"Over the years we have used the NHC facility for rehab as well as long term nursing care. We have been very pleased with the staff. The Physical Therapy department has really been beneficial to my grandmothers progress. We have many family members there. With my husbands grandmother, she was there for 3 plus years and recently pass away. The staff was extremely helpful in helping us figure out end of life choices. The care, love and support we received before, during and after the death process brought us all comfort. "

"I stayed at another facility when I had my hip surgery last year and I had to wait forever for help. I picked grace healthcare of Franklin this time. I was happy that the staff are not locked up in offices so I didn't have to wait for help. I also liked that I didn't have to do all the therapy in the gym and even go to walk outside. The food was pretty good. I like when they grill hamburgers outside. "

"My mother-in-law has had a short one-week stay at Claiborne and Hughes. The care that they gave was excellent. They were very nice and very professional. It's not very good that they were undergoing extensive renovations at that time though. It's a nursing home that takes Medicaid, but it was excellent cleanliness-wise. "

"I was a former resident at NHC Place and it was fine. I was there for about three weeks and the facility is clean. They have a regular hospital room. The food was good. "

"The reason we chose Life Care Center of Centerville is because it has a dedicated dementia unit. Their patient-staff ratio is higher than what you would normally find. Mom has not complained about the food, but I have never tried it. It is located in kind of a rural area, which is the only drawback."

"Christian Care Center of Medina is the best one I have been in because I've been in three of them. Everything was wonderful except the food they cook, it wasn't good. I stayed there for two months for rehab. The rooms were nice. If you needed a private room, you would pay more, but they have two beds mostly and that's what everybody gets. They just opened up in May this year, so it's nice and clean. The nurses and staff are awesome, they take care of me. You get 3 meals a day, they bring it to your room, and they pick up the tray. Overall, it's a good home. They have a therapy room, where you can exercise, and they try to keep you going and doing something. They've got three girls that work in there and they know what they're doing. As for the cost, my insurance paid for it. I just couldn't say enough about them because they're that loyal in taking care of me. Even if the people don't come around, the staff keeps up with them. They wash their clothes and try to keep them busy mentally, and that's a lot right there. I had one girl there that I wish I could've brought home with me, we were that close as far as talking, joking, and stuff like that. They have church services and games that they play. They have a woman that tries to keep them happy, she brings magazines and books if you want some of them."

"Countryside Healthcare and Rehabilitation meets the exact needs that we were looking for, which is a nursing home that can provide round the clock monitoring of a patient and that it's Medicare funded to a certain degree. There is going to be a limited amount of out-of-pocket, but that was the main two things. Having been there myself and looking around it, it looks like a professional facility from what I've seen and talking to the social worker, it seems like a good place for my mother, and what we're looking for is really a short-term solution, because we believe Mother will be able to go back home and be able to function independently. It seems like that place met the need that we have going on right now. It's basically like your typical nursing home. They have a lot of staff that are moving around and there were some patients in their wheelchairs that were up there near the main nursing station talking amongst themselves in the wheelchairs. Other than that, there were nurses moving around from room to room and all that. It's a pretty big facility and they get three meals a day and they are taking care of them quite well, I believe. The area that I was in looked clean. I didn't see any problems with that. I didn't walk all around the different areas, but it was clean and ran well. It was very professional."

"NHC Place at The Trace is the most excellent place I've ever seen, the absolute attention to detail of their staff, their organization, and the personal care given to every resident. The facility was pristine. It's sort of the gold standard. You just can't get better than the Place at The Trace. The location was peaceful, elegant, and accessible. The staff was the kindest, most helpful, and most attentive of all the places that I went to, above and beyond. I got the chance to dine at the place and their food was most excellent and their kitchen staff was incredibly attentive to the dietary needs of all the residents. There were always options for any dietary needs and that was very impressive. They had an excellent kitchen, just unbelievable."

"My husband was in AHC Lewis County for 10 months. It was okay, but it wasn't great, and it was an average community. His room was cramped, but the staff was pretty decent, there's just not enough of them. The food was average. They need more training."

"I visited this facility looking into their rehabilitation amenities, and found much more than I had bargained for. I found that no only do they offer physical rehabilitation, but also occupational therapy and speech therapy, as well as being a short term, and long term care facility. Their staff is always pleasant, and greet every person with a smile and a word of greeting. They have daily schedules posted through out the facility of activities they have planned for the residents and their families, as well as wonderful, caring people who plan these activities. They have two wings in this facility, one for short term patients, and one for long term patients. They have a very advanced common area for all patients and their families to enjoy visits, which is equipped with the newest technology in video games for the kids, big screen tv, and all the cable channels that any one person could view. If you enjoy the outdoors, they have a beautiful garden with benches to sit and enjoy the smell of fresh flowers, and their beautiful bounty of magnolia trees. They also take the dining experience of nursing facilities to a new level with their 5 star dining hall. They have cloth table cloths, as well as cloth napkins, you drink your beverage from a glass goblet, and enjoy your meals by candle light. It is probably one of the best nursing home experiences I have ever had. And best of all, their rehabilitative, and therapy services are not limited to residents, but also available to the public. I would recommend this facility to anyone in the market for any type of rehabilitation, or extended care. "
