I visited this facility

My mother was a patient of Bella Terra in early 2025. She has now passed and I am still trying to process everything but this review has been a long time coming. My mother was a stroke victim age 62 and was sent here for rehab. Our choices were limited due to her state insurance. If you do not care at all about your loved one this is the place for you otherwise you are better off having your loved one live on the streets. I visited my mother daily here and it just kept getting worse. This place should be shut down! When you walk in there is a constant beep from smoke detectors needing new batteries. The staff does not acknowledge you whatsoever and is very quick to ignore and or be rude when you do go to the main desk. The place reeks of e coli and sadness. It is not uncommon to see elderly slumped over in a wheelchair to the point of almost being on the ground in their doorway. My mother went almost (2) weeks without a call button, sometimes did not receive her pills and when she did she was rushed and told “ I don’t have time for this! “ my mother is a stroke victim she needs a moment and a lot of the times her meds were left bedside!!! I even have the photos and documented every occurrence we had here. One of the staff ate her food in the rec area I found her tray with her name and everything! One of the days my mother had fallen off the couch more slid and I asked the first nurse I saw to help me get her up and she said I had to find her nurse that was down the hall giving meds to other patients…. Are you kidding me! Then magically the next day the halls were filled with safety posters and how to handle falls. My mother also got sick and I could tell something was way wrong and she was puking what looked like old blood and I asked the nurse to please call for transport to the ER and I heard her on the phone state my mother was puking Dr. Pepper!! This was not the case, my mother was sent to Good Sam and my husband and I came back and backed up her room and NO ONE asked who we were, what we were doing nothing at all. This place should be closed down immediately; the staff is horrible and need to be educated on conditions that their patients have and how to help them properly not just collect a paycheck. Shame on you Bella Terra people deserve better!

I visited this facility

I went to Bella Terra Lombard. It looked very outdated. It didn't smell bad, which is good, but they weren't very secure for anybody who's ambulatory. The rooms were okay, but there were two people in a room. It looked outdated, too. And I followed up several times, and they said they would review the clinicals, but they were non-responsive.

I am a friend or relative of a current/past resident

My sister is currently in a room that should have been cleaned before placement. She was in a larger room and moved for a patient who needed " isolation". I think it was because of money. The pureed food they give her has no taste, maybe that's why she doesn't eat much. I've had to go to nurse station to get them to see about her myself. They don't answer the call button right away. They really are understaffed for that size of facility. Management needs to do something before a patient dies or is injured from neglect. There are some CNA and nurses that seem to care, while there are some that have attitudes. My sister has not been there long and I thought her insurance would cover her stay there. But recently found out she will have to pay an amount out of pocket, which I'm going to look into to see if it's legitimate.

I am/was a resident of this facility

I was in Bella Terra for a month. The food was terrible. I will give a big shout out to three people in the facility - Crystal and Deloris in therapy and Deana the wound care nurse. Susannah the social worker was okay. Most of the rest of the staff don't seem to give a damn. I wouldn't recommend this facility to anyone.

I am a friend or relative of a current/past resident

My mother is in Bella Terra Lombard. My biggest complaint is that I see residents late at night hitting their call buttons, and it's taken quite a while for nurses to respond to it. I know they're short-staffed, so they get to them when they can get to them. That sometimes has been an issue. I noticed yesterday visiting my mom's, that her fingernails were dirty and that should be attended to. When I come around, I see a little more urgency because a family member is there, so there's some care there. For the most part, they're changing her and they're keeping her bed linens clean. They've taken her to the doctor a few times. They get some things checked out with their arms and legs. Medically, they seem to be keeping up with things nicely. One time, they had all the residents get out of bed and had them all in the cafeteria, and there had to be three or four residents in their wheelchairs, sitting in their own urine that was dripping on the floor. I've only seen it once like that, and I was not happy about that. Her room is clean, and the bed linens, pillows, blankets, and all that are changed. The hallways are clean, the restrooms are clean, and the facility is very clean. The staff and the nurses are friendly. I spoke to a doctor once and he was good. The CNAs were friendly and courteous. They have a couple of hospitals close by and there are a lot of shopping and restaurants nearby that they can order from. I see some of the residents doing DoorDash and having things delivered. It could use more public bathrooms, but the rooms are adequate. It is an older building and it's a little outdated in some areas, but it's adequate.

I am a friend or relative of a current/past resident

My dad is currently in the skilled nursing building of Lexington Health Care Center of Lombard. It's across the parking lot of the independent and assisted living community. He's been there a while, and he has received very good care. I think they're nice people though I think my dad's kind of a pain to deal with. They've done a good job. He started out there in nursing rehab and technically he's still there. With insurance, they wanna kick you out after a certain number of days, but he's now accepted on Medicaid. He was being provided physical therapy to help him recover after a septic infection that diminished his ability to walk. He's kinda getting help with daily living aspects. I think he's generally pretty happy there. From what I've seen, it's clean and organized, and they have a little reception room near the front door where you can have window visits on a pre-scheduled basis. My dad does not like the food there; it's pretty much all the same with other places.

Lexington Health Care Center was very nice. The staff was very pleasant. It appeared to be very clean and also smelled better than other place, but I noticed quite a few weird practices when it came to handling transmittable diseases.

I visited this facility

In my opinion this facility was an extremely unacceptable place for a person to live let alone someone that needs assistance with their daily activities. The living quarters themselves were of an acceptable standard, but the staff was completely unacceptable. Most of the staff appeared very unqualified for their position, as shown by their poor decision making skills, and some of them couldn't even speak English very well. The staff was very slow to respond to requests and on occasion forgot about residents requests altogether. On multiple occasions they changed my grandmothers medication which I had been keeping track of without contacting me or any other family member and put her at extreme risk. I was forced to monitor every move that was made in regards to my grandmas care to make sure her health and well being was protected. On multiple occasions they almost gave her medication that she was allergic to. I was extremely disappointed with the care that was provided and believe that some of the practices preformed here were not only morally lacking, but also problems that could be prosecuted by law.
