I am currently at Pleasant Lake Villa. They're OK, although they have the usual problems with help like the aides. If you're a smoker, they have a smoking area for you to go in. They offer basic amenities. They feed you lunch, dinner, and breakfast. They have activities, but very limited. Everybody complains about the food in most institutions. They have Gordon Food Service, which is a highly rated food service company. But of course, you're not always going to like every meal. They have bingo and stuff like that. For amenities, they don't have anything special about the place. It's just a place to lay your head and eat.

I JUST WANT TO SAY THIS PLACE IS HORRIBLE - PLEASE STAY FAR AWAY. THERE IS NO HELP, MOM SENT FOR PHYSICAL THERAPY FOR A WEEK, WAS UP OUT OF BED MAYBE 3X. FOOD WAS BAD, HELP WAS SCARCE. DONT BOTHER WITH THE CALL BUTTON, TAKES ABOUT AN HOUR OR 2 FOR SOMEONE TO RESPOND. MY MOTHER WAS PUT IN A DIAPER AND LEFT IN BED. NO STARS DESERVED!

Horrible place - Run don't walk away from here. If you want your parent to suffer, send them to this place. I understand the industry is understaffed, but you are not delivering the day to day care you promised in our initial meeting. You do not return calls and cancel our meetings.

I was a resident there. I have to say at first it was ok. Friendly people you know. Then when I began to voice my opinion that's when things got bad. I was treated with great hostility. Sent many emails to the administratior that were never answered. I was accused of something by an agency nurse. Do you think they came to me to talk to me about this NO! They didn't even ask me what happened nothing. Due to this a NEEDED medication was taken away. Leading me to suffer and to stop eating because of the fear of having a reaction due to my disease. So I left! I'm so so thankful I left. I am at a facility that treats me with the utmost respect, trust and help. They have since added my medication back there way it was supposed to be and the correct dose as well. Agency nursing lied to take the blame from him to me and they lost money because of it. I have found out that they have black mold in so many places throughout the building. The DON and the ADON will smile to your face and stab you in the back. The administratior didn't have the decency to return emails that I sent. All in all do not sent e your loved one here. There are so many aides who stay on their phones and during the night you will find so many sleeping but you know it's ok to them. There are a handful of aides and nurses who are good who do their with and treat you like humans. As for the DON, ADON and administrator don't trust them. I am so thankful that I left. The place I'm at is much much much better place. Better, newer, cleaner and they do everything they can to help you. The food is ten times better with many many many options and the best of snacks without having to buy your own from vending machines because they aren't any given. The pharmacy is on the ball they don't make you wait days for meds. The day I came here that evening I had all my medicine that I was on plus the medication that they had taken from me back with the correct dose not half the dose as they were at one time they giving me. So please don't send your loved ones here. A handful of good staff opposed to many good staff is a world of difference. I'm so glad I'm outta there.

If I could give this facility 0 stars, I would. Their complete and total neglect of my father after only a week in the facility caused him to spend a week at Parma General to take care of his dehydration and the UTI he got while he was there. After two days of hydration, the doctor at Parma said he was still "dry as a bone." His veins collapsed to the point that they had to use an ultrasound to find a vein. Instead of keeping my dad in his room and his bed, they wheeled him from room to room in a leather reclining chair so they could keep an eye on him so that he didn't fall out of bed (again.) But because no one actually took responsibility for him, he was not getting the food or adequate liquids he needed. He also started twitching, which was one of the signs of his dehydration level. NEVER in a million years would I have thought a facility with a supposed good reputation for therapy could do so much damage in such a short time. If you're looking for a good place for your loved one, look elsewhere. The rooms are tiny and feel like a hospital - not an assisted living facility. The food is gross - a "ground turkey burger" looked like bread crumbs on a bun. I ordered a grilled cheese for my dad and it took them an hour to bring it. When it finally did arrive, it was a bread end and a regular piece of bread.

Our family is very dissatisfied with Pleasant Lake Villa. My dad had memory problems along with the stroke that landed him in PLV assisted living. It is understaffed and they didn’t give him the help he needed with eating and personal hygiene. I couldn’t find staff to ask questions. When I tried to call them back one evening, no one on staff knew who or what room my dad was in until after numerous calls. He was very dehydrated every time we visited, and helped him get fluids back into him.

Staff does not respond to call lights. We waited over 15 minutes for a patient in distress - I had to go to station for help. They misdiagnosed and it was my assistance he be transferred to hospital. Staff seems more concerned in their cell phones than patient care. One did not even know patient history, and got perturbed at me when I told her patient needs 2 to assist. Got snotty when she responded, 'I have to get someone', and took 15 minutes to do that...after I tracked her down in another room. Family is actually staying there in shifts to monitor patients condition. NOT ACCEPTABLE.

The STNA's are very good in helping people. The nurses are fantastic especially Rhonda and Bob. I enjoy going on bus trips here and am looking forward to going to the zoo.

Pleasant Lake Villa helped my friend's father during end of life care. He was a cancer patient who needed rehabilitation and nursing care before entering hospice. They treated him with respect and dignity. They took excellent care of him and were understanding to him and his family. It is a great facility with empathetic nurses and staff. The food is also great.

Although this nursing home has potential, the overnight shift is a huge concern. My uncle complained that no one was around to help him or wanted to. He fell three times, same time frame, same place the first week of January. Preventive measures put in place after each fall were very weak. He passed away January 12, 2018, it is my belief these falls were a contributing factor. I was advised one of the falls was when he was sitting in a shower chair. That makes me wonder if he was left alone. My uncle did not want me to complain as some of the aids would have an attitude with him if he did and would ask him why he tells me everything. I am not posting because I am upset over his passing. I am posting because three falls in a week should not happen and for others to be aware they may be issues with overnight shift. There were some great nurses and aids on day and early evening shift.

I would like to thank the entire staff at pleasant lake villa, who took wonderful care of my aunt. The staff has been wonderful from nursing to housekeeping. I highly recommend reaching out to Kali in admissions she made our transition from the hospital as smooth as possible.

My mother stayed at Pleasant Lake Villa on and off. It was terrible. They ignored her. She actually thought she had died because nobody ever came in to check on her. While I was there, she rang the bell because she had to go to the bathroom, but it was like an hour before they came to help. She had a private room. They did have activities but they didn't promote them. They just let my mom lay in bed. The food was terrible too.

My father spent 2 weeks there and we had to send him back to the emergency room cause he kept getting sick over and over...the people are friendly but dont do their jobs...when my dad pushed the call light it would take 30 to 1hr. for them to respond...they let him lay in his own feces for 40 minutes...werent giving him the correct meds, cancelled traveling doctor appts. without my mothers permission, food wasn't all that good, when sending him back to the ER when they put him on the gurney the smell from the bed took my breath away from the stench from the not properly cleaning him, showers are slippery where they need to have a more of a non slip floor (dad said he almost fell in it, staff on their cell phones when dad and others need help....this is just a few complaints i could write many more...and what bothers me is they rated this place a 4 star facility???????....we will never go back here....

My mom went there for rehab after hip surgery. I checked her out less than 24 hours later. They didn't feed her during the 16 hours she was there, she had to crawl to the bathroom because no one would respond to her beep for over an hour, and they didn't give her the meds her doc prescribed her. I told my friend about this experience and he told me that a few years back his family sued this same nursing home. His mother in law had severe bed burns. I feel bad for the patients that don't have families. They are stuck there. I wouldn't let them take care of my goldfish!!!

My family member was here for a few days, food was not that good but the worst was the bedbugs in her room! We transferred her out before we all got the bedbugs.

This nursing home is average quality. Many times, the aides do not perform duties, and my family will end up assisting my relative when there should be help. The residents do not receive high quality meals, but the meal times are regular and a less than average form of institutional food service. The cleanliness in acceptable. The facility is not a good value, and my family was cheated out of a lot of money due to the nursing home wanting to deplete our family of money before placing my relative in a Medicare room. Now, my family is struggling to pay for many other expenses since the facility takes most of our income. I would not recommend this facility to anyone considering placing a relative into a nursing home.
