We are relocating my dad to Bridgetown Nursing and Rehabilitation. The first reason we chose was the proximity to where my mom was living. The second reason is they were very personable. It's a small facility, so it's going to be a lot more intimate care. It's less corporate. They were very open about the guests that were living there, like social habits because that was a concern. We want to make sure Dad can make friends when he moved there, and it seems like they do a lot to cultivate some interaction culture there. The receptionist filled us in quite a bit while we were waiting, and then there was an administrator that walked us around and showed us the workout room and the lunchroom and that kind of stuff. She explained how to reserve time for physical therapy and that kind of stuff. They showed us options for rooms. We were originally looking at a semi-private room, so that would have been for two people. And then there's one full bath shared by two people. The room includes a phone and TV, and not all the places that we visited included those. In some places you had to supply your own. It's all-inclusive at this place. They have bingo, happy hour, movie nights, and puzzle stations. A church group comes in, so there will be church services available right there at the facility. The place is very nice like you come in through the front door and there's a garden and the doors open automatically that you can get to, so you don't even need help to get you out there if you want to go sit and read.

I just wanted to write a little review about our experience at Oak Hills. My brother went there after a hospitalization for rehabilitation and had a great expierence. The building is older but the care was wonderful. The nursing staff was very nice and responsive to his needs. The therapist were very good with getting him back to where he needed to return to his home safely!

My mother came in with cellulitis and chronic pain. Always leery of horror stories associated with nursing homes I had to make a informed decision about which one to choose. I had many clinical questions and the DON took an hour out of her day with no appointment to go through step by step what will be done and promise she will make sure my mother would be treated as well as one of her own. This place treats everyone like a family member and not just a number. I am enternally thankful for the great staff, rehab, and most importantly the people who made my mothers experience in a traumatic time nothing short of exemplary

My uncle couldn't walk since his auto accident. I toured many Rehab places on the west side and oak Hills Nursing and Rehab was in my top 2 pick. Since he has been there he has made major improvements in his health. Rehab therapy employees are miracle workers and didn't give up on him, he's improving every day. Great nurses I might add! They truely care and that means the world to our family right now.

This place has recently undergone a management change and has really gone down hill. I have an elderly friend who has resided there for years and in the past was treated like family. Since this new management took over my friend has been treated very poorly. I feel they have neglected her because she has no family to stand up for her and look after her best interest. I am sad to see her feel so hopeless. I will never understand why people mistreat the unfortunate. All I can do is pray changes are made and this new management team are removed from the facility.

I have been to this facility twice. When they accepted me I had Humana Medicare and Oak Hills was an out of network provider. I was aware of this but the Owner\Adminstrator informed me he would waive my out of pocket cost so that I could admit there and complete my rehab. I discharged home then ended up back in the hospital and at Oak Hills again. I was promised the same situation I regards to my out of pocket expenses as they stated "we waive this all the time". A month after my discharge the same man who promised to waive the fees started calling me trying to collect the funds. It became harassing in nature as I would have never come there had I known I had to pay or they would lie to me. I'm 81 year old and on a fixed income. They have now attempted to place a lien on my home and have turned me over to collections. I wouldn't take the word of anyone who works there as they tell you what they want to hear to bill your insurance.

This facility is wonderful! The building is beautiful and the staff truly do care! My family member has been residing here for a while now and even though we have been happy with the facility, I am seeing even better improvements recently. I have even watched the staff learn a new language to better communicate with patients. The patients are very happy there and from what I've seen so are the staff. They seem to enjoy coming to work. If you're looking for a short or long term facility for a loved one they definitely need to be one to look at!

Our mother was a resident at Oak Hills Pavilion and we were very happy with the care that she received there. The staff was very kind and compassionate and did an excellent job. The therapy dept. also provided her with great care. Overall they exceeded our expectations and our family would recommend Oak Hills Nursing and Rehab.
