lighter plates recently. If I needed to come back this would be my first choice!

My mother transitioned from another nursing home facility to Life Care Center of Elkhorn in December 2016 and it was the best decision I could have ever made for my mom, actually she was the one that requested to go to this facility because she had heard so many wonderful things about it and she was right. Unfortunately her experience at the previous nursing home was not a good one. Carol in admissions was so helpful when we first arrived, she explained everything to us about the facility etc. They even had the perfect room-mate for my mom, they get along great and they enjoy one another's company even to the point of eating together and our two families get along well. The other things that we love about this facility is the personalized care not just from the Administrative Staff but also the Nursing Staff, Housekeeping, and Physical Therapy which has really worked so patiently and dilligently with my mom to the point of helping her to build up her strength back up to be able to start walking again after having the stroke and she enjoys all the therapists but really enjoys getting her Awards after walking so many feet. The food options are excellent and if we want to go eat with mom they even have a private dining room available. Everyone really seems to care about my mom and it shows through the care they provide and also because my mom feels the same about all of them, (you see my mom has a pretty good mind and she will definitely let you know if something is wrong) she loves being there.. The last and other most important thing that I would say I love about this facility is the cleanliness, not once in the 3 months that my mom has been there have I nor any of our other family members or friends noticed any terrible odors or uncleanliness as a matter of fact it is always in tip top shape.. I would recommend the Life Care Center of Elkhorn to any family member that is looking for a loving and caring facillty for their loved one, it gets 5 stars from me and my family.

My mom was discharged here for rehab after a hospital stay. Carol in admissions was extremely helpful and welcoming. One of the charge nurses, Judy, took time to talk and laugh with my mom making her feel like more than just a patient. Mom appreciated the food options and felt it all tasted good. Therapy was super helpful and built mom's strength and brought her independence back. Nina, social work, made sure everything was in place for a smooth process when mom was allowed to go home. My mom has been on a medical roller coaster this past year. Life Care Center of Elkhorn is the only place where she said leaving was bittersweet. She appreciated the staff and made friends with some of the residents. We are thankful for the care and services provided to my mom at Life Care Center of Elkhorn.

When I came in I requested a private room, because I get stressed easily, I like quiet and calm. I prefer my personal space and I was very happy with that arrangement. Having been here before and having a good experience. Staff has been very sensitive from the admissions, explained everything. I was concerned about my medications, because I had not had them adjusted since my weight had changed. The therapy department discussed the medications and the relation to my balance. Nursing has been so nice and accommodating especially the charge Nurses. The really went over and above with empathy and compassion. The CNA's have also done a very good job managing my care. I feel that the food is good. I have chosen to eat in my room, again as I stress easily. The Therapy and Nursing have been wonderful. Always asking if there is anything else that I need. The attitude of all the staff has been pleasant all the time. When I need to use my call light the wait has been minimal, even over night. I would come back if I needed to! I would highly recommend Life Care Center of Elkhorn to anyone needing Rehabilitation or Long Term Care.

I want to thank all the staff for being professional in demeanor, supportive, and caring for mother, Virginia, during her stay at your fine center. This covers the care staff, care aides, nurses, therapy members, etc. and also the professionalism of your business staff and the phone operators. Mother really enjoyed your large Christmas Tree by the dining room and the Christmas Celebrations. My last picture of her are by this beautiful Christmas Tree. Thank you so much for the nice memories. When confronted by Susan ( APRN) and the therapy staff about the possible end of Medicare days and taking mother off Medicare, I knew that it was likely that this could be near the end but expected mother to linger for some time. This also was a job well done. When she became less aware, I predicted about 3-4 days more. She rested comfortably with no pain on her final day. For this, I am most grateful. Also, when I requested mother to have the last rights, your staff immediately called St. Patrick's Catholic Church. Father John Norman came over and gave mother the most beautiful last rites ceremony that I have ever heard. Last but not least was that mother had a private room as this was her wish. I personally loved all the staff here were very helpful. Especially the CNA's who helped me a lot and they were so friendly, they made me feel at home. The food was good and always what I needed very healthy. Therapy was something I didn't want to go everyday, but I went and tried. Through my therapy I know I have improved. The therapy staff knew what I needed and made it easy for me to be successful. All of the therapist had great attitudes. We had the right equipment when we needed it. I have told others this community is great and would highly recommend it. I felt "healing" here!!!

The staff at Life Care Center of Elkhorn are so nice, caring, friendly and compassionate. When I first arrived I did not know what to expect. I had great fear of lying down but a Physical Therapist encouraged me to try as they could help me. The therapists pushed me to work hard and have now become my friends. They came to my home to make sure it was safe for me to return when I could. Because of them I am able to get out of bed with out horrible pain. Dining staff was very helpful and would assist with getting what we wanted or needed. Nursing was helpful, kind and attentive. Everyone truly were very kind. I have had a great time visiting with other residents during meals as well! If I would have to do this again I would choose to come back to Life Care of Elkhorn.

This is a terrible place. My grandmother's stuff has been stolen almost on a daily basis. Nothing gets done about it. They tell her she is not able to get up to go to the restroom, but when she puts on her light, they take 30 min or so to come. Which then, she has already soiled herself and tells me that they then get upset with her. She was left in the bath tub for over an hour one day and then left on the toilet for over 20min another time. She said she was "being punished" because they washed her hair when they weren't suppose to and my mother had confronted them. The nurse or CNA came in and said "well thanks a lot for getting me in trouble!" They have forgotten to give her insulin on numerous occasions, which she definitely needs.... One of the physical therapists was very pushy with her, demanding she moved faster and that she stand up. When my mother went to a nurse about this, they said it probably didn't happen and that the people there are confused and don't know what they're talking about. Just talking to my grandmother now, I can tell she is deeply depressed. It kills me that people can get away with treating other human beings like this. One day, they will be old and I can only hope they are treated the same way they treat these poor residents.

I have visited family members at Life Care Center of Elkhorn many many times. Each time I am greeted by friendly caring staff. My family members needs are always meet. They are always clean, hair combed, clothes look nice. I have eaten there several times, found the food to be very tastee. Yes, staff is busy but you can see they all truly care of the residents. I can't say enough wonderful things about the Therapy Dept. They are specially trained to assist residents with Parkinson. I call them the Parkinson experts!! GREAT place

After stay in acute rehab at a local hospital my dad needed to have skilled nursing rehab to finish is rehab before moving to assisted living. The moment we arrived my dad and family were treated very well. It is a scary situation when time comes and you need to place a loved one into a facility for awhile. My dad is continuing PT OT and speech Therapy and the therapist that he has at Life Care Center are top notch. They are very professional, kind and treat my dad so well. He looks forward to his time daily with them. They motivate him to continuing to progress to our goal of assisted living. The nursing care with the nurses and CNA's is very good. Very thorough, kind and treat my dad well. They are busy but do a great job. You can see the kindness shown to other resident as well and make the residents feel comfortable. Family members have eaten with my dad and all found the food good and they have a variety of meals. My mother plans to have hip replacement surgery in the next month and we will be using them for her rehab as well. Our goal is to have my folks in assisted living after rehab. Thanks to the staff at all levels at Life care Center of Elkhorn.for giving my dad a better quality in his life.He is very relaxed there and receives positive support from the Doctor, PA's Nurse practioners, Nurses, CNA's and Therapist. I have been to many nursing homes to visit other family, relation and friends and find this one of the better ones

Such a shame to see a reputable company who once held high standards fall to such subpar level of care for its employees and patients. Looking like a county run facility with lack of direction, management, staff and cleanliness. We have seen first hand while there with Mother, Nurses and Aides trying desperately to keep their heads above water in attempt to care for patients get held from their duties dealing with disgruntled family members over facility issues. The staff are in tears and turn over is high. Considering placing your own loved one like we did? I highly suggest visiting in evening around 7pm and on the weekends that's when you will see the real deal and how long it takes to answer call lights because of lack of staff. Don't make the mistake we did.

My mom was at this facility for a short time. We had her moved because of the following. Within 24 hrs they had put my mom in diapers because they can't answer their call lights in time. My mom does not wear diapers, so as you can imagine it is very demeaning. My mom told me that the aides would come in and turn off her call light and tell her that she was not their patient, and would send her aide. Or they would turn off the call light and tell her it was not their area, and they would send someone. So in other words, for inspection purposes it will look like call lights are answered in a timely manner, when they are only being turned off! When I visited, it was 2:30 in the afternoon and my mom had warm water in her cup. I asked the med aide in the hall where I could get ice. She didn't offer to get any or have anyone do it for me. She directed me across the bldg to an ice machine. Then when I got to it wasn't working. I then went to the nurses station to ask for help, and there was ice and water right behind their station, which was right down the hall from my mom's room! When we first entered this facility to visit we were a bit lost, and nobody asked if they could help us, nobody smiled or even said hello. Upon leaving, we passed nursing staff in the hall, none of them could even smile. This facility was very disappointing, and we removed my mom within a week of her arrival.

I spent a lot of time at this facility. Very disappointing. Luckily I was able to be the advocate for my mother. No real "care plan" was ever addressed. Does anyone know if bath aide truly gives resident a bath? Are nurses too busy to answer call lights when CNA's in another room? Are medicines given out when admitted reviewed at any time? Turn over rate seemed high to me. 5 employees left in our 9 weeks period. Food was less than desirable and served cold at times. Eyes rolled at me when offered a concern. Shift 1 did not know how to answer question from shift 3, general response, "I wasn't on that shift". Not having computers to assemble all info made it hard for nurses to know goings on from other shifts. I would not recommend this facility.

The main complaint about Life Care Center was the food wasn't really good. It wasn't kept warm enough, and they didn't have enough servers to serve the people. They got the food over from the main kitchen, which was in the same building, and there's only one person putting the food on the plates. So if you were toward the end of the line, your food wasn't warm anymore. I think the physical therapy part was good. Mom did seem to like the people. I think the dismissal stuff was a little bit flawed because we were told that she would be dismissed on a certain day. We were in a rush to get her ready, and then they said they didn't have the form that she was going to be dismissed. I had spoken to them the day before, and they were able to get all the signatures on there, and we just assumed that was what was going to happen. I would have liked to have known more about what drugs they had Mom on, what was transferred over from the hospital, and later I found out all these things that she was on that there was no reason for her to be on, and I didn't know enough to inquire about that.

The nurses were very caring but overworked. Half of the CNAs were very attentive, half getting a paycheck. There were often long waits in response to a call light. Resident in wheelchair taken to meals early and had to wait to be returned to room. Activities director was good, hands on and took a personal interest in the resident.
