StoryPoint Forest Hills is a pet-friendly community, and we understand that our residents’ pets are family. Dogs and cats are welcome; however, they must remain inside the resident’s personal apartment. Unfortunately, not everyone enjoys having pets in their lives and we must respect their rights as well as those of our pet owners. All pets must be leashed when outside the resident’s apartment. We request that each resident and/or a family member ensure the care of the pet. Pets are not allowed in the dining rooms at any time.

StoryPoint Forest Hills is designed with a fully integrated alarm system that includes smoke detectors in all resident apartments & throughout the community. Our community is fully-sprinklered ensuring even greater safety for residents. Residents are provided with an emergency call system that is monitored in a central location & alerts staff when a resident needs assistance. All entry & exit doors are alarmed & provide a greater measure of security. Our front entrance is locked after 7 p.m. & requires a code & staff assistance to enter after hours. Guest, residents & vendors will sign in & out of the building with a program called Accushield. We are also monitored 24/7 via security video cameras to ensure the safest possible setting for each of our residents. In addition, we offer an incredible technology that is equipped in each room called Foresite, which is a predictive analytic system that monitors for falls & other diagnose such as CHF, COPD, UTIs & even pneumonia.

StoryPoint Forest Hills provides complimentary transportation via our wheelchair lift van to and from medical appointments. We also use our van to take our residents on outings to local attractions, shopping, restaurants, and entertainment venues.

We have a well-rounded social and activities program that includes outings to area state parks, local museums and shopping. Local author book readings, pet visits, spiritual and cultural programs that enhance daily lives. Our community is located near the many colleges and university’s including, Grand Valley State University, Calvin College and Aquinas and benefits for the dynamic interaction with students from all over the world. We also have the Forest Hills Fine Art Center, aquatic and senior center right in our back yard as well as the YMCA – Mary Free Bed campus just a few miles away.

StoryPoint Forest Hills is located in an incredible area, right next to the Forest Hills Fine Arts Center, reflecting the communities spirited dedication to the arts. Behind out community lies our senior center, which provides forums, presentations and luncheons all within walking distance. In addition, we have many shops and amazing restaurants just adjacent to our community. There are also paved trails that go around the local community surrounding us.

StoryPoint Forest Hills has a diverse and dynamic resident population. Residents come from all walks of life from geologists to teachers to bankers and nurses, professors to housewives and military spouses. These diverse backgrounds give our community a rich and robust group of people who quickly become friends and find common ground. Residents range in age from 55 and older. Favorite activities include shopping, outings to surrounding parks and attractions. Our residents enjoy gathering for piano music from guests who frequently come in to play our baby grand piano in our front living room.

Our Executive Chef has years of food and culinary expertise in fine dining and restaurants. He brings creativity and flavor to our menus and adds special touches and dishes that may be new to our resident’s palates. Our restaurant style meal service is delivered in our beautiful dining room that looks out over a patio and green space. We offer a featured meal each day for all the meals throughout the day, as well as an extensive ala carte menu. Ala carte selections are prepared to order at each meal.

This plan is reviewed on regular basis & as resident’s needs or preferences change & update to reflect changes. We encourage our families to ask questions, visit with our leadership team & staff whenever possible to ensure we are sharing information effectively. Family members are invited to our Family Council meeting where they can communicate with the community leaders & share their experiences with other family members.

StoryPoint Forest Hills is committed to ensuring that our caregivers are provided with ongoing education and training. Upon becoming a member of our team, each staff member is provided with an extensive orientation process that includes both State required training and additional training required by our community. Both new hire training and our ongoing continuing education programs include topics related to Dementia, Fire Safety and Emergency Preparedness, Resident Safety, Confidentiality and Safeguarding of resident information, Safe Medication Administration Techniques, and Customer Service. Training is provided for staff through in-person in-services and on-line continuing education.

Beginning with orientation, our team is provided training on effective approaches, understanding behaviors, and how to communicate with dementia residents. Our ongoing continuing education program includes extensive training for providing the best services for our Memory Care residents. We utilize resources and training techniques that are recognized by the Alzheimer’s Association and other organizations dedicated to providing care and services for those with dementia diagnoses.

Resident safety and security are a priority for the StoryPoint Forest Hills team. Safety measures include Sign-in and Sign-out logs via Accushield at the front entry. All entry and exit doors are alarmed and those alarms are linked to staff pagers. Video security cameras are placed throughout the community and monitored for activity. The front entry doors are locked from 7 p.m.- 8 a.m. daily to provide an additional measure of safety and security. All other doors always remain locked from the outside and require a key code or staff assistance to gain entrance. Residents with a known history of wandering are referred to the care of our Memory Care Neighborhood.

Our regulations dictate the number staff required for a memory care specialty neighborhood. StoryPoint Forest Hills ensures that we meet or exceed all required staffing for our Memory Care Neighborhood.

StoryPoint Forest Hills Memory Care Neighborhood is designed to engage residents and encourage interaction between residents and caregivers. The neighborhood is intentionally designed to provide “activity” stations that appeal to residents who are may wander or have repetitive behaviors. The neighborhood offers an activities table with puzzles, memory games, adult coloring and painting supplies that residents have access to around the clock. The space is open and conducive to those who enjoy walking and offers activities stations for sorting and folding, sensory stimulation, organizing and tactile stimulation. Reminiscing activities often feature a topic and encourage residents to share their experiences from a past time in their lives to stimulate long-term memory.

StoryPoint Forest Hills is in a mix of both a residential area and a business district in a small community of approximately 26,000. The community is surrounded by wooded walking trail and several parks are located nearby as well as shops and restaurants. In the surrounding city setting, Grand Rapids is home to Grand Valley State University and many other colleges and universities and offers many opportunities for our residents to interact with college students and benefits from the cultural and social activities afforded by a college campus. They also offer an array of arts, entertainment and a ‘foodies’ paradise. Our location on the edge of town offers residents a quiet setting along with the hustle and bustle of a big city just miles away.

StoryPoint Forest Hills provides each Memory Care Resident with choices & options for meals. Working with families, we identify “likes” & “dislikes” related to food choices & ensure each resident receives nutritious, well-balanced meals that appeal to the resident’s tastes. Caregivers take orders for each meal & residents join each other in a family-style setting in the dining area of the neighborhood to share the meal. To assist our dementia residents in making their meal choices, our caregivers often use our picture menus to help them decide what they want to eat. Caregivers often share the meal with residents providing a modeling opportunity that encourages residents to eat. As residents progress through the varying stages of dementia, caregivers may request certain types of foods, for example, finger foods or sweet-tasting items that appeal to the resident. To encourage residents to eat, our neighborhood Bistro is set up daily to offer drinks and snacks.

In addition to our trained caregivers, StoryPoint Forest Hills director of nursing and wellness manages the assessment of each resident using monthly reviews, regularly scheduled individual service plan updates and ongoing communication with staff and families. Our team establishes a baseline cognitive level for each Memory Care Neighborhood resident during the admission assessment process. Residents are formally re-assessed and their individual plans are established upon admission and updated every six months or when changes are observed by staff that require a new plan to ensure resident needs are being addressed.

Is StoryPoint Forest Hills affiliated with a hospital if more care is needed?

Accepted Answer

StoryPoint Forest Hills is not directly affiliated with a hospital system. However, we are located approximately 5 miles from Spectrum Blodgett campus, 7 miles from Spectrum Butterworth campus.  We are also located within 6 miles of Mercy Health St. Mary’s campus and have access to their physicians and specialists as well. A little distance away, 18 miles, is Metro Health Hospital.  Residents may use our “house” physician, or they may choose to remain in the care of their personal primary care provider. Our clinical team has developed great communication and working relationships with all medical groups so that we may provide exceptional care for our residents.
